SUBJECT HAD FINISHED CLEARING JAMS IN SHAKER SCREEN. WHEN MACHINE WAS RESTARTED, A LOOSE ROCK FELL FROM THE BOX OVERHEAD AND STRUCK INJURED ON THE HEAD. HE WAS WAS WEARING SAFETY HARD HAT BUT SUSTAINED CONCUSSION AND POSSIBLE NECK SPRAIN/STRAIN.
EMPLOYEE WAS TIGHTENING BOLT OF SHAKER SCREEN WHEN HE SLIPPED AND FELL, STRIKING RIGHT KNEE AGAINST BOLT AND THE CATWALK. EE FAILED TO WEAR FALL PROTECTION AND FAILED TO ENSURE HE HAD SECURE FOOTING BEFORE BEGINNING TASK. SUFFERED SPRAINED RIGHT KNEE. NO LOST TIME, ON MODIFIED DUTY.
AFTER REBUILDING ENGINE, EMPLOYEE REMOVED SAFETY CABLE ON DUMP BED, STARTED ENGINE & MOVED IT FORWARD. INSUFFICIENT OIL IN RESEVOIR CAUSED DUMP TO SLAM DOWN BEFORE HYDRAULIC PUMP COULD BRING ENOUGH PRESSURE TO HOLD IT UP. EE REPORTED INCIDENT IMMEDIATELY, BUT DID NOT SEEK MEDICAL ATTN. UNTIL 6/13/05. EE SUFFERED A FRACTURE TO THE 12TH VERTEBRA, HE IS ON MODIFIED DUTY.
Employee was changing a hammer on the drill rig, when the hammer swung back and the employee's hand got caught between the the hammer and another piece of steel.
EMPLOYEE WAS SHOVELING DEBRIS FROM CONVEYOR WHEN HE FELT SOME DISCOMFORT IN LOWER BACK. HE CONTINUED TO WORK AND ADVISED SUPERVISOR THE NEXT DAY THAT BACK WAS SORE BUT SAID HE COULD WORK AT NORMAL DUTIES. HE ADVISED SAFETY MANAGER ON NOV 23RD THAT HE WAS EXPERIENCING SIGNIFICANT PAIN. HE WAS SENT TO MEDICAL FACILITY FOR EVALUATION NOV 24TH.
EMPLOYEE WAS CLEANING CHUTE ON WASH PLANT WITH A DIGGING IRON WHEN HE STRUCK HIS LEFT INDEX FINGER AGAINST A PIECE OF ANGLE IRON, LACERATING THE SKIN OVER THE FIRST KNUCKLE. SIX SUTURES REQUIRED TO CLOSE THE WOUND.
EMPLOYEE WAS CLEANING WINDSHIELD ON LOADER WHEN HIS FOOT SLIPPED ON MUD/GREASE. HE GRABBED THE CAB FRAME TO STOP HIS FALL AND FELT MINOR PAIN IN LOWER BACK. PAIN WORSENED DURING THE DAY AND HE WAS SENT TO THE HOSPITAL FOR EXAMINATION. LUMBAR STRAIN/SPRAIN. HE WILL BE ON MODIFIED DUTY UNTIL NEXT EVALUATION, 20 SEPT. 04. EMPLOYEE ALSO TOLD TO CLEAN VEHICLE STEP SURFACES.
EMPLOYEE WAS LIFTING A RADIATOR WHEN HE FELT PAIN IN LOWER ABDOMEN. HE OBSERVED THAT A PRE-EXISTING HERNIA HAD WORSENED. HE WAS UNSURE IF CONDITION WAS WORK-RELATED. WHEN HE TOLD H.R. MANAGER, ARRANGEMENTS WERE IMMEDIATELY MADE TO HAVE EMPLOYEE MEDICALLY EVALUATED. EMPLOYEE CLAIMS HERNIA EXISTED PRIOR TO EMPLOYMENT BUT NEVER ADVISED THE COMPANY.
Employee was climbing ladder into cab of euclid, door was open and he struck head on bottom of the door, laceating scalp.
DURING PRE-TRIP INSPECTION, EE STARTED VEHICLE AND RELEASED THE BRAKE. THE PEDAL SPRUNG BACK FORCEFULLY, STRIKING EE ON THE BOTTOM OF THE FOOT, CAUSING A SPRAINED ANKLE. THE LINKAGE TO THE BRAKE PEDAL WAS BOUND UP AND CAUSED IT TO SNAP BACK UPON RELEASE.
While un-rolling spool of 1"cable, the large end retracted back onto the spool. When the EE tried to unwind the cable again, the end sprung free & hit him in the face, knocking several teeth loose. His safety PPE protected his eyes & head from the laceration. It was discovered that 1/2 of the cable spool had been broken either during manufacturing or during shipping.
EMPLOYEE WAS WELDING AROUND AN OBSTRUCTION & FAILED TO NOTICE HIS PROGRESS WAS ENCROACHING ON EXPOSED FACE. HE RECEIVED WELDERS FLASH TO HIS EYES. HE RECEIVED EMERGENCY TREATMENT AT HOSPITAL & RETURNED TO WORK. HE WILL BE DISCIPLINED FOR NOT WEARING PPE.
EMPLOYEE WAS REPLACING GUARD ON TAIL PULLEY AFTER REMOVING STONE WHEN IT SPRANG BACK AND STRUCK HIM IN THE FACE, LEFT CHEEK, LACERATING THE SAME. GUARD WAS UNDER SPRING TENSION WHEN CLOSED TO ASSIST OPENING. THE HINGE SPRING MUST HAVE BOUND WHEN OPENED CAUSING IT TO SNAP BACK WHEN HE TOOK HIS HAND OFF THE GUARD TO RE-SECURE IT AFTER CLOSING.
EMPLOYEE WAS DISMOUNTING A SKID STEER LOADER OUTSIDE THE MAINTENANCE SHOP. SHE SLIPPED ON THE EDGE OF THE WET BUCKET SPRAINING HER RT. ANKLE. SHE INITIALLY REFUSED TREATMENT BUT AGREED TO A MEDICAL EVALUATION ON 4/15/04. X-RAYS REVEALED NO BONE DAMAGE. IT WAS RAINING AT THE TIME OF THE INCIDENT.
SECTION OF HAND RAIL ON CR PLANT HAD A STEEL BURR ON THE INSIDE EDGE. SUBJECT WAS USING THE HANDRAIL ON THIS LADDER WHILE DESCENDING FROM THE TAIL OF CONVEYOR 4. HIS HAND SLID ACROSS THE BURR, LACERATING RIGHT PALM. hE WAS NOT WEARING GLOVES AT THE TIME. SIX SUTURES APPLIED AT LOCAL HOSPITAL ER. NO LOST TIME OR MODIFIED DUTY.
WORKER DISMOUNTED LOADER TO THROW LOOSE ROCK IN CRUSHER. HE BENT AND TWISTED AT WAIST TO PICK IT UP, THEN TWISTED AT THE WAIST TO THROW THE ROCK, STRAINING/SPRAINING LOWER BACK. HE DID NOT FEEL DISCOMFORT UNTIL LATER THAT EVENING AND REPORTED IT ON TUESDAY, 6 JAN. HE WAS SENT TO CLINIC FOR TREATMENT. WORK LIMITATIONS WITHIN DUTIES AS OPERATOR. NO LOST TIME.
EE WAS OPERATING FEL ON RAMP TO ACCESS THE QUARRY SUMP WHEN THE FEL BECAME STUCK. AS THE EE GOT OFF OF THE FEL, HE SLIPPED ON THE ICE, LANDED ON HIS RT. ELBOW AND SAID THAT HE HEARD A POP.
EMPLOYEE WAS CHECKING SCREEN AT THE CRUSHER PLANT SLIPPED & FELL TO CATWALK.
EE WAS OPENING GATE AT ENTRANCE TO QUARRY. GATE WAS STICKING, EE JERKED GATE AND TWISTED HIS LEFT SHOULDER AND NECK.
EE WAS WIRE BRUSHING A MANTLE FOR 48FC TELSMITH IN PREPARATION OF ASSEMBLY. SMALL PIECE OF WIRE FROM THE BRUSH WENT UP UNDER HIS SAFETY GLASSES.
DRIVE SHAFT BEING CHANGED DRIVE SHAFT FELL OUT OF BEARING CAP & DOWN ONTO EE'S HAND
OPERATING HAUL TRUCK. TAKING DUST FROM PRIMARY TO QUARRY FLOOR. TURNED TRUCK INTO BANK (GOING TOO FAST) WHEEL HIT ROCK AND STEERING WHEEL SPUN RAPIDLY. STEERING WHEEL CAUGH LEFTY RING FINGER A ND BROKE IT.
EE WAS WORKING ON A CONVEYOR & WAS LOOKING AT THE TAIL PART OF THE CONVEYOR THE BELT WAS TURNED ON & WHEN THE BELT CLAMP CAME AROUND & HIT RETURN ROLLER GUARD CAUSING DUST TO FLY OUT & GET INT O EE'S EYE
