An unplanned roof fall occurred on 9/19/2026 at approx. 12:10pm due to weathering of the mine roof. The fall measured approx. 16' wide by 16' long and the height is unknown. The fall occurred on Mainline 7 Track at 7-06 crosscut, in the track entry intersection. The area was posted on all sides, dangered off, and will be cleaned up and steel sets will be installed.

Employee was using a slate bar to remove a rock from the bolter station when the bar slipped and struck the employee in the upper lip. Employee received eight stitches. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was cleaning material from a fall area when a rock fell from the rib, striking the employee in the middle of the back. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

An unplanned roof fall occurred on 9/15/2026 at approx. 4:42am. The fall measured 16' wide by 50' long by 8' high. The fall occurred on Mainline 10 Track at 10-41 crosscut, in the track entry. The area was posted on all sides, dangered off, and will be cleaned up.

Employee was removing belt structure when felt a pop in shoulder while picking the structure up to carry it. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

An unplanned roof fall occurred on 09/11/2026 at approx. 9:44am. The fall measured 16' wide by 60' long by 15' high. The fall occurred on Mainline 9 Belt at 9-23 crosscut, in the belt entry. The area was posted on all sides, dangered off, and will be cleaned up.

Employee was traveling in an open-ended rail Jeep when the Jeep came in contact with a damaged, out of service overcast, causing material to fall into the Jeep. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was helping Z-Drill on the Longwall face when a rock fell from the face, striking the employee in the lower left leg resulting in a fractured fibula. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was de-trashing the section when employee bent down to pick-up trash, a rock fell striking the employee on the right arm and thumb. Employee was trying to get a winch rope unstuck when the winch tightened the rope back up causing the employees hand to become caught between the winch hook and scoop bucket. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was watching as a rock was being cut down at a roof fall. The rock was cut and when the rock landed, the roof bolt that was still inside of the rock became dislodged and ricocheted out of the rock and struck the employee that was standing approx. 15 feet away. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

An unplanned roof fall occurred on 08/27/2026 at approx. 11:50am. The fall measured 16' wide by 55' long by 5' high. The fall occurred on Mainline 12 Belt at 12-28 to 12-29 crosscuts, in the belt entry. The area was posted on all sides, dangered off, and will be cleaned up.

IE was moving belt structure off the belt. As they were tugging on the structure, a piece of rock fell from the top and struck them on the right hand. Their right ring finger was caught between the rock and the structure. Injury was not the result of a training or compliance issue.

Employee was taking a shower when the water suddenly turned hotter, the employee moved away from the shower and slipped and fell. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was standing next to the twin boom bolter when a rock fell, hitting the rock deflector then landing on the employees foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was walking by a belt winder in the supply yard when the employee tripped on a piece of curtain that was being used to cover the belt winder. Employee fell and landed on their left hand. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was standing on a ladder that was placed on top of line curtain when the employee tried to pull the line curtain from under the ladder, the employee fell from the ladder hurting lower back. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was in the process of repairing the bolt glue holder when the employee caught finger between rib strap and door. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee had scaled loose rock and then drilled bolt hole with first drill steel. Employee started to add second drill steel when rock fell from top and struck employee on top of left wrist resulting in a laceration that required sutures. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was carrying a belt roller to the beltline when the roller slipped from employees hand and struck lower right leg. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was stepping down from the miner when employee slipped and struck elbow against channel plate resulting in a laceration to the left elbow. Employee continued to work unrestricted until 08/27/2026 when employee sought treatment. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was putting in walkway bolt when rock fell from the roof and struck employee on EE's right arm resulting in a laceration that required sutures. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

An unplanned roof fall occurred on 8/11/2026 at approx. 8:55pm. The fall measured 16' wide by 58' long by 7' high. The fall occurred on Mainline 10 Track at 10-47 crosscut, in the track entry. This fall is a continuation of the previous fall at 10-47 that occurred on 8/04/2026. The area was posted on all sides, dangered off, and will be cleaned up and bolted.

Employee was checking the torque on the miner bolter when the employee pushed on the torque wrench and felt pain on the left side of the abdomen. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was in the beltline hanging belt structure chain when a piece of rock fell from between the roof screen striking the employee in the bridge of the nose. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

IE was hanging pumpable crib bags and was struck by a falling rock in the right forearm. Injury was not the result of a training or compliance issue.

Employee was hosing around the tail roller of surface belt when employee tripped on a piece of grading injuring right knee. Employee continued to work until 08/11/2026 then sought treatment. Employee was walking back to the Miner when employee slipped and reached to stop from falling and caught small finger on rib strap. Mining systems was not a factor, compliance with rules and regulations was not a factor. PPE was a factor.

An unplanned roof fall occurred on 8/01/2026 at approx. 2:25pm. The fall measured 16' wide by 12' long by 8' high. The fall occurred in 6 North 4 East at 10 crosscut, track entry. The area was posted on all sides, dangered off, and will not be cleaned up.

Employee was standing on the back of the miner getting a roof strap when the operator lowered the boom of the miner down on top of the employees foot. The miner operator did not see the employee prior to lowering the boom. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was standing on the back of miner getting a roof strap when the operator did not see the employee and lowered the boom of the miner down on top of the employee's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee injured right biceps while moving a piece of large rock. They worked unrestricted until 8/2/26, began missing work 8/3/26. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

An unplanned roof fall occurred on 7/21/2026 at 1:40am. The fall measured 16' wide by 25' long by 10' high. The fall occurred on Mainline 13 Track at 13-41 crosscut, track to belt. The area was posted on all sides, dangered off, and will not be cleaned up.

Employee was bolting off the miner when reached up to grab a roof strap and struck right elbow on the bolting station. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

IE was taking a break on tight side of belt to drink water. A rock fell and hit IE in the left knee. Injury was not the result of a training or compliance issue.

Employee was driving a mantrip when received a call on the radio. Employee looked down to answer radio when they struck their head on a low hanging bolt. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
