Employee was drilling holes in the granite to prepare for blasting. He was ending work for the day. The bit came loose from the drill. EE automatically grabbed for the bit and got his ring finger, left hand pinched between the bit and the other steel. Part of the bone on finger was damaged.

Employee was loading loose aggregate into the crusher. It was very windy, blowing small particles in his face & eyes causing eye irritation.

The crew was changing a bearing on the sand stacker. The come-along was being used it snapped and hit employees hand, took a chunk out of index finger on left hand.

Two employees were repositioning a conveyor. One was on the loader which was pulling the conveyor. The victim was directing and somehow got caught under the conveyor tire, crushing him.

EE WAS OPERATING THE CRUSHER. HE OBSERVED SOME SAND BUILD UP ON THE ROLLER. HE TOOK A HAMMER TO KNOCK IT OFF AND THE HAMMER SLIPPED OUT OF HIS HAND, HE AUTOMATICALLY GRABBED FOR IT, CAUSING HIS HAND TO GO BETWEEN RUNNING ROLLER AND CONVEYER BELT.

EE WAS CHANGING A FUSE-FORGETTING HE HAD NOT TURNED OFF THE LEECTRICITY. HE PUT HIS FINGER IN THE FUSE HOLE AND GOT ZAPPED WITH 440 VOLTS OF ELECTRICITY.

EE WAS SETTING UP THE JAW CRUSHER. HE PINCHED HIS RING FINGER OF LT. HAND BETWEEN 2 PIECES OF WOOD BLOCKING.

THE CONE GOT PLUGGED WITH ROCK & DIRT (FROZEN). EE USED A PRY BAR TO UNPLUG IT. THE BAR SNAPPED, SPRINGING BACK AND HITTING HIS RIGHT HAND BETWEEN THUMB AND POINTER FINGER.

THE EMPLOYEE WAS LIFTING SOME WOOD BLOCKING AND TWISTED HIS LOWER BACK.

THE EMPLOYEE WAS CHANGING A SHIELD ON THE CONE CRUSHER THE SHIELD SLIPPED & PINCHED EMPLOYEES RT INDEX FINGER NAIL GOT CRUSHED & BONE HAD AN OPEN FRACTURE SPLINT PUT ON FINGER

EMPLOYEE WAS CLIMBING THE LADDER ON THE SCREENING PLANT. HE TURNED TO LOOK @ SOMETHING & TWISTED HIS BACK. HE HAS A SHARP PAIN IN HIS LEFT LEG WHEN HE WALKS OR STEPS ON IT.

REACHING ACROSS SCREEN PLANT AND STRAINED MUSCLE IN BACK REACHING FOR ROOT.

GRAVEL BANK CAVED ON LOADER.
