Employee was trying to loosen a fuel filter on main tank. While utilizing a step ladder, they twisted their back when the filter loosened and felt something in their back. We took employee to the DR and it was found they had a strained muscle in their back.

The employee was grinding on a steel plate. Had all PPE on for the required work, EE stopped grinding, removed EE's PPE and wiped EE's forehead with EE's sleeved arm and the steel speck must have entered EE's right eye at that point.
EE our loader Operator stopped to talk to a truck driver, he was on the deck of the loader knelt down talked to the driver he went to stand up heard a pop in his right lower calf The next morning he said he had hurt is calf the previous day I took him the Enumclaw Hospital He returned to work on 8/4/14 The contributing factor is EE is 5"-11" weighs 305 lbs and 54.
An operator assisting an EE Agg Delivery Driver was straightening the tailgate of his Highway Dump Truck. EE was holding 4"x4" wood block with his gloved left hand against the tailgate, when the operator applied pressure with the 330 excavator' s bucket, the block rolled, pinching EE's left thumb between the block and the bucket requiring removal of the thumb nail and 3 stitches.
Affected employee was operating a dozer during his shift. He complained of back pain but indicated that no particular incident had occurred causing this pain but thought it was from operating the dozer. The employee was taken for medical observation where it was determined that he had a strain to the lower back. He was placed on modified duty.
While working on a conveyor belt repair the employee was using a utility knife, the knife slipped and the employee received a cut to his hand. The cut required 5 stitches.
foreign object in eye, employee used eye wash to flush his eye, thought he had rinsed it out. The next morning eye was irritated so he was sent to the clinic where they removed a steel sliver from the employee's left eye.
lifting stacking bags of floculant into floculant storage shed
Employee was positioning a jackstand under a trailer. While his right hand slipped allowing the head of the jackstand to fall and pinch his left index finger. Employee received 5 stitches and returned to work his following shift.
Worker on conveyor tail pulley on maintenance shop floor knelt down on left knee, grabbed pulley and pushed forward with right hand when something in left lower back popped.
While working with hard hat off, underneath hazard causing exposure, unaware of unsafe conditions and being careless, employee was assisting the shop foreman installing a transmission. The rear cross member was resting on a jack, it slipped and fell striking him in the lower forehead.
Delivery dump driver was in the process of unhooking and dropping off the rear trailer near the office and in the process of lowering the jack on the trailer's tongue EE strained his lower back.
Employee was opening the hood on mixer truck #7643. The hood would not open in the normal manner due to hood alignment pins sticking. So, he went to the lower rear of the hood to lift it off of the alignment pins, when he lifted he felt something pop in his left upper back.
Turned on blue pump about 1/2 throttle, then turned on water valve about 3/4 pressure. Hosed for about 10 minutes when nozzle separated from hose; the band on the hose slid down employee's hand cutting left hand between thumb and index finger.
Contusion to left knee climbing down into the engine well. Foot slipped and hit knee on metal part in well. Fell about 1 foot.
Standing on loose material so lost footing when pressure from water hose caused loss of balance. Dropped hose on foot hitting above steel toe of rubber boot.
Employee was grinnding on metal inside the shop with safety glasses on when a big gust of wind blew in the shop causing dirt and metal from the grinder to blow under his glasses and into his right eye.
Struck left thumb with hammer. Three stitches on thumb.

Striking tooth retainer with a hammer to secure tooth on 988B loader bucket. A metal sliver broke off of tooth striking him in lower left front of neck. Required minor first aid to stop bleeding. He finished shift and went to doctor, X-ray shows metal inbedded in muscle. doctor chose to not remove
Operator was operating excavator with door open. He was sorting large boulders when he dropped a boulder on the pile and a chip flew in the cab through the open door striking him in the left eye. Operator was not wearing safety glasses. He was examined by an optometrist, given prescription eye drops and sent back to full duty.
No special conditions to list. Employee was using a 2x6 piece of wood to lift a piece of conveyor belt, the wood had a nail protruding from one side. The wood slipped and the nail punchured his arm.
Employee was installing screens in a confined area and struck his left arm with a 3# hammer.
Employee was trying to bend a 3/8" piece of plate that had been heated in the middle by jumping up and down on it. The next morning the employee had some pain in his leg/knee but thought it would go away.
Water truck hose was being used to wash down a spill at the tail of a conveyor, when employee unhooked the hose from the truck he dropped it, the metal nozzle landed on his right foot.
EE was replacing a return roller. When he lifted the roller he hit his head on the conveyor frame. He was not aware he was so close to the conveyor frame.
pov berth #14 employee atempted to climb down ladder onto dredge, ladder slipped out of position onto the deck of dredge. employee fell aproximately 10-13 feet to the deck, inturn landed onto feet then fell on his shoulder. the employees failed to secure ladder before descending to the dredge.
While preforming maintenance, some particles apparently fell inside his safety glasses and got into his eye. The employee thought the particle had come out of his eye, however later in the evening his eye became irritated so he sought medical attention where they removed a piece of rust. There were no restrictions and no lost time at work.
While repairing a guard on a conveyor, the socket slipped off the bolt and the ratchet struck employee's upper lip pushing tooth back.
EE was using 16 lb. hammer on pipe and missed the lip on the 18" pipe. EE caught his finger between the handle of the hammer and the edge of pipe. The handle then swung around after hitting the pipe and hit EE's left side of torso. Injuries resulted in a laceration to the right forefinger and contusion to left side torso.
Employee was changing tickets in printer when a customer's truck bumped the printer enclosure bruising arm. Note: this injury was limited to one doctors visit and was first aid only until 1-31-05, She still had a sore arm so she is going to physical therapy, this is why this injury is being reported at this time.
While removing screen panel from screening deck, employee's glove caught on screen. Finger was caught between sliding screen panel and edge of screening plant. Left index finger required 6 stitches, employee returned to work with no restrictions.
Employee cut left thumb with utility knife while removing electrical tape from lugs in junction box of electric motor. Received 4 stitches in emergency room. During follow up doctors appointment, doctor applied more stitches internally. No work restrictions.
Failure to lock out tag out 480v electrical control cabinet. Employee was inside control cabinet while energized. He touched live ciruit with wet sleeve causing flash to occur. Employee has 1st and 2nd degree burns on his face, neck and right hand. Employee said he did not get shocked. He was treated and released at Snoqualime Hospital Emergency.
14' piece angle iron - fabricating steel structure overhead. Poor body position, over exertion - strain low back.
Employee was cutting conveyor belt with razor knife when the knife slipped and cut his leg above the left knee
FELL FEEDING ROCK BOX. THE ROCK BOX WAS PLUGGED W/ A LARGE BOULDER & 3 EMPLOYEE'S ASSESSED THE PROBLEM. LOCKOUT CONVEYOR LINE, CO-WORKER AT JAW TOWER AND FATALY INJURED EE TRIED USING A PRY BAR TO FREE PLUG & HE CRAWLED INTO THE ROCK BOX TWO TIMES, ON THE SECOND TIME MATERIAL ENGULFED HIM.
Employee was looking in hopper while doing maintenance, when he reached up for a hand hold he cut his left index finger on a sharp edge.
HE ATTEMPTED TO REMOVE SOME ROCKS THAT HAD BECOME LODGED IN THE FLY WHEEL OF THE JAW CRUSHER. WHEN HE MOVED A ROCK ANOTHER ROCK STRUCK HIS LEFT HAND PINNING IT TO THE FRAME AND BREAKING HIS MI DDLE FINGER. THERE WAS ALSO A LACERATION TO THE HAND THAT REQUIRED SOME STITCHES.
EE WAS ENGAGED IN CHANGING A BOTTOM ROLL ON A D9R, WHILE MOVING THE ROLLER THE FOAM PROTECTIVE COVER SLIPPED OFF THE SHAFT. THE ROLL BENT HIS LEFT RING FINGER.
INSTALLING TAKE UP FOR NEW CONVEYOR. TAKE UP WAS TOP HEAVY & TIPPED OVER ON LEFT HAND CRUSHING OR PINCHING LEFT INDEX FINGER, LACERATING MIDDLE MEATY PORTION, 6 SUTURES.
EE TRIPPED AND FELL BACKWARD EE TRIPPED ON A MATERIAL SAMPLE.
EMPLOYEE WORKING ON T-ROLLS IN TUNNELL, IN A HURRY, TRIPPED IN TUNNEL FALLING INTO A PIECE OF STEEL, LAC RT LITTLE FINGER KNUCKEL.
UNKNOWN ORIGIN DUST OR FORIEGN OBJECT FELL INTO LEFT EYE. EE WENT TO DOCTOR. REMOVED FOREIGN OBJECT WITH A DRILL & GAVE ANTIBIOTIC DROPS.

EE WAS REPAIRING A SCREEN SPRAYBAR, NOTICED AN IRRITATION IN LEFT EYE. THOUGHT HE WAS FLASH BURNED IRRITATION DID NOT GO AWAY. WENT TO HOSPITAL ON 4-9-03 TO HAVE PARTICLE REMOVED.
STEPPING OFF DREDGE ONTO EXTENDING LADDER TO RAILING, WHERE CATWALK WAS LOCATED. STEPPED OVER RAILING AND LANDED ON A PIECE OF ANGLE IRON TWISTING LEFT ANKLE.
EE WAS WORKING ON A GEAR BOX DRIVE FOR A WATER CLAIRIFIER. THE EE REMOVED A SECTION OF DECKING TO ALLOW ACCESS FOR OIL DRAIN BUCKET. EE STEPPEDINTO OPENING AND SPRAINED HIS RIGHT KNEE
