Employee had moved the Cannon drill that EE had been operating away from the face to park at the end of the shift. As EE exited the drill EE twisted EE's ankle resulting in an ankle sprain.

Mostrando los 45Employee was using an angle grinder at the plant during the maintenance shift. The grinder kicked back causing the employees thumb to strike a sharp piece of metal resulting in a laceration on thumb. Laceration received 20 stitches.
Employee was walking around a set of saw-horses while cleaning in shop. Employee's foot hit the bottom of a saw-horse leg causing EE to trip and fall to the concrete shop floor. Employee tried to catch self by putting left hand down as EE fell. This caused injury to wrist, hand and index finger. (Broken). Was not reported until next day.
EE was loading a flat deck company barge (ORS-722) at the upper dock. The barge was about 75% loaded when EE stepped up onto the barge that was approx. 12" higher than the dock level with right foot. Using hands EE pulled self up and right foot slipped in a small amount of sand causing right knee to hit the barge resulting in swelling and a sprain knee.
Employee was exiting the drivers side of the wall header blasting truck in the mine. EE failed to use three points of contact and slipped on the first landing catching self on the mirror by hov bar causing a strain like sensation in lower back. Advised management the following morning
Employee has had previous back injury (not work related).
Setting in a 16 foot "I" beam with come-a-longs. Beam became hung on scalper. EE pulled the beam loose with left hand. The beam swung out and pinched left index finger between the beam and a handrail. EE was wearing gloves at the time. The impact of the beam lacerated the end of the finger, resulting in stitches.
Employee hopped off an 8inch I-beam onto the floor hurt right knee
Cutting belting for skirt board when knife slipped causing cut on right hand that required 4 sutures
Employee was removing a bolt from the mine tunnel conveyor. Left ring finger against metal. Employee treated at local physician with 5 sutures and released to full duty.
Changing the swing cable on clyde crane when employee's finger was caught in the tension bolt
Employee was kneeling/squatting a lot and noticed soreness in the ball of left foot. Condition worsened with slight swelling on 2/24/2012.
Employee was moving hubs weighing approximately 85 pounds and felt pain in back.
EE and another EE were putting strut on a R-35 Euclid Truck and EE was jacking the front of truck up and the jack jumped out from under truck catching his hand between truck and jack and broke first finger and maybe second finger on left hand.
EE was helping Co-worker change a broken 5/8" cable on the winch that moves the barge back and forth on our lower dock. He was going down the steps to get onto the crane barge. He stated that he stepped off the last step onto crane barge, his right knee popped. He sat down but couldn't straighten out his right knee.
Employee was found unresponsive in loader. Employee had no pulse & was not breathing. Gave him CPR until medical personnel arrived. Employee was air lifted to hospital where he was pronounced died.
EE was cleaning screens in a PEP Screen.EE was finished cleaning and was climbing out of screen when his pant leg hooked on a piece of metal in the discharge chute, pulling his groin muscle.
Driver was standing on load stand while his truck was getting loaded by conveyor belt. When the truck was loaded, he went to get back in his truck from stand. Driver said he didn't know exactly what happened. He believes he slipped and fell, hitting his left knee on walkway of truck.
2 employees were attempting to start a 440 Case skid loader by putting a battery charger on the battery. When injured hooked the battery charger to the battery, co-worker started charger. The battery on skid loader sparked & injured turned to look at it. As soon a injured turned to look, the battery exploded in his face.
Employee was walking toward a conveyor belt to help get it running after the belt had chocked down when he slipped on some ice & fell twisting his left ankle. (There was a severe ice storm 6 days prior to this accident.)
EE was in control shack, he radioed co-worker to come back to shack when he could. Co-worker went back to see what was going on & EE was in shack. He had knot on his head and scratch on right hand. He did not know what happened. He possibly blacked out. No evidence to show what happened.
The company received a loader shipped in by truck. The cribbing boards had to be off loaded to remove loader from low boy. The injury incurred while removing cribbing.
EE was cleaning a pile of material from a catwalk, as he was shoveling, the pile collapsed and fell on EE's legs.
Injured EE and another EE were putting a Planetary gear set on the the 992-loader. The treads on the lifting eye pulled off, causing the gear set to come out, striking injured EE on the foot and knocked him under the loader.
2 EE's were putting wheel back on truck when wheel slid too fast & caught inj EE's finger between wheel & hub. Inj EE was sent to the dr, did x-rays, nothing broke, does have small cut, bruised, they released pressure behind nail and EE was sent back to work same day.
EE attempted to pick up a heavy rock to throw out of the way while cleaning out jaw bowl, when he strained his lower left abdomen causing burning in area. He did not attempt to see a doctor until May of 2007. He did work up until his surgery date.
EE was attempting to climb down the ladder & over stepped & fell to the rock floor hitting his lower back & right forearm.
River belt was choked down; they shoveled belt off and were putting shovels in chute opening to hold material back. He put shovel in chute and said his knee went out, falling onto belt skirt board with left side and knee.
EE was helping replace screen cloth in washing shaker. He picked a polyurethane screen panal up to place in shaker screen is flexible it swung around and hit him in the neck area.
EMPLOYEE FELL ASLEEP WHILE HAULING TO THE IMPACT CRUSHER UNDERGROUND AND STRUCK A PILLAR.
HE WAS LEAVING THE IMPACT CRUSHER UNDERGROUND & DID NOT LET HIS BED DOWN. THE BED HUNG INTO THE CEILING CAUSING HIM TO HIT HIS CHIN ON THE STEERING WHEEL.
WAS STEPPING UP ON CONCRETE WALL AROUND FUEL AREA & MISSED STEP AND FELL.
WAS CHANGING A PILLOW BLOCK BEARING ON CONVEYOR,EE WAS USING AN IMPACT WRENCH ON THE BOLTS WHILE CO-WORKER WAS HITTING THE BEARING HOUSING. THE HOUSING CHIPPED OFF & STRUCK EE IN THE LT. SIDE.
EE WAS HELPING TO CLEAN OUT A CRUSHER THAT HAD CHOKED. A ROCK FELL INSIDE THE CRUSHER ON HIS LEFT HAND, MASHING HIS FINGERS.
EE WAS DRIVING IN MINE & HIT A POT HOLE IN ROADWAY CAUSING HIM TO HURT HIS BACK.
WAS CUTTING CONVEYER BELT, KNIFE SLIPPED, CUT RIGHT LEG.
EE WAS HELPING BUILD A SET OF STEPS FOR OUR DOCK. THEY WERE CUTTING AND WELDING METAL-PIECE OF METAL GOT IN HIS EYE.
EE WAS HAULING FROM MUCK PILE TO CRUSHER. HE STOPPED TO MOVE A ROCK FROM ROADWAY BY HAND AND STRAINED BACK.
EE HAD FINISHED DRILLING A FACE SHOT & WAS PREPARING TO MOVE HIS EQUIPMENT. HE BENT OVER TO PICK UP A SAFETY CONE USED TO BLOCK THE HEADING & PULLED HIS BACK.
EE WAS POURING ANTIFREEZE INTO AN OPEN LINE ON AN AIR COMPRESSOR. AS HE WAS POURING IT IN, HE ACCIDENTALLY HIT A VALVE CAUSING THE ANTIFREEZE TO BLOW BACK INTO HIS FACE.
EMPLOYEE WAS POURING AIR LINE ANTI-FREEZE INTO AFITTING ON THE AIR LINE GOING TO THE AIR COMPRESSOR. TRAPPED AIR IN THE LINE CAUSED IT TO BLOW BACK INTO HIS FACE. HE WAS WEARING GLASSES.
EMPLOYEE WAS HEATING A PIECE OF METAL, A PIECE OF RUST OR SOME LIME WENT INTO EYE.
WAS HELPING REPLACE REAR SPRINGS ON 1998 CHEVY 3/4 TON PICK-UP.
EE WAS WORKING ON A 6" PUMP ON OUR LOADING DOCK HE REACHED FOR A WRENCH & CUT HIS FINGER ON A SHARP OBJECT ON THE PUMP
CONVEYOR BELTS WERE FROZEN TO GROUND. EMPLOYEES WERE CHIPPING AND DIGGING FROZEN MATERIAL FROM UNDER BELTS. OPERATOR HAD STOPPED BELT WHEN EMPLOYEE REACHED IN TO REMOVE A ROCK FROM TAIL PULL EY, OPERATOR WAS NOT PAYING ATTENTION AND STARTED BELT.
