MAX 45 PDI SAND LINE (BULL HOSE) HOOKED ON DRILL DECK AND PULLED APART A TEMPORARY SPLICE THAT HAD BEEN INSTALLED THE AFTERNOON PRIOR.THE WHIPCHECK THAT HAD ALSO BEEN INSTALLED HELD THE 2 ENDS BUT AS A UNIT THE HOSE CAME INTO CONTACT WITH THE CAB SIDE WINDOW AND BROKE IT INWARD TOWARD THE OPERATOR.EITHER THE AIR FORCE OF THE HOSE KNOCKED THE OPERATOR OFF THE SEAT INTO THE FLOOR.
EMPL CHANGING A TIRE ON A HAUL TRUCK.USED AN AIR WRENCH TO TIGHTEN LUG NUTS.THE EXHAUST AIR OF THE WRENCH BLEW DIRT,DUST AND POSSIBLY LOOSE PAINT ABOUT FACE.A FOREIGN PARTICLE BLEW INTO LEFT E YE.EMPL WAS WEARING SAFETY GLASSES AT THE TIME OF INJURY.REPORTED ON 2-25-92 SOUGHT MEDICAL ATTENTION ON 2-26-92.
EMPL HAD BEEN OUTSIDE WORKING.IT HAD BEEN RAINING AND MUD COLLECTED ON BOOTS.REENTERING THE MILL,WALKED OVER TO SOME GRATING OVER A WASH TRENCH AND WHILE CONTINUING TO WALK ,WAS SCRAPING THE M UD OFF BOOT SOLES.TRIPPED AND SLIPPED FELL TO THE GROUND AND JAMMED LEFT WRIST AND ELBOW TRYING TO BREAK THE FALL.
EMPL REPAIRING A TRUCK TIRE.WHILE USING HAMMER & WEDGES TO SEPARATE WHEEL RING,THE HEAD OF HAMMER FLEW OFF THE HANDLE AND HIT MIDDLE FINGER WHICH WAS ON THE EDGE OF WHEEL RESULTING IN AN IMPAC T CRUSHING INJURY.THIS IMPACT FRACTURED MIDDLE FINGER LEFT HAND IT REQUIRED MEDICAL ATTENTION.
EMPLOYEE WAS CHANGING WORN LOADER BUCKE TOOTH WITH NEW ONE. HE HIT HIS RIGHT HAND WITH A SLEDGE HAMMER WHEN DRIVING THE RETAINING BOLT OUT WITH A CHISEL
EMPLOYEE WAS ATTEMPTING TO BLEED A BRAKE LINE. BRAKE LINE WAS PLUGGED AND UNDER PRESSURE. WHEN EMPLOYEE CLEARED THE LINE, BRAKE FLUID SHOT INTO HIS EYE.
EE WAS STEPPING OUT OF A TRUCK. HE PLACED HIS RIGHT FOOT ON THE GROUND STEPPING ON A ROCK HE THEN TWISTED HIS ANKLE AND FELL TO THE GROUND. WHEN FALLING HE PUT OUT HIS LEFT HAND TRYING TO CATC H HIS FALL.
EMPLOYEE WAS SITTING IN A 35 TON HAUL TRUCK WHILE IT WAS BEING LOADED BY A 988 LOADER.THE LOADER OPERATOR DROPPED A LARGE ROCK INTO THE TRUCK WHICH JARRED THE TRUCK AND TRUCK DRIVER.
EMPLOYEE WAS INSTALLING CEILING DUCTING FOR A COOLING SYSTEM WHILE REACHING FOR A SCREW DRIVER HE SLIPPED FALLING TO THE FLOOR.HE PUT OUT HIS LEFT HAND AND WHEN HE FELL CAUSING A SMALL CUT AND JAMMED HIS THUMB.
CHECKING ELECT. PANEL, TOUCHED HOT WIRES POWER SHORTED FLASH AND FIRE FROM BOX,
EMPL WAS CLIMBING OUT OF 3' SERVICE TENCH E' DEEP. WIND WAS GUSTING TO APPROX 25 MPH WHEN EMPL HAD EXITED TRENCH HE WAS THROWN OFF BALANCE BY WIND GUST & SLIPPED BACK INTO TRENCH.
DRAINING FUEL TANK WIND BLEW CAUSING FUEL TO BLOWN ARM SOAKING HIS SHIRT SLEEVE WITH FUEL
EMPLOYEE WAS CARRYING A 100LB CAN OF ZINC UP A STAIRWAY.AT THE TOP OF THE STAIRS HE TURNED AND SET THE CAN DOWN.WHEN TURNING HE TWISTED HIS BACK.
EE WAS MAKING AN ADJUSTMENT TO THE CONE CRUSHER. WHILE MOVING THE ADJUSTING RAM THE RAM CONTROL WAS OPERATED CATCHING HIS LEFT THUMB BETWEEN THE HOUSING AND CONE HEAD.
