Emp. was standing on the track hoe tracks filling with DEF. When EE finished, EE turned to the left and EE's left foot slipped off the tracks and EE fell landing on the fuel trailer that was parked beside the track hoe. EE suffered 4 fractured ribs and nicked EE's lung on left side. The area received a heavy rain the night before, the area, the tracks, and EE's boots were muddy.

Employee Strained EE's back operating loader. Employee didn't want to go to doctor but pain got worse and decided to go on 02/18/26.

On 12-27-24 emp reported pain in lower back, didn't seek treatment until 1-14-25. EE was seen by clinic, treated for lower back pain and placed on light duty.

Emp was injured when a box holding a bearing fell off a tailgate and struck the top of EE's left foot causing a laceration and fracture to EE's toe.

Foreman suffered a fatal wound during a non-mine related firearm discharge event.

Emp. Stepped off a track hoe and as EE turned to walk away EE stepped on the side of a rut and twisted EE's right ankle causing a fracture.

Employee was using a pipe wrench to seat a belt on the PS1 Shaker Pully, when the belt seated the pressure released and they smashed a hand between the wrench and a guard brace that was under the pully. This resulted in a laceration that required 13 stiches to close up. EE was given a prescription for antibiotic and placed on light duty until the stiches are removed in 10 days.

Emp. was fueling the track hoe and standing on the tracks. The fuel trailer was backed up next to the tracks. As employee was waiting for the tank to fill they shifted their weight, lost balance and fell. Employee reached for the rail on the trailer and missed, falling in-between the trailer and the track, causing a fracture to their right forearm.

Emp. was working on a belt and a piece of metal fell and hit EE's right ring finger causing a cut and a bruise.

Emp strained a muscle in Rt shoulder when emp jumped after a mild shock from welder caused emp to jerk away from it.

EE was coming down the stairs from the shaker deck platform onto the middle platform. When they turned and claimed to feel a small pop in right knee. They did not request medical treatment until 7/25/23, they were taken to the Co Dr and examined. It was determined that a pre-existing bone spur was causing the pain and swelling. EE was placed on LD and a follow up appt. on 7/28/23.

Emp was using a pry bar to keep a pump impellor from turning as they were replacing it. The impellor turned and EE's rt hand was caught between the pry bar and the pump housing causing a bruise to the top of EE's hand.

Mechanic was working on a track hoe and removing a tooth form the bucket. The tooth was stuck and they struck the tooth with a hammer and sliver of metal sheered off and struck them in the leg, causing a puncture to right leg.

Employee was using a utility knife to remove a zip tie from the grease line on a loader. The knife slipped and cause a laceration to the left arm of the employee requiring 2 stiches to close.

Employee was pulling a wrench, tightening a bolt on the pump at the plant and the wrench slipped causing EE to stumble. EE then fell onto right hand, cause a clean break to EE's right hand.

Pit loader backed into the Track hoe causing damage to both and minor injury to the track hoe operator.

Technician was driving a track pin in because the track press didn't push it in all the way. While hitting the pin with a sledge hammer, a flake of metal flew off the pin and stuck in the Tech's left hand.

Technician was working on a machine at a mine site. When EE stepped down off the machine, EE felt a pain in the knee.

EE was exiting the loader, missed bottom step, and had already released his one hand grip causing him to lose his balance and he fell to the ground striking his elbow and shoulder.

Using shoulder to rest pry bar on attempting to lift head pulley to place bearings, he strained his shoulder.

Attempt to remove stone from impeller vein. Applied more pressure to remove stone. Stone came loose and allowed hand to slip off screwdriver. Then small fifth finger struck impeller cover which was worn by sand and gravel to a razor sharp edge cutting employee's finger.

Employee not sure of time nor place of occurrence. He believes that backing of equipment choked and bruised his lower back.

Employee was pulling himself up onto trackhoe to operate.

FAULTY EQUIPMENT, HAND HOLD ON SUB CONTRACT HAULER VEHICLE NOT OWNED BY APAC

ATTEMPTING TO TAKE OUTER INSULATION OFF POWER SOURCE CABLE TO SPLICE WIRE TOGETHER FOR A LONGER CABLE TO REACH BATTERY, HE USED HIS PERSONAL POCKET KNIFE. HE CUT HIS RIGHT RING FINGER.

EMPLOYEE WAS EXITING DOZER WHEN HE STEPPED ON A DIRT CLOD AND JUMPED TO THE GROUND CAUSING A SPRAINED LEFT ANKLE.

EE WAS PUTTING WATER LINE IN TO PLANT. HE ALLEGES BACK STRAIN WHILE TIGHTENING BOLTS.

EMPLOYEE WAS REPAIRING AIR CONDITIONER ON LOADER(THE SCREEN CAME OFF AND HE WAS REPLACING IT). HE GOT TWO FINGERS CAUGHT IN THE AIR CONDITIONERFAN.

EE WAS REACHING FOR FOOD LEFT ON TOP OF ENGINE TO WARM. WHEN HE TURNED AWAY, HE GOT HIS HAND INTO THE FAN BELT ON THE ALTERNATOR

THIS EMPLOYEE WAS CHANGING SCREENS AT WASHER AND FELL APPROX. 2 TO 3 FT AGAINST RAIL. HE WAS CARRIED TO DOCTOR AND CHECKED. NEGATIVE X-RAYS. RETURNED TO WORK SAME DAY. WAS GIVEN A PRESCRIPTION FOR PAIN MEDICINE.

THIS EMPLOYEE WAS GREASING THE LOADER BUCKET AND STRAINED HIS KNEE.

EMPLOYEE ALLEGEDLY INJURED FOR WHILE MOUNTING OR DISMOUNTING LOADER. HE DID NOT KNOW WHEN ITHIS HAPPENED. PHYSICIAN TOLD HIM THAT IT HAPPENED ON JOB.
