A truck driver was in the pit to get a load of gravel and passed out at the scale house. EMS was called and driver was transported to hospital in Pueblo. Pit supervisor checked on driver later in the day and was told the driver is fine but is being kept for observation. The ill individual is not an employee of the mine.
Miner was filling a plastic jug of DEF from a barrel of DEF. The pump had a leak in it and DEF fluid sprayed in to the miner's face.
Employee was walking from control van to screen deck and stepped on a rock and fell down. Supervisor took employee to the hospital, had left leg x-ray found both bones above ankle were broken. Surgery is scheduled for 4.9.19
Driver had gotten out of truck WAS STANDING IN FRONT OF left front axle, when another truck approached from the rear contacting trailer and pushed parked truck forward pushing truck onto employee's foot and axle. Damage to parked unit was broken tail light. Other unit had damage to tarp arm.
EE WAS SECURING A PLATE TO THE BOTTOM OF THE CONE. IT SLIPPED OUT OF HIS HANDS AND HIT HIS FOREHEAD ABOVE THE LEFT EYEBROW, JUST BELOW THE HARD HAT.
ROCK LODGED IN TAIL PULLEY WHICH STOPPED BELT. EE WAS IN LOADER, HE HONKED HORN TO GET EE ATTENTION THAT BELT REACHED IN TO DISLODGE ROCK WITH HAND. EE WAS STANDING ON GROUND WHEN HE REACHED INTO TAIL PULLEY. ONCE ROCK WAS REMOVED, BELT RESUMED MOTION AND CAUGHT EE'S ARM.
