A repair job completed, a chain was attached to the excavator bucket. The bucket & the chain were on the ground. When the excavator was moved, the employee was standing on the chain as the bucket was swung. Employee was tripped, but did not fall.

Mostrando los 36Employee was operating a haul truck. Employee complained of headache & light-headedness. Employee was taken to ER and later diagnosed with a sinus infection & BP issues. Employee was placed off-work until Monday, July 26.
Employee was replacing skirt board on stacker belt. Wrench slipped and impacted finger. First aid completed. Employee reported Jan 20 finger was infected and went to clinic. Clinic provided antibiotic/meds; employee released to return on Jan 25.
Miner was changing parts on belt weight scale. Dropped a part and it impacted their right pinky finger.
Miner was helping move/set a wooden form in preparation for a concrete pour. EE got finger caught under the form as it was placed on the ground.
Employee was exiting cab of service truck. Turned knee when foot contacted the ground.
Using impact air wrench to remove bolt to replace gravel screen. Placed hand on nut head and wrench spun nut head, cutting finger.
Picked up five (5) gallon bucket out of company truck - pulled back muscle.
EE was training a new haul truck driver. EE was sitting in the jump seat next to door. Haul truck encountered a rough road condition and the EE bumped his shoulder against door of truck.
Employee stood up after kneeling, experienced knee pain (popped). Incident reported 8/25/14. Visit to Dr on 9/2/14, with light duty work.
Operator adjusted fan & cut finger on fan housing. Initial treatment first aid only. Soreness led to doctor visit on 9/20/13, with cleaning and medicine only. No lost time.
Lifting out scrubber roller, roller shifted to caught finger.
Working/walking near sand pump/pipe. Hit leg on lifting eye. Required 4 stitches.

EE Chipping slag off weld, got something in eye. Did not report until questioned by foreman 6/5/12. Sent to doctor for check, Received meds and returned to work. Not regular mine Employee
Leaned on pipe that had just been welded & burned spot on left lower forearm. First aid completed. Failed to inform anyone that he went to ER on 4/20/12.
Miner using a socket wrench to adjust belt. Tool slipped and miner struck elbow on frame. Miner did not want to go to the doctor on date of incident. Elbow soreness continued & he went to the doctor on March 3, 2011. Medication prescribed. No lost time or restriction.
EE was trimming belt wiper. Knife slipped and cut his left thigh.
EE was lifting plate to begin repair felt pull in his back.
Employee fell exiting the excavator.
Employee put left hand on cable to remove slack. He was holding control in right hand. Employee pressed incorrect button & finger was pulled in sheave. Returned to work (light duty) on 11/9/09.
Moving cutting edge in storage area. Paint caused two (2) new edges to stick together & lifted both - straining shoulder.
Counterweight was being removed with power purge. Piece broke off counterweight to struck foreman in back.
Employee climbed over framework (I-beam) between frame and belt to repair belt clamp. After repair, employee bruised rib area while exiting.
Used pocket knife to cut cable tie. When knife passed through cable tie, he stabbed left hand.
Cleaning material chute with punch bar. Hit finger on side of chute.
EE was shoveling material under tunnel belt, strained shoulder. Reported injury to foreman on 3/29/07 in the PM. Doctor vistit on 4/2/07.
Backing dozer in stripping area, backed over a pile & jarred his back on the landing; aggravated an old back injury.

HE WAS HELPING SET A MUD PUMP INTO THE POND AND THE CHAIN LINK BROKE. THE BROKEN LINK FLEW ABOUT 20' AND HIT HIM IN THE FACE.
USING A WRENCH TO TIGHTEN A BOLT ON THE SAND PUMP.
USING A SHOVEL TO DIG MUG FROM A DRAIN PIPE.
HE WAS CUTTING A PIECE OF BELT WHEN THE KNIFE HE WAS USING SLIPPED AND CUT HIS LEG.
EE WAS WORKING TO ASSEMBLE/SETUP CRUSHER SCREENING UNIT. HE FELL/STEPPED INTO A HOLE, BRUISING ELBOW.
