Night shift crews were entering the underground at the start of shift through portal four. The crew sensed a bitter acid taste as they entered. The crew immediately retreated out of the mine and contacted management. The mine was left to ventilate, MSHA 1-800 number was called. Mine rescue team determined a sulfide dust explosion occurred during day shift blasting.
The electrician was removing de-energized power cable from #1 portal. While using a cordless band saw to cut the cables the electrician's hand slipped making contact with the blade. The result was a laceration to the left index finger.
Employee was loading wire into the back of a truck, the wire caught on an obstruction causing it to spring back. The wire struck the employee's right pinky finger. Causing an oblique fracture to the right pinky finger. Currently the employee is working on restricted duty.
Miner was replacing striker bar on jumbo, when flushing head slipped and impacted right ring finger. Miner received a fractured finger and five stitches.
Miner was attempting to install splitset rock bolt. Miner inserted bolt into drilled hole. While positioning driver into splitset, miner bumped splitset causing it to dislodge and fall from back striking him on bridge of nose. This caused a laceration requiring 6 stitches.
While riding the conveyance down the shaft at a slow pace, employee tried knocking off a piece of timber from the shaft steel set getting his right index finger caught between conveyance and guide resulting in a laceration requiring 8 sutures.
Crew was using a cutting torch to remove inaccessable guide bolts; a piece of molten metal inadvertantly fell down the shaft. It is unknown what was smoldering as the shaft is under rehabilitation at this time and it is unsafe to travel down the shaft to extinguish the fire.
THE DECLINE WAS STILL A BIT HAZY FROM PREVIOUS BLAST. THERE WAS NO VISIBLE BOOTLEG AT THE FACE.

EMPLOYEE WAS BARRING DOWN SMALL PIECE OF LOOSE WITH BLOCKY GROUND. CONDITIONS ANOTHER SMALL PIECE CAME DOWN & HIT EMPLOYEE IN SHOULDER.
EMPLOYEE WAS UNLOADING SCRAP METAL FROM DUMP TRUCK. PULLED/LIFTED ON METAL STRAIN BACK.
EMPL WAS WORKING ON LHD & STRAINED BACK BACK HAS BECOME PROGRESSIVLY WORSE. DR HAS EXAMINED HIM & BELIEVES HE SHOULD NOT RETURN TO WORK UNTIL FURTHER NOTICE DR EXAM 6/20.

EMPLOYEE WAS BOLTING, STEEL RETAINER CAME OFF, THREW EMPLOYEE DOWN. MEDICATION ISSUED.
EMPL WALKING ,SLIPPED ON ROCK AND TWISTED ANKLE.MEDICATION ISSUED.

EMPLOYEE WAS WORKING ON A PUMP REMOVING BEARING WITH RUBBER HAMMER. HAMMER BOUNCED BACK & HIT EMPLOYEE IN NOSE.
SKIN IRRITATION SLOWLY BOT WORSE AND MEDICATION ISSUED.
EMPL STEPPED ON ROCK AND TWISTED ANKLE HAIRLINE FRACTURE RESULTED.
EMPL BREAKING DOWN LONG HOLE STEEL HIT COUPLING WITH HAMMER,FOREIGN OBJECT HIT EYE.
EMPLOYEE WAS DRILLING OUT OF LHD BUCKET SLIPPED AND FELL FROM BUCKET.
WHILE EMPLOYEE WAS BOLTING A SMALL PIECE OF ROCK STRUCK HIM ON THE ARM.
