Occupational Illiness that did not orignate at Buzzi Plant. We feel we need to report. Employee originally stated it was a personal injury then later retracted that and claimed it was work related.

Employee was performing regular maint. on boiler tubes. Employee alleges that the rebar he was using became lodged between two tubes. When he pulled rebar out he felt a pain in left shoulder. Employee started physical therapy as of 11/29/2007.

Employee used his own pocket knife to cut a plastic clip off a quard so he could do some repair work. Knife slipped and cut left hand.

Employee was standing on a small rock and slipped off. Caused strain to lower back due to degenerative disk disease. Did not report until 8/27/2007. Did not have surgery until 4/16/2008.

Employee was in the process of unplugging a line. When covering was removed back pressure in line blew cement dust into both eyes. Abraded both eyes.

Employee was installing roll on raw mill. Roll hung up & using protapowers to get it to fall into place. Got hand into a pinch point when roll fell into place. Hand caught between a board and the 2 ton roll, pinching left index finger.

A 12 foot air lance was being run down #2 kiln feed pipe to unplug choke.

Employee alleges that wind blew dust in his eyes.

Employee was rolling over a hammer on the secondary hammer mill. He alleges that while he was bent over putting a bar under the hammer he felt something give.

Employee has been working in the waste heat dept since 9/20/2005. He alleges that the task of unplugging the boilers has been repetative motion and has caused discomfot in his hands and wrists. HAVE BEEN AWATING DIAGNOSIS FROM COMPANY DR. REPORT FILED WHEN THAT BECAME AVAILABLE.

Employee claims he was leaning over railing to weld. He felt a pain in his back.

Employee claims as he was pulling on heavy wire cable manually he strained his back.

Employee claims that his Lanyard got wrapped around boot when squatting to work on cable tray. THIS ACCIDENT DID NOT BECOME REPORTABLE UNTIL 11-3/2005. IT WAS DETERMINED THAT SURGERY WOULD BE NEEDED.

Employee was drilling holes in metal cable tray. The weather conditions were windy. Employee was wearing safety glasses but the wind blew metal fragment into eye.

Employee states he was walking down steps and felt a pop in his leg.

Employee claims he was pushing angle iron from front to back of rack with left foot.

Employee allegedly lifted tool bag out of back of truck and strained his shoulder. He was trying to hurry. Emergency repair work on a burned 2300 volt circuit. This injury did not become reportable until 8/31/2005.

Employee was working in the clinker cooler room. Dust/particle blew in right eye. Abrasion with prescription medication.

Employee alleges that he had been picking up trash all day and as the day wore on his back got sorer.

Employee was exiting the 988 F Loader and missed the last step. twisted right knee as he went to ground.

#4 kiln burner caught on fire- As employee turned corner into the kiln building a puff of hot coal dust blew out of burner into both his eyes.

Employee was using a cutting torch to remove a beam in sections. When he turned with beam section in hand his left knee popped. This was not a reportable injury until employee was evaluated on 6-6-05 and the diagnosis was received today.

Employee was working at # 3 Kiln Pier #1 prying on bearing cap to pull off and felt pain between shoulders.

Employee was dumping a load of dust and chunks from clean up and wind blew the debris onto his face and into his left eye. Scratch lower part of cornea.

EE was cutting t-Bolts our of #2 cooler and hot slag got down shirt sleeve. Minor burn on right wrist.

Employee was helping with the repair of #2 kiln. Pulling back tire and moving cribbing. There was a lot of heavy lifting on this repair job. Right groin Strain.

EE was carrying grate from #2 cooler and one dropped on right foot. Contusion of right foot.

Employee was pulling on bar in screw box to line up holes to put bolts in screw.

Employee was demolishing old 3" conduit when the conduit fell on his left hand.

Employee was working in maintenance shop area. He was removing something from a top shelf as he pulled item off shelf dirt fell into eyes. Employee reported accident sought medical attention the same day. He was wearing protective eye gear, washed eye out at eye washing station immediately following occurrence.

Employee was looking into cooler #1. When he did so cooler dust blew into his eyes. EE reported accident but did not seek medical attention until a later date. EE was wearing protective eye gear. EE washed eye out at eye washing station immedately following occurrance.

Employee was blowing flu's at waste heat with a crooked lance. Employee did not inform supervisor or office of x-rays. Did not know it was reportable until hospital sent bill for x-ray taken.

Employee tripped on half step while out of town on business in St. Louis MO. When fell employee hit concrete post with left hand.

Employee stepped on loose clinker and twisted left knee.

Employee was climbing into new structure using unsecured concrete blocks. As she stepped up one of the blocks tipped and employee fell to ground.

Employee states that as he was moving from west to east on cat walk his wrist watch caught on railing. He alleges this pulled arm straight out while body was moving away. He said it created a jerking motion of the shoulder.

Employee was shoveling at the kiln building. While doing so he felt a pain in his back.

Employee using rebar to remove loose buildup of hot dirt in between boiler tubes. Employees claims he felt a sharp pain and elbow started swelling.

Employee was removing a light fixture as doing so dust fell from top of the pillar and covered the employees hard hat, safety glasses and clothing. Employee states that he did wipe off the top of the fixture before attempting to remove.

Employee uses airlance to blow dirt from boilers. Also uses rebar to break caked on dirt. He states there is a lot of hand, wrist & shoulder movement to this task. This task is required every day. Employee was given medication first. Then Dr. decided he needed thearpy. That was done on 10-12-04. Reported as soon as it became reportable.

Employee had been working on top of So. Boiler. Plant was getting ready to put boiler back on. As he was coming down the ladder he missed the last rung. Employee states that he felt himself start to fall and grabbed the ladder. By doing this he felt a pain in his back. Did not request medical treatment until 5/4/04.

Employee was changing brass on the rolls on # 3 Kiln. He picked up a 1 1/2" impact to tighten the bolts. Felt a pain in shoulder. EMPLOYEE DID NOT REQUEST MEDICAL ATTNETION UNTIL 5/5/04. AT THAT TIME HE WAS SENT IMMEDIATELY TO THE COMPANY DOCTOR.

EE WAS CLIMBING ONTO THE BASE OF #1 PIER, WHEN HE STOOD UP, HE FELT A SHARP PAIN IN LOWER BACK. EE WAS PERFORMING MAINTENANCE WORK. HE WAS CHANGING OUT TRUNION BRASS ROLLS. EE DID NOT SEEK MED TREATMENT UNTIL 4/23/04.

EE WAS PULLING OUT FUSE. HE HAD BEEN TOLD POWER WAS OFF. SHOCK & BURN FROM CONTACT WITH 2300 VOLT.

Employee was repairing a part on the loader. He was bent over alignining a tooth to the shank. A piece of metal broke off and flew up.

HE WAS HAMMERING A METAL PLATE INSIDE A BALL MILL AND A PIECE OF PLATE CHIPPED OFF AND STUCK IN HIS RIGHT LEG.

EE stated that a rock was laying in front of steps. He kicked it out of the way, it was observed by the EE that the rock was about 2"X 2" & weighted less that a pound. This was a statement made by EE to his foreman the following morning when he reported his accident/injury.

EE WAS USING A JACK HAMMER ONT UNNEL FLOOR AND SHOVELING DEBRIS INTO BUCKET. HE WAS THEN DUMPING THE BUCKET OF DEBRIS ONTO THE TUNNEL BELT. AS HE WAS DUMPING THE BUCKET OF DEBRIS HE FELT A POP BETWEEN HIS SHOULDER BLADES. HIS NECK AND RIGHT ARM THEN BECAME PAINFUL TO MOVE.

EMPLOYEE HAS WORKED AT THIS CEMENT PLANT FOR OVER 20 YEARS. HE FEELS EXPOSURE TO NOISE HAS CAUSED HEARING LOSS. HE HAS RINGING AND HUMMING IN HIS EARS.

EMPLOYEE WAS PULLING BEARING HOUSING OFF #3 COAL PUMP AND STUMBLED AROUND A CONCRETE BOLCK CAUSING BODY TO TWIST. THE BEARING HOUSING WEIGHS APPROX 50 LBS. EMPLOYEE DID NOT REQUEST TO HAVE MED ICAL TREATMENT UNTIL 2-10-04.
