Employee alleges that he was welding or cutting and sparks went into the air and wind blew them into his ear. Employee states he was wearing appropriate PPE.

Employee was helping to put V belt on rock shaker. His screwdriver slipped and hit him in the lip, causing a laceration to the lip.
WHILE HITTING WITH A HAMMER, THE TOOTH OF BACK-HOE BUCKET, A PIECE OF THE TOOTH CHIPPED OFF AND STRUCK/CUT UPPER ABDOMINAL AREA OF EE. RESULT WAS A CUT SHIRT AND CUT SKIN CAUSING BLEEDING, BUT NO EVIDENCE OF TOOTH PARTICLE REMAINING IN WOUND. THE CUT WAS APPROX 1 1/4" IN LENGTH. SUPERVISOR TRANSPORTED HIM TO RECEIVE MEDICAL ATTENTION.
STRUCK CORNER OF PIECE OF ANGLE IRON AT FOREHEAD. 1/4 INCH LACERATION REQUIRING ONE STITCHES.
DAYTIME OPERATION 2, OPEN AREA AROUND AGGREGATE STICKNILE. WEATHER AND VISIBILITY NOT A FACTOR. INJURED WORKER DROVE TRUCK PERSONAL THROUGH STOCKPILE AREA AND FAILED TO YIELD TO LOADER IN OPER ATION.
BACK STRAIN WHILE WORKING AROUND PLANT. HAD BEENINVOLVED IN SOME LIFTING DURING COURSE OF DUTY.
