Mostrando los 18Excavator operator was repositioning. Swung machine to operator's blind side and bucket impacted side of truck, bending mirror bracket and hand rail.
Employee was exiting water truck when employee spotted a snake in the area. Employee hurried to get back into the truck and sprained ankle while entering.
Miner was in the shop using a hand-held grinder to prep a metal surface. EE moved to another section and felt pain in lower back. EE was taken to a medical clinic, examined, given anti-inflammatory meds and released to light duty work. On June 19th EE returned to the clinic and was released to return to full duty work.
Employee set grinder down before wheel stopped. Grinder kicked back & hit finger (L index). Stitches required. Guard in place. Eye/face protection worn.
Employee was removing a tire from the rim using a tire tool. When tire broke loose from the rim the tire tool hit employee on the left cheek, approximately 2" below the eye. The result was a contusion and laceration on his cheek.
Employee was assisting in changing out a 4" check valve in the water piping to a log washer. A rope was used to aid in lowering the valve to the work platform. While lowering the valve it came loose from the rope, falling and hitting the employee on the left fore foot causing a contusion and bruising of the foot,
Assisting in replacement of hydraultc cyinder holding cylinder to prevent movement.
EE was assisting his foreman remove a damaged spray baiz when the pipe wrench being used to remove the pipe fell and hit employee above his upper lip on the left side of his mouth causing laceration just above his lip.
Gravel chute clogged. Pushing chute lever with left hand, chute cleared, lever shifted pinching finger on metal bracket.
USING PRY BAR TO ALIGN BOLT HOLES STRAINED RIGHT SHOULDER.
REMOVING SANDROCK FROM GRIZZLY. DROPPED ROCK ON RIGHT HAND.

MINER WAS CHANGING OUT WORN SCREEN PANELS ON TOP DECK OF SHAKER. WHILE LIFTING 4X5' SCREEN PANEL AND DROPPING TO GROUND BELOW, TENDON IN RIGHT BICEP TORE.
WHILE HAULING MATERIAL FROM PIT AREA TO PLANT EE APPARENTLY GOT OFF THE LEFT SHOULDER OF THE ROAD CAUSING BED OF TRUCK TO REAR THIN ONE THE CAB TO OVERTURN EE WAS WEARING SEAT BELT BACK AND NE CK SPRAIN APPARENTLY CAUSED BY TWISTING ACTION DURING OVERTURNING OF VECHILE.
EMPL ASSISTING INSTALLING BOOM POLE ON LOADER BUCKET.AS HE PULLED POLE IN PLACE IT SLIPPED HITTING EMPL LEFT ANKLE.
EMPLOYEE ASSISTING REPAIR TO GRAVEL RETAINING WALL AT PLANT. AS ANOTHER EMPLOYEE NAILED BOARD INTO PLACE A ROCK HIT I.NAME IN EYE, CUTTING HIS EYE CAUSING A CORNEAL ABRASION OF HIS RIGHT EYE.
