Lab tech was in preheater elevator when it stopped between the 4th and 5th floor and wouldn't move. Elevator technicians were called out to assist. Employee got out of elevator at 5:39 PM. No life threatening injuries resulted from this incident. Elevator tech stated the door board and edge sensor needed replacing. The elevator was taken out of service until repairs were made.
Emp lost grip on ladder between 2nd/3rd rung, fell approx 2 1/2 to 3 ft onto floor. Right knee hit floor. No medical treatment needed, no prescription, X-ray negative.
EE was opening a bag house door. The employee was using a wrench to loosen the fastening nuts, and when the nut broke loose, the employee left hand struck the frame of the door frame. The impact resulted in a laceration on the left index finger. The wound required 5 stitches to close.
Employee was changing silos by using a bar/pole to switch. Upon using the bar to switch silos he pulled and shifted his weight on his left knee causing a strain. Employee originally reported this as a first aid on 4/2/15 but came back the next day requesting to go to a doctor. The doctor prescribed restricted duties on 4/3 making this a reportable to MSHA.

During wear plate replacement; old plate slipped scraping employee's lower legs.
Employee slipped on a patch of ice while walking by the old raw mill. This was originally treated as a first aid but turned reportable when he went for a follow-up on 3/23/15 and was given an Rx for anti inflammation and scheduled physical therapy. No restricted duty was given.
EE was injured when trolley style manual chain hoist fell during coal mill roll replacement and chains struck him in the lower back. EE was x-ray'd with no impairments found. EE was seen by doctor and pain RX's prescribed and removed him from work for one week due to soreness and inflammation. EE in PT.
EE was pushing a wheelbarrow full of wires. When he went to get off the elevator the wheelbarrow started to tip because of the snow and he had to lift and push with extra force to get through snow. He felt a little pain in his groin area. Traveled back to Demopolis that night and pain got worse.
Employee became stuck at the 5th floor landing of the 1141 passenger elevator. Call was made to elevator company to retrieve employee. Employee exited elevator at 10:50 AM.
Contractor with Helsman Sweeney Company was entrapped in the 1141 passenger elevator due to elevator door inadvertently stuck in closed position. Contractor exited elevator at 11:20 AM.
Employee was aligning a pump to insert bolts when he sustained an elbow fracture.
Employee sustained left wrist sprain while changing out drive belts to air blower motor.
Employee was installing overhead rigging when the sling slipped and struck the employee above the left eye. The employee received two stitches to close the wound. The employee was wearing appropriate PPE at the time of incident.
EE was helping to unload bundle of rebar from truck standing on bed with foot against rebar while 2 others were pushing rebar off side. Assuming as bundle went off truck, rebar caught pant leg & he lost balance falling off truck with rebar landing on ankle/leg. Acute sprain & contusions to ankle & chest & abrasion to elbow. * Closed TXT Medial Mallelous Fracture with manipulation.
Elevator was temporarily stuck with miner between 4th and 5th floor of the 1141 Mill causing an entrapment. Miner was released without any harm at 10 AM. The stop switch was corroded and was replaced with new switch. Elevator was returned to full operation status the same day.
Independent Hauler slipped and tripped on steps to scales and fell 18 inches twisting left knee.
Employee was unloading RR car - when employee tried to open coal car door, it was stuck and the movement caused a pain between right shoulder and neck.
After installing and releasing finish mill access door employee went to raise the safety cage. When reaching for the hand crank winch, twisted his back resulting in a left lower lumbar strain.
Employee sustained a hernia while lifting a guard cover over the 945-14 screw.
Emp was closing a refractory door after clearing buildup. There was a bolt at the top of the door that had to be held up due to improper clearances in the frame work. While closing the door emp used his weight to close the door while holding up the bolt. The employee's thumb was in the pinch point. Thus smashing his thumb.
Employee was grinding sheet metal on bench grinder. Sheet metal kicked back horizontally and cut through glove on left hand. Employee suffered laceration to left palm requiring 9 sutures.
EE was carrying buckets of oil walking down a narrow hallway that was dark and covered with cement dust. EE tripped over metal object he did not see & landed on his left ankle. EE was asked by Cemex Lyons supervisor why he was limping and EE told him he had fallen. Personal Touch supervisor took him to dr. X-rays were done results not broke.
EE was working in hot area sweeping down tubes with hot materials inside the tubes. Before lunch break, EE began to experience cramps in arms and legs. He told supervisor who contacted cleaning manager/safety manager. Wet cloths were applied to back of neck and Gatorade given. He was taken in ambulance to local hospital treated for dehydration and released.
When removing door on rotary ball mill, the door lodged in the opening and employee went to dislodge without relieving tension, the load released and employee was pulled into the opening sustaining trauma including bruising and 2 cracked ribs.
Employee was traveling on pre-heat elevator when the elevator became stuck between the 3rd and 4th floor. Contractors mechanics were called to troubleshoot the elevator. They brought elevator down releasing the employee. There are elevator entrapment procedures inside the elevator that instructed the employee to contact management. The employee exited the elevator at 2:30 PM.
Employee was greasing equipment, it was very hot this day, when he felt that his left foot felt very heavy. He sat down & drank some water & noticed that his left arm was going numb. Ambulance was called & taken to hospital. Nothing has been determined what results are yet. More testing will be done near future.
Employee sustained back strain while removing sprocket off of shaft.
Employee sustained fractured rib when come-along hooked to air compressor shifted making contact against his left rib cage.
Elevator became stuck between the 2nd and 3rd floor entrapping an employee. No one was injured as a result of this incident.
Employee was trying to cut a .003 inch shim stock. He had the shim stock locked in a vise. He could not find a pair of shears but thought he could tear the shim stock by hand. He was not wearing gloves. As he tried to tear the material, his left hand slipped and lacerated the palm of his hand. He received stitches and was referred to a hand specialist.
Employee got dust in eyes while working on dust collectors.
Without checking to ensure elevator car was present, contractor stepped into elevator shaft and fell approximately 25 feet landing on top of elevator car.
Employee was cleaning return rollers on a conveyor belt with a poke bar. The conveyor belt was running. As the employee was scraping, the poke bar caught on the roller and jerked his right hand up jamming it against the frame of the conveyor belt. He broke two bones in his hand and received a laceration.
Miner was climbing down a fixed ladder and lost footing on the ladder. Miner's foot and leg went through the rung of the ladder causing him to twist his knee. Miner was believed to be in a rush causing him to lose footing on the ladder.
Replacing return roller on a belt conveyor, roller slipped from carrying pin and smashed employees finger between the roller and the roller.
Employee complained of pain in his wrist that began 2 1/2 to 3 years ago. The doctor diagnosed it as a sprain and prescribed a splint.
Employee was cleaning out a plug with a poke bar when he bent his thumb back. He was given first aid at that time but later, on 1/2/2014, stated a desire to go to the doctor. The doctor prescribed Naproxen and gave the employee a splint to wear.
Employee operating power washer when he lost his balance. The water from the washer hit his left foot penetrated the boot and broke through the skin. Surgery was performed to clean out the wound. He was placed in boot and given work restrictions
Employee walking down stairs his foot slipped off. Put his hand out to catch himself. Employee fractured his metacarpal on right hand. Meds - given splint and placed 0n lifting restrictions
Air lancing a bin out and a chunk fell and hit the lance causing him to hurt his arm

Employee operating water wagon; watering on a slope at quarry when lost control and it flipped over.
Employee was assisting in air lancing the coal chute. As he broke through the material in a downward motion, he jammed his left little finger and left ring finger against the top lip of the hood housing. This tore the two fingernails off an fractured the tip of his ring finger.
Employee was changing bags in a bag house. Dust had accumulated on his hard hat and on top of his goggles. When he removed his goggles to clear off the fog build-up, dust fell into his eyes. He flushed his eyes with water and went home. The next day he went to the doctor and was prescribed Tobrex and Pred-forte.
On 7/1/2013, while shoveling material under the finish mill belt, employee extended to lift a shovel of material and felt pull in lower back. He did not feel the injury was severe enough to go to the doctor and treated it at home. He felt it "tighten" up on 7/18 while picking up tree branches and visited the doctor on 7/22. He was placed on lifting restrictions and prescribed PT.
Was climbing a ladder and hit his head.
While walking from barge office to the barge bins, employee slipped in a muddy spot and his left arm extended up behind his head as he attempted to maintain his balance. (He did not fall). Employee experienced dull pain in his shoulder which was diagnosed in late August 2013 as a rotator cuff tear. Employee will undergo surgery for repair.
As the affected employee was climbing onto the 990 Loader he felt his right knee pop when he transitioned from the first step up to the second step. Employee felt pain and wanted to go to the doctor for evaluation. Doctor did an X-Ray which revealed fluid on the employee's knee.
