EE was cleaning up at Hawkeye Quarry and claims he tried to pick up a flat piece of steel, which he estimated was 180-190 lbs. EE claims he has lower back sprain and strained ligaments and tendons in lower back.

Mostrando los 22This 7000-1 is being submitted under protest to terminate Citation No 6186925 & Order No 6186930. Cessford Construction Company disputes MSHA's allegation that an "accident/injury" occurred at the Biggsville Quarry on 6/28/07. The employee that MSHA alleges was injured, did not work on 6/28/07. This 7000-1 is being filed to avoid enforcement action and/or civil penalties.
Employee climbing inside rock crushing feeder to assist putting chain around stuck boulder, lost footing & fell into feeder. Another employee in feeder grabbed him & helped break his fall. Employee taken to ER & x-rays taken. Released with pain meds & no broken bones, bruises & strain to lumbar & side. Later visit to dr. on 6/21, x-rays showed back contusion & rib fractures.
Employee was welding on wing; received a welding flash burn to his eyes.
Employee was changing hammer tips in the hammer mill which is part of his regular job duties. The shank of the hammer mill dropped down pinching the employee's finger between the disc and the shank. Employee received a cut to the ring finger on the left hand which required stitches.
Employee moved the metal steps to the lab trailer away from the trailer to paint. He went inside the trailer and came out after a while and forgot that the steps were pulled away from the trailer and stepped out and fell between the trailer and the steps. He received a laceration to his left shin from hitting his shin against the metal steps.
Employee was putting air in a truck tire. The sidewall on the tire blew out into the employee's face. Employee received scratched eye & concusion to right ear and minor cuts on the face. Employee was performing his normal duties.
Employee was removing a large rock from his feeder on a limestone crushing plant. His injury was diagnosed as a double hernia.
EMPLOYEE WAS CHANGING THE BUCKET ON THE HITACHI. HE STRUCK THE BUCKET TOOTH TO KNOCK IT LOOSE AND A PIECE OF METAL LANCED INTO HIS LEFT FOREARM BETWEEN HIS WRIST AND ELBOW.
EMPLOYEE WAS CHANGING SCREENS ON SCALPER. GOT FINGER PINCHED BETWEEN SCREEN AND SCREEN BOX. CUT RIGHT HAND RING FINGER. WENT TO ER LATER THAT EVE AND RECEIVED STITCHES. NO LOST TIME.
EE WAS WORKING AROUND WELDER & RECEIVED WELDER FLASH BURN TO EYE.
EE WAS WORKING ON MACK DUMP TRUCK CHANGING THE FRONT SPRINGS. HE WAS USING AN AIR TOOL AND THE HOSE CAME OFF OF THE TOOL, SLAPPING HIM IN THE EYE. RECEIVED A SCRATCHED EYEBALL.
EE WAS CHECKING A NOISE IN THE MILL. EE OPENED THE DOOR OF THE MILL CHECK NOISE. HE TOLD ANOTHER EE TO ENGAGE THE CLUTCH ON THE MILL NOT REALIZING THE ENGINE WAS STILL RUNNING. EE S ANKLE/FOOT GOT CRUSHED/BRUISED IN THE MILL.
EE WAS MOVING WATER PUMP AND CAUGHT FINGER BETWEEN PUMP AND FLOAT. THE TIP OF THE RIGHT HAND RING FINGER WAS SMASHED, BREAKING THE BONE AT THE TIP OF THE FINGER AND REQUIRING 5 STITCHES.
EE WAS CLEANING UP AT THE PLANT SITE. HE WAS MOVING A TAIL PULLEY AROUND ON BACK OF THE SERVICE TRUCK WHEN HE SLICED THE END OF HIS MIDDLE FINGER.
EE WAS COMING DOWN OUT OF MACK HAUL UNIT AND SLIPPED OFF BOTTOM STEP, FELL BACK AND HELD ON TO RAIL. STRAINED HIS BACK MUSCLES.
EE OCCASIONALLY HELPS WITH BULLDOZING & CLAIMS THAT THE BOUNCING & JARRING FROM THE DOZER CAUSED SEVERAL DISCS TO RUPTURE IN HIS BACK.
EE WAS DOING A RIP RAP SAMPLE & PICKED UP A ROCKOFF OF THE SCALE & PINCHED HIS LEFT INDEX FINGERIN BETWEEN TWO RIP RAP.
EMPLOYEE WAS TAKING AN OIL COOLER OFF OF A 988 LOADER. A PIECE OF FOREIGN MATERIAL FELL INTO THE EMPLOYEE'S LEFT EYE.
ON 5-1-00 EMPLOYEE'S KNEE BUCKLED WHILE SWINGING A MAUL BREAKING BOULDERS. NO TREATMENT WAS SOUGHT AT THAT TIME. ON 5-24-00 EMPLOYEE'S KNEE LOCKED WHILE BENDING DOWN GETTING READY TO MOVE A RO CK. EMPLOYEE ENDED UP HAVING ARTHROSCOPIC SURGERY ON 6-1-00.
EE WAS TAKING A SAMPLE OF 3/4" CLASS "A" BIN WITH IDOT INSPECTOR. WHILE CLIMBING DOWN AFTER OBTAINING THE SAMPLE, THE EE MISSED THE NEXT TO THE LAST STEP ON THE LADDER AND INJURED HIS LOWER BA CK AND LEFT LEG.
THE WIND WAS BLOWING VERY HARD THE DAY OF THE ACCIDENT AND BLEW A PIECE OF METAL INTO THE EE'S EYE. EE WAS WEARING SAFETY GLASSES WITH EYE SHIELDS. EYE BECAME IRRITATED.
