The Employee was caring an electric winch onto the Raw Mill to help with the Discharge grate job. While descending the steps to the mill The Employee trip and fell on top of the mill with the winch in hand. They injured their left ankle during the fall.

Mostrando los 27While descending a portable ladder, an employee missed the last steps and landed awkwardly on the ground, resulting in a knee injury. The employee was promptly attended to and received medical evaluation.
On 5/7/2025, an employee was injured while attempting to remove an Easy Skirt from the Feed Belt of Finish Mill #4. The worker was using a pry bar to detach the skirting from its mounting plate. During the task, the bar slipped while being forced downward, and the sharp end punctured the employee's right palm. The injury required medical attention, including three stitches.
An employee sustained an injury to left thumb. The injury occurred when employee was using a pry bar to remove a rock lodged between the conveyor belt and its supporting structure.
Employee was getting out of haul truck to load production and experienced spasms in their back. Called supervisor for assistance and was taken for medical attention. Placed off work for 1 week.
Miner was working in the quarry, exited excavator. While walking back, miner tripped over a rock. Miner landed on shoulder injuring the shoulder and thumb. Miner was treated and released to work restrictions within the miner's normal work duties. On October 4th, miner had surgery to repair the shoulder injury and placed off work.
Miner struck mill liner with sledge hammer. A piece of metal chipped off the liner and struck miner in the right leg just below the knee, embedding itself approximately 2" into the leg.
A softball sized rock broke loose from the screen side opening and fell onto the miner's hand while the miner was barring into the oversize chute resulting in a broken pinky finger.
Miner had pipe on sawhorses to weld on flange. As EE was turning pipe to weld on flange, the flange fell off the pipe and dropped on the edge of EE's foot. The flange sliced through the boot and lacerated the edge of EE's foot. The miner received 6 stitches and modified work duties.
Miner was walking around the corner of the mill building, slipped on ice and caught themself awkwardly. Miner incurred knee strain and declined initial medical treatment until 11/7/23. Clinic prescribed work restrictions and physical therapy.
Employee was making a gasket with gum rubber and was cutting the pattern with a utility knife on a table in the carpenter shop. When the knife caught on the material and more pressure was applied, the blade came free and went across the glove on the left thumb. Which resulted in employee's thumb being cut requiring 3 stitches.
Miner broke right ring finger when the coal system baghouse door slammed shut, catching EE's fingers between the door and frame.
Employee was using a skid steer to clean up core sample bags. While exiting the equipment to load bags into the bucket, EE stepped on the edge of one of the sample bags and twisted ankle. The employee was placed on modified work duties.
High CO reading indicated a Coal smolder in Coal mill baghouse. CO2 and Fire Suppression required more than 30 minutes to extinguish smolder. CO & heat source identified as out at approx. noon. See Root Cause Analysis for full details.
Lowering hand rail on scissor lift and a hand was in the pinch point.
The employee was using a cordless impact gun with a hobble head and 3/4" deep socket, the socket popped off and hit employee in the mouth.
Employee was scooping a sample out of the crusher bowl and the bowl slipped and landed on EE's finger.
This incident was not reported to the company until July 7, 2020. The employee was helping lift the core drill in place when EE felt a sting in the groin area.
Employee was lifting the bag cage, while changing bags in polishing bag house.
During the startup of the kiln, we believe that the flame was lost for a short period of time and raw natural gas spread across the kiln system. When the fuel re-ignited, the blast damaged the precipitator casing causing $65,000 in repairs.
A fork lift with a barrel dumper attachment with a band and clasp around the barrel was being used to move barrels of balls. One employee reached down to unclasp the barrel before the forklift had come to a complete stop. The forklift rolled forward and the employee caught EE's finger between the barrel dumper on the forklift and another barrel on the ground.
Smashed finger between pipe and motor while removing the guard of main clinker conveyor.
An employee parked a truck underneath the muck chute to load kiln brick. The steps of the truck were even with the edge of a concrete pad with a two inch difference to the ground. Employee stepped out of the truck, foot was not squarely on the even surface. As they put full weight on that foot, it rolled to the lower surface causing a sprain to the ankle.
Cable was bound up on the CKD load out spout, the employees finger/glove was pulled into a pinch point.
An external truck driver was attempting to go up the driver platform to shut the hatches, upon attempting to climb the steps they slipped and fell on the stairs. Since this is not an employee I am not sure of the exact start time of the shift.
EE smashed EE's fingers while bricking. While using the ram to put pressure on the bricks, the tube broke and smashed EE's fingers against the shell.
Employee was attempting to weld a plate on the crusher, the plate slipped, EE tried to catch it and left a sharp pain in EE's right shoulder.
