A contract employee was positioned near the rib, when the corner broke off and fell and struck a timber. The timber then fell and hit the employee in the left shoulder, nose, and mouth. This resulted in a bruised left shoulder and a cut to the lip requiring 4 sutures.

Employee tripped hitting EE's knee on mine floor. Employee was transported to get x-rays and they was negative. EE received a full release. Two shifts later EE went back and was referred to a specialist, on March 23rd EE was diagnosed with a torn meniscus. Due to covid-19 and furloughed we are just returning since March 20th.
condition was damp. area was 19 feet wide and 6 feet 10 inches high. EE, and 2 others, was stacking a brattice when another man threw a block into work area while EE was picking another block striking EE in the right index finger resulting in an impact laceration. EE finished the shift. Went later to the Emergency Department and received 5 sutures.
Employee was using a slate bar to remove a rock from the Right CM on 006 MMU. While pushing down on the slate bar to slide the rock off the slate bar slipped out of employee's hands causing the bar to hit employee in right eye.
Employee was lifting oil can off of maintenance ride and felt a sharp pain in lower back.

Employee was installing a conveyor motor on a shuttle car. While aligning the motor, the chain ratchet slipped causing motor to hit frame of shuttle car and roll out on employees right foot. Employee was diagnosed with a fracture to the right third toe.
Employee entered track motor and was sitting down to operate when EE felt a sharp pain in lower part of left back.
Employee was prying down on the top belt trying to get belt back inside skirting. The pry bar slipped and struck employee on right side of face causing a laceration. Employee received 2 sutures.

Employee was on the outby end of a track rail preparing to flip rail over with slate bar. Co-worker was on the inby end of the same rail attempting to slide the rail outby with a slate bar. When co-worker picked up the rail it began to curl causing injured EE's slate bar to strike EE on the left side of the head.

Rock broke into two pieces, measurements - 27" long x 24" wide x 5" thick & 20" wide x 14" wide x 3" thick. Incident occurred along 2nd Northwest Mains between the #2 & #3 entries at spad numbers 5237 & 5250.
Employee was installing a supply insert to connect a sled to track motor, when guiding insert into holes, the insert kicked and pinched employee's right hand between insert and metal on motor. Employee received 5 sutures.

The continuous miner operator stepped across the (MRS)Mobile Roof Support trailing cable just as the MRS operator activated the cable reel and the trailing cable knocked the miner operator off his feet and he fell on his right side fracturing four (4) ribs.

Employee was dragging timbers up the # 3 beltline when he slipped and twisted his right ankle. He went to Pikeville Medical Center and received x-rays no broken bones and was diagnosed with pulled tendon in his right ankle, is now waiting to receive MRI.
Roof Bolter was getting ready to bolt #4 left this was the 1st cut out of the break. Co-worker was using the inch tram to set up for a corner bolt when he activated the tram he ran over employee's left foot. He was transported to Mcdowell ARH hospital where he received x-rays which were negative for broken bones, but the toe nail of the 2nd toe was torn off.
Employee was walking between the # 3 and # 4 entries when stepped on an uneven surface and rolled his right ankle. he was transported to St. Joseph Martin Hospital he received X-rays that were negative for any broken bones, he was diagnosed with a sprain and released same date
Employee was changing reel chain on shuttle car, when a piece of the coal rib fell and struck the employee in the upper back and neck area employee was complaining with back pain he was transported to ST. JOSEPH MARTIN HOSPITAL he received Cat scan which was negative and was transported to PIKEVILLE MEDICAL CENTER and received more CAT scans also negative, was released same date.
Employee was carrying 3 rib angle brackets across his shoulder when he stumbled and fell. Employee was complaining with lower back pain, he was transported to ST. Joseph Martin Hospital he was given a cat scan which was negative. He was transferred to Pikeville medical Center and had an MRI and diagnosed with a hernia to the left testicle.
Employee was setting up to install rib bolts in the # 4 heading when he got off the drill station platform on to the mine floor he lowered the platform on top of his left foot causing bruising to the top and bottom of his left foot.
Employee was operating a Fairchild scoop to clean ribs in the # 9 entry when he said the scoop bucket caught the coal rib and caused him to be bounced into the operators canopy hitting his head. employee was treated and released at ST. Joseph martin hospital and was diagnosed with neck spasms.
Employee was moving the continuous miner cable out of the haul road with the help of shuttle car operator when he said he felt pain in the back of his left leg. Employee continued to work until 9-10-2014 when he said his leg was still bothering him and he sought medical attention at Williamson ARH Hospital where he was diagnosed with a pinched nerve.
Employee had just cleaned the coal feeder with a 488 scoop and he backed into the intersection at the feeder and got off the scoop and stood by the scoop bucket when the shuttle approached the feeder to unload the shuttle car caught the edge of the scoop bucket pushing it onto the scoop operators foot causing a fracture to his left foot.
Employee was walking across the section when he stepped on some gob in the # 6 entry one crosscut inby survey station # 157 and twisted his right knee.
Employee was holding a roof bolt that was being used to drive the pins out of the cat pads on the joy continuous miner when a co-worker who was using a six pound sledge hammer to strike the roof bolt with missed the roof bolt and struck the employees right wrist causing fractures to the wrist and hand
Employee had pulled the bolter up to get the last row of bolts installed when a piece of rock fell between the two inside bolts landing on the A.T.R.S and then sliding to the left side of the A.T.R.S and striking employee on the top of the left forearm causing a laceration that required sutures.
