equipment operator slipped on the steps of a 980H loader. Fell back tried to catch himself, resulting in a broken left wrist.
While drilling on a sheet of steel victim got small sliver of metal embedded in his right eye. The victim was wearing safety glasses at the time , however it was breezy in the area he was working. Once he felt the sliver he irrigated his eyes at the eye wash station, and he thought he had removed the problem. Several days later his right eye was red, itchy and very watery.
Employee twisted his knee on 12/9/2013 while working in the plant but thought it was fine (First Aid only). He reported it to the Manager and returned to work that day. On 2/19/14 employee informed the Superintendent that his knee was not healing. He was transported to the clinic at that time where the physician recommended physical therapy.
Employee was stepping off the tag axel on the portable jaw and twisted his knee.
EE was handling screen to another employee while standing on screen deck. The weight of the screen shifted pinching EE's finger between wire cloth on screen and metal bar on rock box.
Was holding up inspection door to surge hopper with right hand. When done inspecting, his hand gave out causing door to slam down onto left hand. Injury refused to have his hand checked out immediately but since it would not stop hurting he decided to have it looked at today, 11/30/09. The doctor said it was broken and he is now in a cast and on restricted duty.
Swelling of the right elbow while replacing eye wash station. Thought it was a spider bite. Next morning bruising started on the arm so went to the clinic. The clinic concluded that it was a contusion, was given a prescription for an antibiotic.
Employee was cleaning up around screen #2 with equipment #56 and flying debri landed in his right eye.
Injury was spinning the head of the cemco & other employee was using an air gun when his finger got stuck between the head of the cemco & the air gun socket.
EE was climbing back into the loader and slipped on the step and gashed his shin open.
Employee was pulling the welder trailer out of the tool van and the tires dropped off the 6" elevated floor. Pushed forward running up on his left ankle and pushing him into the scraper can smashing his right hand.
Unload tail guard from pickup and was dragging it to the conveyor. Tripped on something and hurt back/right shoulder catching himself as he fell.
Area is muddy due to significant rain. Employee was cleaning under the pit belt feeder by shoveling out the material. His foot slipped in the mud while shoveling and he twisted his back. Employee transported to Doctor who indicated that he was unable to return to work.
Pulling top deck screens off for replacement and screen hung-up when pulled. Incident not initially reportable, employee then transferred to mine ID # 0401846 (Waterford) where he then aggrivated injury. Placed on restrictive duty on October 6, 2005.
CHANGING V-BELTS AND HIS LIP MADE CONTACT WITH GUARD. CONDITIONS WERE GOOD, DAYLIGHT & 50DEG. TEMP.

WHILE PRYING OPEN FEEDER DOOR, A BIG ROCK CAME OFF OF THE TOP AND STRUCK EE'S RIGHT WRIST.
EE WAS ON THE DECK OF THE DREDGE AND A ROCK FELL OFF THE GRIZZLEY AND HIT HIM IN THE HARD HAT. (HEAD) HE WAS WEARING A HARD HAT.
EE WENT HOME EARLY FOR A DR'S APPT. NEXT DAY HIS WIFE CALLED & SAID HE WAS IN HOSPITAL BECAUSE HE HAD A HEART ATTACK.
WHILE OPERATING LOADER, EE HIT A BUMP AND THE SEAT BOTTOMED OUT AND INJURED HIS LOWER BACK.
