Employee was loading rocks onto the thin veneer machine when lower back began to hurt and employee could not stand. Employee had previous back issues employee did not disclose before being hired to handle rock. Employee is out until employee can get a doctor to release employee to work.
A person came on site and did not let the office know they were on site. They began looking at rocks they may want to purchase and was wandering around the site unsupervised. Person was climbing on rocks away from all work areas when they fell and broke a leg. Person crawled their way to the office where they were found. We called an ambulance and had them taken to the hospital.
Employee was exiting the porta-potty and rolled EE's ankle
Employee was working behind the chopper stacking rock on pallets. While stacking rock on pallets a stone slipped out of their hands. While trying to catch the stone after it slipped out of their hands they pinched finger between the stone that slipped out of their hands and a stone that was on the pallet.
The employee was stacking rocks and felt a pain in EE's chest. EE appears to have a hernia.
Worker was reaching to pick up a box, bumped into a pallet of finished material and knocked a rock off of the pallet on to the top of their foot .

The employee was talking to a mechanic about a problem with the skid loader. Mechanic was finishing up the repair and heard a noise. The employee went to the west side of the wet saw placed left hand on the gantry travel rail resulting in hand being run over by the gantry travel gear.
Employee was moving a diesel container on the fork of a skid loader (Cat 272 C) when the container lost balance. The employee got off the skid loader and pushed back to balance the container. After that, the employee started feeling pain in the lower back.
Was placing a #8 rock in container at this time an employee was not looking and went to throw a rock at this time rock came in contact with said employee Right index finger causing a cut on top of and on the bottom of finger.
Rock on conveyer belt started to fall off the conveyer and employee reached for the rock causing EE to get EE's right index finger caught between rock and guard.
EE was helping push a rock manually with a rock bar at this time another EE started to chop a rock at this time a small rock broke off of the rock that was being chopped and came back and struck EE in the right Hand and middle and ring finger causing EE to have a small cut on EE's fingers.
worker was removing rock from a conveyor to place on a pallet. Another worker working across from IE was doing the same. one reached to grab a rock the other was going to break a rock and make smaller with a hammer. They both went for the same rock the EE with the hammer struck the left hand middle finger causing a smashed tip of finger.
Employee was working on the discharge side of the stone splitter stacking stone on pallets. Went to pick up a stone that approximately weighs 40 pounds when the stone rolled over and hit employee's right hand which resulted in a fracture of middle finger.
The worker operating the 321e wheel loader noticed that the cobra wet saw was traveling along the gantry wall before the blade had exited the rock. The worker went to hit the E stop to stop the saw, before worker could get there the blade broke into 3 pieces and separated the shroud from the saw sending it over the gantry wall striking the operator.
