Employee was in the process of changing the bucket on the skid loader bucket. During this process the bucket came off of the mounting tab while in the air and the bucket came down on employees right foot

operator was cutting hole in oil line to drain pipe when fire started. Burned face and forearms

Guard fell on the back leg.

Employee was in the work boat and working on the back of the dredge. The employee tried stepping on to the back of the dredge hitting EE's head on the exhaust pipe attached to the dredge motor. This results in the employee falling back into the boat striking EE's rear end on two guide bolts.

349F excavator was trying to dig a big rock out of pile against west wall, operator was trying to find the top edge of rock to pull rock from pile. The teeth slipped off the rock which caused machine to jar hard against the pile. The operator didn't notice any discomfort until operator got out of machine at end of day and stood up.

Installing drive belt around sheave and cut finger on edge of sheave

laceration to right forearm, above wrist, (12 stitches) due to expanded metal guarding on cone crusher feed opening

Aluminum ferel coupling pushed off end of 4' lay flat hose,which struck employee in right ankle.

Independent non-employee tri-axle driver reported to scale house with 3 severed finger tips on right hand. Driver pulled to dump off area, proceeded to lighten loaded material. Driver was cleaning off tailgate, middle, ring and pinky finger tips were pinched between body and tailgate of truck, resulting in removal of finger tips. Driver was transported to local ER by lab tech.

employee notified supervisor fuel filter on filling station was clogged. employee was told not to perform task of changing fuel filter. employee performed task by using half of an extension ladder to climb to shut off valve on fuel tank. employee climbed ladder,which slipped out from under their feet. which lead to a fall approx 6 feet, bruising right ankle.

Employee was removing a drive shaft from a pickup truck, when EE's hand slipped off and got caught in the universal joint and put a deep cut on EE's pinky finger.

Slip and fall incident because of compacted snow conditions. Caused injury to right wrist.

The incident happened to a Non Quarry Employee. ee was an outside truck driver that slipped off of truck and injured leg while in the quarry at the scale location.

While performing maintenance on plant, employee sustained a superficial laceration on knee while cutting a belt into sections.

while performing housekeeping on screen plant employee tripped and fell injuring left hand and left leg.

Mechanic was working in a cramped space on Cat 769 on top of engine compartment. twisted and injured back while lifting engine lid to perform repairs.

While working a cable lever hoist, employee cut his right hand ring finger.

Equipment mechanic was pulling on large torque wrench. Felt pain in his left shoulder.

Quarry laborer strained a muscle in his lower back while shoveling built up crushed stone during plant cleaning.
