Removing a flat tire from a haul truck. The tire moved and caught employee's finger.
Hydraulic hose broke in engine compartment of Ed's Drilling & Blasting rock drill. The resulting fire destroyed the engine compartment and operator's cab. The operator off the drill used fire ext. and then evacuated the area. No injuries occurred during this event. No quarry employees were involved.
Operator was assisting driver with tarp. Operator slipped off side of truck and injured wrist. It was very windy and caused the initial problem with the tarp.
A rock was stuck in the jaw crusher. EE entered the crusher to break the rock free. The area couldn't be reached with the breaker arm. As the rock broke free it rolled on another rock & struck his knee. He was able to get out & just complained of a bruised knee. After tool clean up the supervisor asked to see the knee & recommended he go to the hospital to check it out.
EE was driving truck too fast downhill into pit. EE did not utilize a lower gear, retarder or emergency brake. Left side tires clipped the berm and caused the bed to overturn. The cab did not overturn. As a precaution we had a supervisor drive the operator to the hospital where minor bruises were found and the operator was allowed to return to work that day.
Operating a highlift(Loader) caused carpal tunnel.
EE WAS ATTEMPTING TO LIFT A DRIVE SHIAFT ON DUMP TRUCK WHILE BED WAS INT HE AIR. THE BED FELL ON EE PINNING HIM BETWEEN THE BED AND FRAME OF DUMP BED.
EMPLOYEE WAS INSTALING A NEW IMPELLER IN A DREDGE PUMP. WHILE THE IMPELLER WAS BEING LOWERED INTO THE DREDGE THE LIFTING STRAP SLIPPED OFF THE IMPELLER TO FALL ON THE EMPLOYEE'S RIGHT ANKLE AN D STOMACH AREA.
EMPLOYEE WAS CUTTING LIMB OUT OF A TREE WITH A CHAIN SAW. EMPLOYEE WAS STANDING IN FRONT-END LOADER BUCKET IN LIEU OF STANDING ON A LADDER TO CUT LIMB. THE LOADER OPERATOR WAS CHANGING THE POS ITION OF THE BUCKET WHEN EMPLOYEE'S RIGHT HAND GOT CAUGHT IN THE LIFT MECHANISM OF THE LOADER BUCKET.
