While EE was cleaning material off a catwalk a rock from an overhead conveyor fell and struck them in the back of the head as EE was bending over causing a laceration. The EE received staples at a local clinic to close the wound.

Employee was operating a front-end loader. Employee got too hot and this resulted in a heat stroke injury.

Individual was working on a bearing change. Individual jammed pinky finger.

Employee injured shoulder pulling cable

Employee lost balance and caught themselves with right arm. Employee hurt shoulder.

A rental crane rolled forward during the process to dismantle the machine. The boom of the machine struck the porta-john that employee was in.

While welding, a shop fan was re-positioned blowing dust in the victim's eye. The following morning, victim was in discomfort and was treated by a physician to remove a piece of rust and was issued a prescription eye drop.

Employee was washing under the supports under the KVS scrubber. Employee discovered a hole through his right boot. Puddle water got in his boot and contacted the top part of his right foot. He noticed irritation a few minutes later.

Employee was on top of a pneumatic trailer, working with lid number 2, when he fell off the trailer and landed on the ground. He sustained 3 broken ribs (left rear) and a broken left thumb.

The employee struck right knee on a metal table while dismounting a forklift resulting in a laceration requiring 4 staples.

Employee was helping with kiln brick removal and in the process got lime dust in his left eye. He was wearing company supplied safety glasses at the time of the incident. He tried to flush out his eye repeatedly while still at work and afterward. His eye was still irritated several days later when he was take to the doctor and given a prescription for eye drops.

Employee was shoveling when material became airborne and lodged under eye. Employee was wearing safety glasses.

Airhose broke in kiln causing airborne dust and debris to get into employees eyes causing burns.

Employee slipped and fell on muddy surface carrying hydraulic pump.

Suffered Post - Traumatic Stress

Employee was walking on the ground from one location to another and stepped on a small rock twisting his ankle.

Wet and muddy work area. Lime loader fell while loading truck.

Co-worker was hitting pin in old drill when a piece chipped of striking employee in the left jaw.

Employee bent over watching other replace motor in plant air.

Accident was reported on 3/23/07 as a strain to lower back will driving haul truck. Employee continued to work his regular job hauling mud until 4/3/07 when he returned to work that morning at 8:00am with a doctor restrictions until the 12th of April.

Pipe wrench slipped causing employee to fall to lower level.

Lime burns to eye

employee was cleaning brick out of fuller kiln, got lime dust in left eye

Employee was pulling water blaster hose to the srubber at the back of the fuller kiln.

Employee recieved test results of a lung abnormality in April 2006. He then went to a doctor and returned to work with restriction, we then removed him from his regular job as scaler and placed him on the crusher. We sent him to an occupational Dr. in Ft. Smith with results of no abnormality and no restrictions.

employee was replacing product screen at the kiln when he bumped a beam, while lifting the screen to another employee up above causing lime to fall on his face and in his eyes.

Raking coal ring out of the kiln, coal fell out hitting the end of the bar cause it to strike employee in right arm pit.

Employee lost footing on wet stairs grabbed hand rails to keep from falling causing strain to lower back.

Employee held up welding hood to inspect for cleaning, trash fell in eye.

Raking kilns when a large piece of coal ring fell off the discharge dam hitting the rake. The side of the rake struck him on the right side of nose resulting in stitches.

Cleaning fuller multicones. bumped shovel causing lime to fall getting in eyes.

Lime dust accumulated in the bend of his right arm while cleaning in penthouse. The lime activated with sweat and caused burns. The injury became infected and employee went to the doctor on 11/17/05.

Limescreen belt tore resulting in bogging down the belt and sreen. Employee shoveled material resulting in straining his abdomen and back muscles.

Employee was plant oiler at the time of injury. He stepped out of the oiler truck twisting his left ankle on August 26 2005.He has continued his regular work duties until April 25 2006 when is Dr. removed him from all work activity for 30 days. At the end of 30 days he will be reevaluated.

Restrained lower back replacing screen at crusher

Employee was operarting the skid steer cleaning out the loading floor. Quicklime on the floor was damp, steam vapors surrounded the skid steer. Operator then tried to get out of the loader and slipped. Using his left hand to break the fall, the employee's hand stuck in the quicklime.

EE WAS WELDING ON THE CHUTE AT THE FULLER KILN. SLAG WAS CHIPPED WITH A HAMMER AND A PIECE FLEW INTO HIS RIGHT EYE.

A LICENSED BLASTER SET OFF A SHOT IN THE 25 NORTH K INTO A PILLAR. AS THE SHOT WAS IGNITED, INJURED WAS DRIVING BESIDE THE BACKSIDE OF THE PILLAR ON THE HAUL ROAD. PART OF THE SHOT BROKE THROUGH EXPOSING THE EMPLOYEE.

A SMALL ENCLOSURE SURROUNDING A BLOWER FOR A PNEUMATIC CONVEYING SYSTEM CAUGHT FIRE. THE STRUCTURE WAS A METAL SIDING, WOODEN FRAME BUILDING WITH PLYWOOD WALLS. THE SOURCE OF THE FIRE IS UNKNO WN. THE FIRE WAS PUT OUT WITH THE PLANT WATER TRUCK. THE LOCAL FIRE DEPARTMENT WAS CALLED IN. NO ONE WAS INJURED.

WHILE WALKING DOWN THE HILL FROM CLOCKING IN, EMPLOYEE SAID HE WAS WALKING AND STEPPED IN A DITCH IN THE ROAD AND TWISTED HIS RIGHT ANKLE.

EE SAID HE WAS PULLING ON A COME-ALONG WHEN THE HOOK SLIPPED AND HE WAS THROWN AGAINST THE GUARDINJURING L. SHOULDER AND NECK.

EMPLOYEE WAS ATTEMPTING TO DRIVE A BEARING FROM A SHAFT WITH A SHOP HAMMER, WHEN HAMMER GLANCED OFF THE BEARING AND STRUCK HIS RIGHT FIRST FINGER.

EMPLOYEE SAID HE WAS STANDING IN FRONT OF THE FULLER KILN WHEN IT PUFFED WHEN THE COAL MILL STARTED BLOWING LIME DUST IN HIS EYE. HE WAS STANDING IN FRONT OF THE OPENING IN THE KILN HOOD AND H E HAD SAFETY GLASSES ON, DUST BLEW IN EYES.

THE OPERATOR WAS BAGGING HYDRATE IN 50# BAGS. ONE OF THE BAGS BURSTED OPEN ON THE BAGGER, BLOWING HYDRATE INTO HIS FACE AND INTO HIS EYES. OPERATOR WAS NOT WEARING SAFETY GLASSES WHILE BAGGING .

OPERATOR STARTED TO CLEAN OUT HOPPER ON COAL ELEVATOR. HE OPENED THE DOOR ON THE BOTTOM OF THE ELEVATOR AND HE THOUGHT IT WAS BOGGED DOWN WITH COAL. WHEN HE POKED IT WITH A BAR THE ELEVATOR BUCKET CAUGHT THE BAR FORCING IT OUT OF HIS HANDS. THE BAR HIT HIS LEFT LEG SHIN.

THE OPERATOR WAS PARKING THE MANLIFT WHEN THE FRONT END SUNK IN A HOLE THE BASKET SPRING BOARDEDTHE OPERATOR INTO AIR & HE HIT HIS RIBS ON RAILING AROUND THE BASKET
