Miner was pressing a pin into a dozer track. The press malfunctioned and the saddle tool of the press hit the miners right forearm resulting in a fracture of the Ulna.

Customer was using 773 haul trucks to be loaded with boulders from LVP pit to be taken off site. During execution the trail haul truck lost brakes on a downhill slope between 6-10 degree decline. The trail truck struck the lead truck pushing it under LVP dozer trap. 911 called immediately and flight for life was dispatched. Front miner has broken ribs and back.
The miner was going to utilize a rare earth power lift magnet weighing approximately 65lb. They placed the strap of the magnet over right shoulder to carry it to a piece of steel, they then turned to walk and struck left foot on a sawhorse losing balance and falling onto the concrete floor with the left side of their body, the magnet landed on their right hip.
The operator was watering a road with a CAT 621G water pull. The operator was driving approximately 10 miles per hour when they drove into the equipment parking V-ditch. The operator stated that they knew it was there, but didn't notice it due to an optical illusion. The impact bounced them in the seat straining their lower back. The operator was having a hard time breathing.
Miner was on top of a piece of mobile equipment to remove the fuel cap to begin fueling. When climbing back down, EE missed a step with EE's foot and caught self on the ladder with their hands. EE stated that it felt like EE pulled something in their shoulder when EE caught self. Miner did not fall all the way to the ground.
The operator was climbing down a ladder on a 980XE CAT loader. Operator misjudged the last step and fell backwards to the ground approximately 3'. Operator sustained a left wrist fracture.
Jaw feeder at plant primary plugged up and overflowed material. Miner was cleaning the jaw feeder underbelt with a shovel because the skid steer was tagged out due to mechanical failure. Miner was the ground person in the area and noticed the tail pulley was about to be buried. Miner decided to start clean up while belt and jaw were operating which is against company policy.
Miner failed to access a working area in a safe manner, resulting in a loss of arm.
Lifted a day box into the back of a pickup truck, felt pain in right elbow/arm.
Miner was ripping the pit bench on a Dozer with the blade in the raised position, when he came to end of the bench he continued off the steep slope and the abrubt force threw the Miner into the friction stearing levers making contact with his head/face causing minor contusions and bruises.
The Driver backed up to dump at the dump site, he lifted the bed to dump and the berm gave way and truck went over the berm backwards, when the driver was removed from the cab he complained of back pain. EMS Transported Miner to the hospital.
The Miner had a 1/2" cotton rope coiled around his left forearm, as he drove the JCB forklift forward, the tail end of the rope went under the left rear tire, pulling the miners left arm down against the door jamb breaking the bones in his left forearm.
Clearing 30" rock from feeder and got foot smashed between the rock and track hoe bucket in efforts to remove rock.
State Scale Inspector pulled his truck next to scale stepped out on wall. Began to walk toward another truck and lost footing and slipped and fell between truck and wall. Possibly hitting head or body on truck bumper.
EE was stepping out of lube truck # 172160 and stepped on a rock twisting his ankle.
While exiting equipment he lost hand hold and slip tripped.
Repairing a conveyor belt and the board in which was supporting the belt slipped and caused EE to fall 5-6' on the ground.
Employee was getting out of Bulk truck. As he went to get down he lost his grip and fell to the ground. Employee went to emergency room and was told he was ok. Still suffering from pain 3 days later he went to doctor who found a fractured rib. Doctor released him to work. Still in pain 2 weeks later he went to doctor on 1-29-08 and was told to take a week off.
EE gasses up a air compressor at start of his shift. spitting some gas while fueling on his right hand. after fueling up air compressor. He went over to tother area to start a project, which required to use a torch. When he struck the starter to light the cutting torch a spark hit his right hand, causing it to burn his hand that the fuel spilled on. He went to umc burn right
Operator was pushing material with dozer parallel with edge of slope. Material gave way on right side of dozer track. Dozer rolled down a 30 foot slope, turning over coming to rest on its track.
EE fell getting off equipment.
After completing a repair on the deep pit belt, EE was trying to release the clamps holding the belt in place. He got on the belt & released the clamps and the slack which was in the belt moved the belt forward & took him upside a roller which pinned him at the chest for about 1-3 min until a co-worker cut the belt and removed him from roller.
Employee was lifting up grizzly bar and it slipped and pinched his pinky finger on right hand.
Climbing off loader, slip & fall on the ladder, hurt back.
An ee doing maintenance on a stacker was thrown off the stacker by the plant operator failing to visually check the plant before starting the stacker, resulting in a back injury.
INSTALLING RETURN WATER LINE FROM DERRICK SCREEN TO PONDS. EE WAS HELPING INSTALL THE PIPE, HOLDING ONTO THE PIPE TO THE PLACEMENT BEAM WHEN IT MOVED AND PINCHED HIS MIDDLE FINGER IN BETWEEN THE PIPE AND THE BEAM. CUTTING TIP OF THE FINGER ON HIS LEFT HAND.
EE WAS ON THE CONVEYER CHECKING THE MOTOR DO TO A KICKOUT ON THE BREAKER. AFTER CHECKING THE MOTOR HE STARTED TO WALK DOWN THE CONVEYER TO GET OFF WHEN THE CONVEYER KICKED ON. HE FELL ONTO THE CONVEYER BELT, TRAVELING HALFWAY DOWN THE CONVEYER WHEN HE ROLLED OFF THE CONVEYER AND HITTING THE RIGHT SIDE OF HIS FACE ON THE FRAME OF THE CONVEYER, FRACTURING JAW DAMAGING EYE.
THE APPRENTICE WAS CHECKING THE SCREEN DECK, WHEN HE SLIPPED AND TWISTED HIS RIGHT ANKLE. IN RESULT, BREAKING THE ANKLE.
EE WAS CARRY A PULLY ROLLER ON HIS SHOULDER WHEN THE ROLLER SLIPPED OFF HIS SHOULDER. HE TRIED TO CATCH IT BUT ENDING UP CRUSHING HIS HAND. BREAKING THE LEFT HAND.
EE WAS REPAIRING EQUIPMENT (WELDING) THE FLASH OF THE WELDING WAS BRIGHT AND GAVE HIM A FLASH BURN ON THE EYES. THE WELDING SHIELD LENS WAS OLD.
LIFTING SCREENS THEY FELL ON HIS ARM WITH SHARP EDGE'S HITTING HIS ARM.
WHILE EXITING THE BULK TRUCK #229. OUR EE STATED THAT HE STEPPED ON A ROCK SPRAINING HIS RIGHT ANKLE.
GETTING ROCK OUT OF JAW BY A PRY BAR, BAR HIT EE IN THE HEAD KNOCKED OFF HARD HAT AND HIT HIM IN THE HEAD.
EE WAS TORCHING BOLTS OFF IMPACTOR. AS HE WAS TORCHING, HOT METAL SLAG RAN OFF AND BURNED THE ACCTYLEVE HOSE IN HALF, CAUSING A FIRE. IN ESSENCE THE FIRE BURNED EE S HAND.
EMPLOYEE GOT ON THE LAND CONVEYER TO GREASE THE HEAD PULLEY, WHEN HE WAS DONE GREASING, HE TRIPPED TO GET OFF AND SLIPPED OFF THE BELT FRAME, WHEN HE HIT THE GROUND HIS LEFT KNEE GAVE OUT.
EMPLOYEE WAS TRYING TO UNPLUG THE LIME SYSTEM, WHEN A HOSE CAME OFF ITS VALVE FROM THE BACK, ANDLIME SPRAYED HIM IN THE FACE. HE WAS WEARING SAFETY GLASSES , BUT THE PRESSURE WAS TOO GREAT, AN D IT GOT INTO HIS EYES.
