Employee was moving 4 schedule 40 carbon steel pipe (35lbs) into position at a receiver tank. The employee he felt pain in his stomach but he thought it was something he ate. The employee reported his injury the next day via phone (Sunday 06/23/13) and was taken to the doctor on Monday for examination on Monday 06/24/13 for examination and was diagnosed a Umbilical hernia.

Mostrando los 11While erecting structural steel components, an employee was using a sleever bar to align holes in preparation for inserting bolts. The employee was pulling the bar with both hands when it became dislodged from the connection point. The sleever bar struck the employee in the face at the nose-cheek juncture. Employee suffered a laceration and broken nose.
Employee was laying steel checkered plate at the hog tower when he felt a pop in his left arm.
A millwright was assembling a drag chain by using a crane to stretch out sections of the chain so the next section could be attached to the end. While a section of one of the drag chains was hanging from the crane the rigging came off of the hook and the load fell striking an employee.
Employee was driving rebar stake to establish danger barricade around an excavation. He picked up too high and missed the stake pinching his finger between rebar and driver.
After installing a girder beam in the ESP, all rigging was removed from the crane. The employee was attempting to remove a tag line from the girder beam. He pulled on the rope to see if it was entangled or still tied. The rope broke and the employee fell backwards 10' until his PFAS arrested his fall. During his descent his left foot impacted a walkboard fracturing his left ankle.
Insect bite, cellulitis (soft tissue infection).
Employee was cleaning an area for a weld repair. Employee right eye became irritated with what felt like a foreign object.
EE was welding and stopped to replace the welding rod, picked up his hood and something got in his left eye.
Operating front end loader in coal yard. While scooping a load of coal, tried to roll the bucket back, rear end went up in air. Coal pile gave way, rear end came down hard . The air seat came down and bottomed out. Injured his lower back.
Employee was mowing lawn around office building, grass & dust was blowing around. Later in the day he noticed something in his left eye. He washed out his eye & it felt better, later in the evening he felt something in his eye again. Dust in left eye, irritated , Foreign body left eye, Corneal abrasion. Employee has achieved M.M.I. with no functional limitations.
