After a wiper blade came loose on the screen discharge conveyor belt, employee and 2 other coworkers were attempting to secure the wiper blade's pivot arm to the belt frame with a wire. During the process, the wire came loose, and employee's hand became pinched between the crusher's frame and the wiper blade's pivot arm, resulting in a cut to the palm of their right hand.

Employee was operating a loader all day and there were some pot holes that they encountered causing bouncing and a sore back.

Employee was changing the liner on the HP300 cone on the newly acquired crushing equipment. During this activity EE was swinging a 12lb sledgehammer to loosen or tighten the mantle to the head. Employee had an MRI and on 1/29/25. EE was given the results that EE has a torn rotator cuff.

Employee was lifting a steel square tube off a flatbed, along with another employee. Employee felt some pain in EE's lower back.

Employee kneeled down to look at controls on a vac truck, EE felt EE's right knee pop. Conservative care until September - MRI was done. Then referral to ortho for MRI results. No tear, but the ortho Dr. ordered PT, making this a recordable injury, reason for delay in reporting. On 9/27 - PT was ordered but on 10-11 it was confirmed that PT was necessary along with injections.

EE was moving material from underneath a stockpile line tire of loader. EE came off a 1 foot edge of material causing the loader to rock from left to right. EE had a seatbelt on and as loader rocked, reached out to brace self. After the right tire went into the hole the left tire proceeded to go into a drainage trench causing the loader to rock.

While attempting to remove a race from the tail pulley shaft on the feed belt by standing on a pry bar to lift the tail pully from the frame, EE's foot slipped off the bar causing EE to fall forward to the ground and landing on left elbow. EE said that EE dislocated a shoulder when elbow/forearm contacted the ground causing pain in shoulder.

Employee was operating a loader. While driving along employee was twisting to keep an eye out for any hazards. As the loader went over a couple ruts in the dirt, it jarred the employee causing some lower back pain.

Employee was driving a loader across the yard with the bucket lifted, speeding, and hit a spillage pile on the flight side of loader causing the loader to go left on two wheels. When that happened EE jerked it back to the right side, hit the ground and the loader continued in that direction and fell over.

Employee was changing screens on top deck of wash plant and another crew member pulled the screen to the side pinching employee's finger. Employee received laceration to finger requiring sutures.

Employee was finished grinding on spring pads for the wash plant when EE had some discomfort in right eye. EE wore all PPE during the task. EE went to supervisor they went to doctor who in turn referred them to an Eye Doctor. Took EE to the doctor looked and removed a small metal shaving from employees eye.

Employee sprained back while doing work he had not done for 6 mos to 1 year.

Two employees were loosening the nuts for the mantle on equipment. Employee 1 was holding the wrench on the bolt and employee 2 was holding the ratchet on the bolt head, as employee 2 was moving the ratchet employee 1 lost his position and ended up pinching his fingers between the wrench and equipment. Pinched tip of left index finger requiring 2 stitches.

Employee states he twisted his ankle walking toward the crusher. Did not notify employer of injury until 10/31/11.

Two employees were changing the roller on the crusher belt. As one employee was trying to place the ear of the roller into the bracket the roller slipped out of his hand. The other employee was holding the other end of the roller and couldn't let go of the roller before it smashed his right thumb. Required 5 stitches.

Employee was adjusting a tail pulley with a one inch driver ratchet. He was pulling the ratchet toward him and the ratchet went into "neutral". Caused employee to smash his finger between the ratchet and the angle iron. Ratchet was determined to be faulty. Employee's 5th finger has fracture.

WELDING GUARDS ON CRUSHER. WELDING ELECTRODE STUCK TO IRON. PULLED TO FREE STUCK ROD AND ROD TORE HOLE IN ARM ONCE PULLED FREE.

WAS REPLACING BELT ON SHORT CONE AND FELL APPROX 18 INCHES.

EMPLOYEE WAS SERVICING THE CRUSHER AND HE FELL BANGING HIS KNEE ON THE WAY DOWN.

EMPLOYEE WAS OPERATING A DOZER, PUSHING UP FOR THE CRUSHER. SHE SAID THAT SHE HAD HER HEAD TURNED TO BACK UP WHEN THE DOZER HIT A BUMP, LURCHED AND STRAINED HER NECK.
