A large rock fell off of conveyor belt and became lodged on the top of the underside return belt. Conveyor belt turned off and injured worker tried to remove rock. The rock rolled while attempting to remove the rock from the conveyor. The rock compressed left index finger against conveyor support. The result was a laceration to the left index finger.

Mostrando los 12The Injured Worker(IW)was kneeling on the ground next to a I-beam(A) that measured 15'(long) by 15"(height) and had a 6" flange. IW was welding plate onto the I-beam(A). Two workers were moving a like sized I-beam(B) next to I-beam A. I-beam B struck I-beam A causing I-beam A to fall onto IW. The impact caused a fracture to the left fibula.
Injured Worker(IW) was changing screens in vibratory screen deck. IW was laying on screen media while using a pry bar to remove screen from fastening rail. The pry bay slipped from under screen media and opposite end struck IW in the forehead.
The injured worker (IW) was moving cribbing material for a feed hopper for sand and gravel. IW had placed cribbing material into bucket of front-end loader. IW stepped backward and away from loader when EE tripped over cribbing material laying on ground nearby.
Injured worker (IW) stepped down (6") from elevated concrete slab. IW lost balance and fell to ground. IW fell on right shoulder. IW examined by co occupational doctor and found no injury. The latent nature of injury became apparent overtime. Surgery scheduled and completed 03/14/12. IW continued to work after initial fall until normally scheduled layoff.
Employee was repairing a chute liner in the gravel plant. While performing the repair work, particles from overhead fell upon the employee. A particle became embeddded in the employee's left eye.
Three employees were turning over a piece of plate steel. A miscommunication caused one employee to get his right middle finger pinched between the plate steel and the burning table.
New splice to conveyer belt was being installed splice was finished and clamp was being removed. Other employee removed bolt holding splice and released underside of clamp. Underside of clamp dropped and caused a pinching action at other end. Injured's finger was caught between clamp.
EE WAS EXTRACTING SAND & GRAVEL FROM HIGHWALL. MATERIAL HAD SLOUGHED OFF HIGHWALL AREA. A LARGE STONE ROLLED BEHIND LOADER USED FOR EXTRACTION OF MATERIAL. HE DID NOT SEE STONE & BACKED OVER IT. LOADER DROPPED TO GROUND & JARRED HIM.
INDIVIDUAL BENT OVER TO PICK UP A GREASE GUN. INDIVIDUAL FELT6 A SHARP PAIN IN LOWER BACK. INDIVIDUAL COULDN'T STRAIGHTEN BACK UP (STAND UPRIGHT) FOR ABOUT 5 MINUTES.
INDIVIDUAL WAS ASSIGNED TASK OF REMOVING SCREEN CLOTH FROM VIBRATING SCREEN DECK, WHILE ATTEMPTING TO REMOVE NUT, HE LEANED TO RIGHT FOR ADDED LEVERAGE. HE PULLED ON WRENCH WITH BOTH HANDS AN D THEN FELT A POP IN HIS SHOULDER.
EMPLOYEE WAS INVOLVED IN PLANT MAINTENANCE REPAIR. WAS REPLACING RUBBER SCREEN CLOTH IN VIBRATORY SCREEN DECK. WINDY CONDITIONS EXISTED AND CAUSED DUST TO BE AIR BORNE. DUST PARTICLES WENT INT O EYE. TREATMENT WAS IMMEDIATELY GIVEN TO FLUSH OUT EYE. SUBSEQUENT IRRITATION THE FOLLOWING DAY.
