Low Light Conditions. Step into old probe 4-inch borehole, twisting right knee.

A wrench was struck with a hammer to loosen a nut. When employee's thumb was struck, causing a fracture.

While installing emergency stop gate the gusset caught on the flange and the assembly came in contact with employee's left wrist. Originally thought to be a contusion but after MRI it was determined to be a fracture

While using a utility knife to cut the conveyor belt, EE accidentally cut EE's thigh.

Injury to the right foot while lowering the skid steer bucket. Their right leg and foot were extended beyond the cab when they lowered the bucket catching their toes and crushing their foot.

EE was removing a pipe from a fitting with a 4ft. pipe wrench. When EE picked the wrench up to reset the pipe wrench to pull on it again when a finger got caught between the top of the wrench and the hydraulic tank frame which resulted in a laceration to right pinky, requiring stitches.

The employee was disconnecting a chain (come-along), that was connected to the feeder box that was being taken down. EE slipped off the frame EE was sitting on (approximately 3-4'). When EE landed, EE rolled their ankle over. EE ended up breaking the ankle in two places.

The Middle left finger was pinched between half-inch piece of steel and a quarter-inch piece of steel. While putting a new guard together for the HP500 crusher. The injury resulted in needing stitches.

While grinding on a skid steer bucket that was detached from the skid steer it slipped and struck EE's leg causing a long scrape. EE's first day missed from work was 2/10/2021.

The employee was laying on stomach in the screen deck, pulled the center rail toward themselves and felt a pop in left shoulder. Never reported it until 12/11/2020.

Miner was lifting one end of a 8" x 20' long piece of channel while the other end was on the ground. EE was handing the end to another miner in the man lift as to upright it and install as a conveyor leg. After lift EE felt a strain in EE's lower abdomen.

Employee was grinding a steel plant when metal fragment got in right eye.

While walking to speak to the manager, slipped on the ice and fell on left knee.

While working to free a stuck battery box lid, the lid fell on EE's left thumb causing a fracture and laceration.

While cutting steel for a staircase the torch snuffed out and when EE went to restart the torch gas entered EE's glove and ignited. Another EE heard a pop and then saw EE restart the torch and then pulling EE's glove off like something was wrong went to see what happened and saw that EE burned EE's hand.

Tripped getting out of skid steer, and leg got wedged between bucket and skid steer frame.

motor was bad on the crusher, EE locked out the breaker, EE went to shop to get a picking eye, at this point IE was gathering tools off the truck to remove the motor/guard. IE proceeded to remove the guard, IE climbed on top of the hydraulic tank/reservoir to access guard. While unbolting guard it sprung up, knocked IE off balance, causing IE to fall 10'

Grinder slipped and cut left leg just above the knee.

EE and another employee were carrying a PVC pipe. When the pipe slipped it cut EE's left index finger requiring 7 stitches. EE missed no time from work.

Changing Pitman Bearing with a hammer when a piece of the race broke off and entered left leg, had to go to the hospital to have it removed and stitches.

He was moving steal plates on the crusher, repetitive motion. Had carpal tunnel surgery and was out of work for two days.

Employee was changing a screen panel, by lifting it from its operation position at that point his left bicep ruptured.

Unloading steel deck near edge of the woods. While disconnecting the chain, a branch poked him in his right ear.

Employee was changing hydraulic hoses on dozer & struggled pulling hoses causing strain/tendonitis in his arm/elbow(left).

EE was using a weedeater with a blade attachment. It hit a rock and kicked back at him, striking his foot.

Employee lifted a 5'I-beam for support of crusher & strained his back. EE initially didn't think he was injured, later he experienced stiffing & extreme pain.

Using temp. gauge (gun) checking radiator heat on loader, opened door on loader; doing so knocked him off loader. He fell approximately 4'7" on heel of left foot.

Employee slipped and fell off loader while checking screens.

Employee was adjusting the jaw crusher w/4' rachet, rachet slipped off nut. Employee fell on top of loader bucket, hitting his leg on the gusset on top of the bucket. At the time of the accident the loader bucket was down on the ground and shut off, bucket was tilted forward w/material behind bucket so it wouldn't move.

Climbing steps on equipment, feet slipped, held on with hands & strained shoulder.

TURNING ON WATER TO THE PLANT, EE SLIPPED ON STONE & STRUCK FOREHEAD.

WHILE MAKING REPAIR ON CRUSHER EE KNEELED ON HIS RIGHT KNEE FOR SEVERAL HOURS, EVIDENTLY HE AGGRAVATED HIS KNEE AND IT BECAME INFECTED.

EMPLOYEE SLIPPED WHILE PUTTING OIL IN PRIMARY CRUSHER MOTOR.

EMPLOYEE WAS USING HAND HELD GRINDER, WHEN IT FLIPPED CUTTING BACK OF HAND.

TURNING A WATER VALVE ON, PVC PIPE BURST. THE PLASTIC STRUCK THE EMPLOYEE IN THE FACE.

WHILE CLIMBING LADDER TO A PLATFORM APROX. 6' HIGH EE SLIPPED LOST GRIP OF LADDER AND GRABBED EDGE OF PLATFORM, WITH LEFT ARM EXTENDED UPWARD.

EE WAS CHECKING BARMAC CURSHER WHICH WAS PLUGGED. WHILE CHECKING EE LOST HIS BALANCE AND FELL APPROX. 12'. THIS IS A P0ORTABLE CRUSHER WHICH DID NOT HAVE A WORK PLATFORM AROUND THE WHOLE DECK. MODIFICATIONS HAVE BEEN MAD SINCE TO RECTIFY THE PROBLEM.

EMPLOYEE WAS PREPARING A CONVEYER TO BE MOVED. CONVEYER BELTING HAD TO BE CUT. WHILE CUTTING BELT WITH A UTILITY KNIFE, KNIFE SLIPPED CUTTING EMPLOYEE'S LEFT HAND, IN WEB OF THUMB AND FOREFI NGER.

EE WAS TIGHTENING BOLTS ON TOP DECK OF A SCREEN. WHILE PULLING ON A WRENCH HE TWISTED HIS KNEE. HE ORIGINALLY DID NOT SEEK MEDICAL ATTENTION. HOWEVER DURING OUR WINTER SHUT DOWN HE HAD ARTHROS COPIC SURGERY ON HIS KNEE.

EMPLOYEE WAS CHANGING SCREEN ON A #4 SCREEN DECK ELEV. APPRXO WAIST CHEST HIGH. SCREEN WIRE CAUGHT SHIRT CAUISNG SUDDEN LUNGE FORWARD.

EE WAS CLEANING OUT UNDER PRIMARY CRUSHER (FEEDER) EE STANDING ON A BOARD WHEN IT BROKE SENDING EE 6-7 FT DOWN ON A CEMENT & GRAVEL SLAB CAUSINGINJURY TO HIS RIGHT WRIST, RIGHT ANKLE & RIGHT HEEL.

EE WAS PUTTING AWAY A 48" X 82" CRUSHING SCREEN. AS HE HELD THE SCREEN OVER HIS HEAD TO SLIDE IT ONTO A RACK, HE LOANED FORWARD AND WHEN HE DID HE FELT A POP IN HIS BACK CAUSING IMMEDIATE PAIN .

LOADER OPER. CLIMBED DOWN LADDER OFF MACHINE ANDTWISTED ANKLE ON ROCK @ GROUND LEVEL.

EMPLOYEE WAS ADJUSTING A CRUSHER. HE WAS CLOSING A COVER WHEN HE PULLED SOMETHING IN HIS RIGHT SHOULDER.

WHILE EE WAS SPLICING 6"0 RUBBER SUCTUM HOSE. HE CUT PALM OF HAND AND THUMB ON WIRE WHICH WAS LINED INSIDE HOSE FOR REINFORCMENT. 3 STITCHES WERE REQUIRED. EE RETURNED TO WORK SAME DAY.

EMPLOYEE TOLD SUPERVISOR THAT HE HAD BEEN TO SEE A DR. WHO DIAGNOSED HIM WITH CARPAL TUNNEL SYNDROME IN THE RIGHT WRIST AND THAT IT WAS PROBABLY WORK RELATED.

EMPLOYEE WITH PREVIOUS BACK PROBLEMS, ATTEMPTED TO LIFT CURSHER BLOCK WEIGHTING APPROX 100 LBS. [ANOTHER EMPLOYEE WAS ASSISTING IN THE LIFT] COMPLAINED OF BACK PAIN, WORKED THE REMAINDER OF HI S SHIFT.

DURING THE EE DAILY PREPARATION AT THE PLANT HE SLIPPED ON SOME FROST FELL TO THE GROUND AND SHUHURT HIS SHOULDER.

AT END OF SHIFT DRIVER ALLEGEDLY PULLED T ENDON IN PALM AREA OF RT HAND WHILE CLIMBING DOWN TRUCK LADDER (VEH ICLE =R35 EUCLID HAULER)

EE WAS OPERATING PAY LOADER, TURNED TO CHECK FORTRAFFIC BEHIND VEHICLE, PULLED SHOULDER MUSCLE.
