Employee was prying on a belly pan, when the belly pan came loose causing the belly pan to come down. As it came down employee's left hand was smashed between the ground and pry bar.

The employee was troubleshooting the main hopper belt. The employee removed the equipment guarding to check the motor and gearbox. The employee touched the v-belts for tightness and the v-belts began to turn, pinching the finger tips between the v-belt and motor sheave.
Employee was feeding electrical cables for a t-former. A coworker dropped part of the cable, causing the injured employee to slip, resulting in a same level fall. EE had some pain in EE's L knee. EE was evaluated and no FA was given. EE later stated EE was feeling some pain. On January 14 EE was given a MRI which showed no tears. EE was given PT
The miner was standing in an awkward position attempting to move a piece of metal and felt a pop in lower back.
Two employees were picking up a 6 inch hard suction hose that weighs approximately 80 pounds. EE says foot slipped and EE pulled a muscle in the back and requested to see a doctor.
Two miners were replacing a pipe on top of a conveyor tunnel. The pipe was secured to the backhoe bucket with a chain. When the backhoe driver lowered the pipe into position, the bucket allegedly struck the miner on EE's upper back.

The decedent, an o/operator, was located behind the truck, behind the raised dump trailer when a load of sand dumped from the trailer. The decedent was buried in the sand. Mgmt became aware of the missing driver at approx. 10:45 am and promptly began a search. EEs began digging in the pile of sand behind the decedent's truck and the decedent was discovered at approx 11:43 am.

Employee was trying to remove a bearing from the bearing housing. EE was using a sledge hammer to knock the bearing loose. When EE struck the head of the sledge hammer against the bearing a small piece of metal flew off the bearing and punctured left leg. Employee received one stitch.
Employee was stacking material with an excavator as ee was lifting the bucket towards the cab a rock flew causing the windshield to break. Employee felt something hit eye.

He continued to work for eight days without any issues. On 1/12/2016 employee went to his doctor because of dizziness and a headache. The doctor diagnosed the employee with a slight concussion that was caused by the fall.
Employee was working on the 850 track hoe. As he was working he felt something bite him on his left leg. Doctor prescribed the employee an antibiotic for a spider bite.

Employee was replacing a bearing on the head pulley on C-1. He was chipping on the shaft to slide the bearing on. As he was chipping a piece of metal hit his eye. This caused a large cut on his left eye. He needed two medical procedures to repair the eye.
Employee lifted a 10 inch pipe into bed of truck and while setting it down the pipe rolled and struck his left wrist causing fracture. employee was released to restricted duty.
The employee was greasing the plant and rock fell between the screen and guard. The rock struck him on the lip.

Employee was installing a bearing on a conveyor head pulley shaft, he slid the bearing onto the shaft and was using a chisel and hammer to adjust the bearing. While hammering on chisel the employee felt something strike his left wrist, when he examined his wrist he could see a piece of metal embedded in his wrist.
Employee complained of pain in right shoulder, neck and back from driving haul truck.
Employee was drilling holes on Mason sand conveyor to install a belt wiper, while drilling the drill jammed and twisted, fracturing employee's right ring finger.

The hammer and employees left arm was pulled between the troughing idler and conveyor belt causing pain to left wrist. Employee was examined by physician and released to restricted duty and is to follow-up with doctor on 2-8-12.
EE was cutting steel I-beams into 8' lengths. After making cut, slag was still holding the cut together, EE picked up the opposite end and dropped it to break the slag. As he did this, the I-beam caught his glove and smashed his right ring finger between two pieces of I-beam.
