IE was bitten by some sort of spider/insect, and EE's right leg became infected. IE reported it on April 24th and did not miss work until 04/29/2026.
IE was working on a skid steer's hydraulic system and when they stepped backwards, they tripped over a bucket they placed by them. IE hit their neck area on the metal plate of the skid steer directly behind them.
IE was stepping over belt structure and when employee went to step over structure lost footing and felt a pop in right ankle. IE was treated conservatively and didn't miss any scheduled shift until 03/31/2026 when it became an LTI.
IE was changing a Lee Tram tire in the underground shop. IE was balancing tire in order to get the skid steer underneath the tire, and the operator bumped the tire causing it to fall on IE's right leg causing a hematoma and fractured fibula.
IE was helping repair a 4' heavy-duty hose that feeds the dewatering system's #7 shaker. IE was holding the hose when it shifted, striking and pinching IE's right pinky finger. IE was taken to emergency room where it took 4 sutures to close the wound.
IE was traveling in the mine and the driver of the EDV cut a corner to close and struck a standing timber in the roadway. The IE's right forearm was struck causing a muscle strain. IE continued to work normal duties until November 13th when they missed the first scheduled shift.
IE was helping set feeder in place and had EE's foot underneath load end of feeder. IE's foot was smashed when feeder was set on the ground. IE was taken to emergency room where EE suffered two fractures on EE's left foot.
IE was walking in the belt entry and stepped awkwardly on a small rock. IE suffered a fracture on the outside of the right foot.
IE was doing normal work duties and felt pain in both shoulders. IE continued to work full duty up until December 5th and missed their first scheduled shift.
IE was moving a cable spool and felt a pain in lower left leg. IE was transported to emergency room and diagnosed with an Achilles heel tear to left leg.
A roof fall above anchorage fell at J-1 Gate Road entry #2 XC 10 and MSHA was notified
IW was shoveling belts and slipped. IE threw their shoulder backwards with shovel in it to catch themselves. When IE threw shoulder back, they heard a pop. IE was taken in for evaluation and when results came back, they suffered a rotator cuff tear on right shoulder.IE continued to work until 07/22/2025 and then went out for surgery.
IE was cutting bottom and slowly trammed miner to the right. The miner cable loop rolled up and struck IE in the back of left knee. IE continued to work until 08/15/2025 where the MRI showed they had a slight tear in their meniscus. IE chose to have surgery to repair.
IE was changing shearer bits on the Tailgate drum while kneeling on the ground. The rib sloughed off striking the employee knocking EE to the ground. IE was struck on left side of EE's body causing a bruise and when employee tried to stop EE's fall EE was cut under right armpit causing 6 sutures and fractured EE's right wrist.
IE hanging high volt cable and hit right index finger on a roof bolt plate causing a laceration. IE received two stitches due to the laceration on their finger.
IE was walking down Longwall face and tripped over relay bar landing on their hand and causing Carpel Tunnel in their wrist. IE continued worked through the carpel tunnel until June 27th, 2025, when surgery was required to fix the issue.
Employee was taking the top cover off the driver to work on the gear box. The EE was positioning themselves by the machine when they slipped on a hose, which was on the ground, and fell forward. They grabbed onto the guard of the machine, but continued to fall and contacted the ground causing a Closed Fracture to Upper Left Arm.
Employee exiting out of skid steer stepped between the bucket and mast. IE's foot got stuck, and IE went forward fracturing left ankle.
IE was doing hoist check on Elevator and was walking down stairs and slipped catching themselves. IE pulled their shoulder in the process. The incident took place on 02/20/2025 and employee never missed a regular scheduled shift until 08/04/2025. IE had shoulder surgery.
IE was setting a timber in the beltline. A piece of scale rock fell when EE was hammering in wedges striking the employee on the back of EE's head/neck area causing a laceration. IE received two staples to close the wound.
IE was assisting in moving the TG drive with 25X scoop. The metal forks slipped, and a sliver of metal stuck IE in the shin. IE removed the piece of metal and thought they would be ok. IE continued to work, and the wound became infected causing the employee to miss work starting 01/20/2025. First day missed from normal job duties was on 01/20/2025.
IE was standing on relay bar clevis when the pan was pushed pressurizing relay bar and pushing IE's foot underneath step at Head gate smashing EE's foot causing a fracture to EE's left big toe. IE continued to work EE's normal shift from 12-12-2024 up to 12-31-24 when decided needed to be cast and off work.
Employee walking down the stairs on NM #1 beltline. Employee slipped and fell landing on their left shoulder. IE continued to work normal shift until MRI was complete and determined EE would need surgery on their shoulder. IE didn't miss the first day of work until 01/17/2025 which is first day it became reportable lost time.
A roof fall above anchorage occurred on the outby side of intersection at West Mains 58XC in the #1 entry.
IE was installing rip pins in the CM section when the rib rolled striking the employee. IE suffered fracture to EE's hip and pelvis area.
High winds knocked power on surface causing power loss to the Main Hoist. Power was restored and hoist went back into operation.
Employee was learning how to lower a shield on LW tear down. The return gob side PTO line was missing staple holding hose in place. When the function was pushed, the hose came out of the PTO spraying emulsion into IE's eye.
IE was walking around belt winder and tripped over hoses connected to belt winder. EE stumbled and when EE went to catch self came down on EE's right foot awkwardly and fractured EE's metatarsal on EE's right foot.
IE slipped and fell on EE's left shoulder while rock dusting on an overcast area. The incident took place on 4-14-24 and employee continued to work through EE's scheduled workdays. Employee received an MRI where EE suffered a torn tendon in EE's left shoulder on 5-1-24.
IE was getting into underground ride and slipped hitting IE's head on the top door frame jarring IE's neck causing a stinger down the shoulder blade. The incident occurred on 7-11-2023 and employee was still able to perform the normal work duties until 3-5-24 when employee wound up having surgery to fix the issue.
IE was retrieving shearer bits to service the shearer. IE grabbed a bucket with the left hand and when IE lifted the bucket, the lid popped off spilling the shearer bits onto EE's right hand fracturing a bone on top of EE's right hand.
IE was pre-shiftng LW face when IE tripped walking off the LW face. Employee went to catch self with right arm falling on it and sustained a fractured right shoulder.
IE was setting a timber and a piece of rock fell from the roof striking employee on the back of their neck causing a laceration resulting in sutures.
Main hoist electrical disconnect wouldn't close to power up the hoist.
Employee was rolling up belt at the surface prep-plant with a belt winder. The belt winder came loose striking the employee on the shoulder and arm area.
Employee was rolling up conveyor belt at our surface location with a belt winder. The winder came loose striking the IE on top of hard hat causing laceration to the head area.
Main hoist went down due to stabilizer breaking on North cage door.
Employee was putting in a belt frame and it accidentally fell and caught finger between the frame and a bolt for the structure.
IE was taking apart dust line to unplug the line. The pressure of the host burst the line and struck employee's left elbow causing a fracture and triceps tear. Employee did not miss work until surgery scheduled on 9-1-23.
Blown fuse causing the main hoist to be down.
Lost power to surface areas causing the main hoist to be down.
Main hoist went down with low hydraulic pressure to the drum brake so brake would not release.
An unplanned roof fall above anchorage occurred at the West Mains Left Roadway 56XC at approximately 12:09PM on 7/7/2023.
IE employee was walking the warehouse yard conducting an audit on supplies needed ordered. A 1/4-inch nylon rope was sticking out from underneath a pallet and IE snagged their foot on the rope causing EE to trip and fall. The initial accident was on April 18, 2023. June 2nd employee became a reportable.
A thermal event occurred on the J-2 Longwall around Shield 125.
Employee was traveling mine entrance road after shift was over and was struckby another vehicle coming down the mine entrance road.
Employee working on a loader bucket stepped back on a rock and twisted ankle. Employee continued to work until MRI was done and Injury became reportable lost time on 04/10/2023. Employee suffered a tear in the left ankle.
IE was putting a 4" pipe together using a hammer. IE was swinging hammer towards EE's body and missed the pipe striking EE's left shin causing bruising.
IE was struck in foot by a rock during the mining cycle. The rock came over the sloughing plate. IE was taken in for X-rays and they were negative. IE was able to return to work and was waiting for MRI. The MRI was received on April 5th, 2023. MRI showed a minute fracture in the foot and on April 5th was when it became reportable/lost time.
Main hoist went down with a bad transducer and accumulator.
