Employee exited the mantrip and started walking to the face when EE stepped off of the concrete surface onto the mine floor and lost balance causing right leg to be caught in under EE's body fracturing the right ankle.

Miner was repositioning around CM machine and received a minor burn when they touched a bit lug that had just been cut off by a welding torch. Miner applied first aid and then sought medical attention on 01/05 and received medical treatment only.

Miner was using a pry bar to move a chain and the bar slipped from their hand and struck their arm, causing a contusion.

Employee was stepping out of skid steer on July 3 and caused injury to knee but did not want to seek medical treatment. On July 9, 2018, employee decided to receive medical treatment for incident and was placed on light duty.

Employee lifted a tooth for the 1800 hoe & placed it upon the shank. Employee felt immediate pain in the lower back. Employee was examined in a medical facility and diagnosed with a strained back. Employee was provided with a prescription, and discharged. Employee returned to full duty work the following morning.

Employee was cutting a piece of metal with a torch when the piece fell on one end. The opposite end moved upward and hit EE in the knee, causing a laceration. Employee was taken to American Family Care where the wound was cleaned and closed with 2 stitches. Employee was given a tetanus booster.

A roof fall occurred on MMU-001 located at spad 408 in the number 2 entry 2 feet above the anchorage point.

Employee was assisting welder when a hot piece of welding debris had gone down his waistline at his belt/lower back. Welder was above him on a ladder when the debris fell causing burns to his lower back/upper buttocks. Employee went to American Family Care where the 2nd degree burn was cleaned and dressed. The employee was given a tetanus shot and prescription antibiotics.

Employee was cutting belt to be used as guard for feeder batwing. He was cutting belt when his knife slipped and cut his hand. Employee had gloves on but was cut between thumb and index finger. He was given first aid onsite and transported to Shelby County hospital. He received three stitches.

Employee was throwing cribs to stack, and mashed his finger between blocks.

Employee was operating the Fair Child 35-AC Scoop on the haul road near the #2 belt drive. When a tire ran over a bridge board, the block flipped up, hitting his left hand and smashing his pinkie finger.

A roof fall above anchorage occurred on the main haul road between break number 2 and break number 3 on number 2 belt. The fall occurred at survey station number 2932 in the number 3 entry. Be advised that no one was injured at the time of the fall.

Injured was attempting to drag a piece of shuttle car cable across the section tailpiece when his foot slipped on loose material causing him to strike his left upper thigh against the belt structure. This resulted in an approximately two inch laceration. He was treated at the mine and taken to the hospital where the wound was closed with nine stitches

Injured was assisting in hanging an air pump from a roof bolt when the pump slipped. A sharp burr on the cut off handle cut his left hand. He went to the doctor and received five stitches.

Injured pulled the diesel mantrip underground to park in the first crosscut inby the drift. As he attempted to back into the crosscut, he shifted from forward to reverse but missed reverse putting the mantrip in neutral. The mantrip took off down-dip to #5 crosscut where it struck a steel set. Injured had a laceration to his forehead but unknown exactly what he struck.

Injured was bolting the mine roof when his drill steel became fouled in the roof. As he was attempting to remove the steel, a piece of roof (3' X 2') fell striking his head and shoulder. He finished his shift and went home.

The employee was operating a diesel powered, rubber tired, man trip on an incline underground . He was negotiating a turn which required him to back up and go forward in order to make the turn & then lost control. His legs were bruised. EMS transported him to hospital and he was released.

The employee was utilizing a utility knife to trim a piece of belt when his hand inadvertently slipped and cut his left hand. The laceration was 2.5 inches long and better than 1/4 inch deep. He received 5 sutures in order to close the wound. He was treated and released to full duty.

A roof fall occurred above the anchorage zone in the left mine return 8 breaks underground from the fan duct work .

Employee was bolting the roof in the face of the #2 entry. As he was pulling the drill steel from the drilled hole a small piece of roof rock struck him. This caused a laceration to his forearm. The wound was cleaned and he received 8 stitches.

While using a cutting torch to repair the section feeder upon completion he inadvertently put his right hand down on a piece of hot metal burning the palm of his hand.

While bolting a rock fell and hit boom and broke and a piece hit employees forearm cutting it.

While installing the last roof bolt in the last row a piece of roof rock was dislodged that struck the employee on the head, neck and left shoulder. The resulting impact caused an incision behind the left ear requiring 4 sutures and a contusion to his head, neck and left shoulder.

Employee stated he had passed out along the #1 belt conveyor. There were no witnesses. He was transported to the hospital and released with negative findings. The company treated this as an illness since there was no mechanism of injury.

While riding on a diesel powered man trip enroute to the active mining section the conveyance ran over a ledge in the roadway thus causing the employee hand which his face was resting on to strike his jaw. The impact of which fractured his tooth.

Reaching above head using a come-a-long and pulled a muscle in his back.

The injured employee was riding in a diesel man trip conveyance traveling to section when the man trip slid off of a bridge causing the vehicle to rise up abruptly fall back striking the mine floor. The resulting force caused him to feel immediate pain in his lower back. He was diagnosed with ruptured spinal disc. The injured had a previous history of back problems.

A roof fall was discovered during the on-shift examination by the section foreman . The fall was above anchorage and did not impeded ventilation. The location of the fall was at survey spad number 660 in number 4 entry.

This report is being filed as part of a msha citation that is currently being contested. So I have to file this report so msha will not shut my coal mine down . There was an weather anonomaly that deposited over 5.5 inches of rain fall that compromised or return airway after we had withdrawn the miners 2.5 hours earlier and overwhelmed our dewatering pumps.

The employee was installing a rub rail back on the continuous miner when he inadvertently smashed his right thumb resulting in a fracture to the thumb tip.

The employees was walking the belt entry into the mine at 17 break on the number 4 conveyor belt when he slipped and fell to the mine floor striking his knee and right shoulder.

The employee was attempting to remove a sprocket from the cutter head of the superior high wall miner by striking it with a 4 lb. hammer. Where upon a small piece of metal broke off of the sprocket striking the employee and becoming lodged in his left eye.

The roof bolter operator was drilling a hole when his forearm was struck with a wrench. The resulting impact caused a simple fracture of his left forearm.

Employee was loading bolts on the Fletcher RR II bolter when he bent over to pick up two 48 inch roof bolts and suffered a muscle strain to his lower back.

A rubber tired personnel carrier rolled out of the adjoining break through and struck the employee's left leg between the scoop door and the frame of the machine resulting in a fracture above and below the knee. The steepness of the grade was a contributing factor.
