Employeee was leaving a lube trailer through a low door at Hanson Heidelburg AA Quarry in Grayson KY when employee raised up before clearing the door and struck head on the door frame. Employee was wearing PPE (Hard Hat) .

Employee was working on repair of a loader when EE fell off, hitting the ground and breaking the wrist.

Employee was operating a CAT 740B Haul Truck and hit a rut in the road, causing back pain. The injury was not reported to management until 9/9/19.

A Master Mechanic was servicing a D8 Dozer on the morning of 4/22/17. As the Master Mechanic stepped down from the tracks to the blade bar, right foot slipped on mud that accumulated on the bar, causing left foot to twist inward.

A delivery truck driver was loaded with stone and pulled forward. He called his truck owner saying he was having trouble breathing. 911 was called and our employees performed CPR till life squad arrived. He was transported to Christ Hospital where he passed two days later.

Maintenance employees were loading a feeder onto a flatbed trailer. The flatbed trailer had raised wheel wells. As the feeder was being lowered, a Maintenance Helper used his right hand in an attempt to keep the equipment from swinging. As the equipment lowered, the Maintenance Helper's right fifth finger was pinched between the feeder leg and the trailer fender.

Assisting mine employees to remove a bushing from 4 1/2 Simons cone crusher. Injured was holding a steel bar for another miner who was striking it with a sledge hammer. The bar slipped off and his right thumb was pinched against the bushing. Received 6 stitches and then returned to work.

Employee was unloading a roll of conveyor belt from the back of a van. When the employee and co-workers rolled the belt out of the van, the belt hit the ground and tipped sideways, striking the employee in the back of the left leg resulting in a bruise.

Employee was using a folding knife to push the locking pin of a wiring harness. The knife slipped off of the pin resulting in a finger laceration on the left hand requiring sutures.

While operating the Hitachi EX 1100-3 shovel loading haul trucks with stripping material, the operator noticed flames on the equipment. The employee exited the cab immediately and Eagle Crushed Stone's emergency action plan was initiated. MSHA and the local fire department were notified. The Hitachi EX 1100-3 shovel sustained significant damage.

While using a utility knife to trim rubber from a conveyor belt, knife slipped and struck employee on left forearm. Treatment required 4 stitches to forearm.

Employee was walking across a parking lot towards his haul truck when he slipped on a patch of ice and fell, resulting in sprains to his left knee and shoulder and abrasion to his left knee. Employee was placed on three days restricted duty, operator accommodated restrictions - no lost workdays were incurred as a result of this injury.

Employee turned to look at the gauge to see if it was working, while a tech was reparing the Cat Loader. Employee felt a twinge in his back.

Employee removed lid from top of anfo truck and lowered hand rail on top of truck while on top. He then stepped bacwards and caught his foot in hand-rail. apparatus causing him to fall on belly auger of truck and truck rear bumper. approx. 8'

Injured employee was kneeling in area between the cab and engine compartment on a Cat 988B Loader while performing maintenance to the loader. When employee stood up he heard a snapping noise in his left knee and felt that his left knee had popped out of socket. Initial medical review of this injury resulted in restricted duty with no lost time.

While setting an oxygen cylinder onto the back of a service truck, the employee pinched his middle finger between the cylinder and a spud bar which was laying on the floor of the truck, resulting in a fracture to the tip of the middle finger of his left hand.

While palletizing flat rock miner slipped/ tripped over another rock and injured left ankle. Conditions were wet that day.

The Dozer was running hot. EE took antifreeze to the Dozer. He let the equipment cool and then loosened the radiator cap to release pressure. When he turned the cap it blew off and sprayed fluid on him.

While sledging track pin in track a sliver of steel went into wrist of employee

EE had a stone fall on his leg in the palletizing area.

Employee was removing starter from 330 loader, starter slipped twisting left wrist.

Loading rock in bobcat bucket fell and broke in two pieces fell between bucket and bobcat and hit leg.

Employee was on scales. He got out of his truck and stepped on the guardrail they have for them to step on to get his load ticket. He turned to get back in his truck when his foot slipped on guardrail and he fell hitting his left side on guardrail and fracturing a rib.

Employee was setting up protable ramp on to the back of a box van truck for the forklift to run on - and felt a tearing in his arm.

Employee stepped off hazemag and scraped shin.

Employee was using 12 volt tester. The tester slipped off temp switch and came in contact with fan blade. The fan blade struck tester and drove blunt end into palm of right hand requiring 16 stitches.

Employee was operating a 2003 model 226 cat skid steer cleaning out between beltlines. He backed skidsteer upon existing stockpile with bucket raised cousin skid steer for overturn. He substained scretches and lacerations requires medical attention.

wHILE PECKING ON A AXLE GEAR WITH HAMMER A SLIVER CAME OFF AND STRUCK MACHINC IN UPPER LIP EMBEDDING INSELF IN LIP.

Employee was cleaning out around the wobbler feeder. He was attempting to flip big rocks out of the way and one fell againist employees ankle and foot. Employee's first doctor visist was first aid. On 10-29-04 employee went for follow up visit and he was given prescipriton medication due to infection in the wound.

A miner was installing a bucket tooth on the shovel, when he struck the tooth with a hammer and a piece broke off and imbedded in his right cheek.

2/24/04 OPERATING LOADER, BACK INJURY DUE TO JARRING OF EQUIPMENT ON ROUGH ROADWAYS. WENT TO ER ON SUNDAY 2/29 - STAYED OFF WORK 3/1 & 3/2. EE CALLED IN ON 3/2 AFTER SEEING FAMILY DR. & REQUESTED TO BE PAID FOR VACATION THE REST OF THE WEEK. RETURNED TO WORK 3/8. AT THIS POINT DID NOT DECLARE IT WAS WORKER'S COMP UNTIL HE STATES HE RE-INJURED ON 3/20/04.

OPERATING 980 G END LOADER-EE STATES THE ROUGH ROADS WERE BOUNCING AND JARRING HIM AROUND IN HIS SEAT.

EE WAS HELPING HIS CO-WORKER WITH SCREENS ON TOP DECK OF 6X16. HE WAS GRABBING THEM AS CO-WORKER WAS HANDING THEM DOWN. HE CAUGHT HIS RIGHT RING FINGER BETWEEN SCREEN AND EYEBOLT ON SHAKER. FI NGER WAS SMASHED, LACERATED, AND BROKE TIP OF FINGER. 3 STITCHES TO CLOSE WOUND.

EE WAS SHUTTING THE DOOR OF THE CAT TRUCK, HIS HAND SLIPPED OFF OF THE HANDLE & HE GOT IT CAUGHT ON THE LATCH SHUTTING HIS HAND IN THE DOOR.

A MINER WAS CUTTING A PLASTIC TIE STRAP WITH A POCKET KNIFE WHEN THE KNIFE SLIPPED AND LACERATED HIS WRIST.

OVERHEAD CAGE PINCHED MY LEFT RING & MIDDLE FINGER IN THE HINGE WHEN I WAS PULLING IT DOWN. THIS CAUSED A LACERATION TO MY MIDDLE FINGER AND PARTIAL AMPUTATION OF MY RING FINGER (HALF-WAY DOWN TO THE FIRST JOINT). THIS LEAD TO NO USE OF MY LEFT HAND & SEVERE PAIN. GLOVES WERE BEING WORN AT THE TIME OF THE ACCIDENT.

EE STARTED TO FELL PAIN IN HIS LOWER BACK WHILE STACKING BAGS OF LIME. THE EE WAS TWISTING HIS BACK WHEN MOVING THE BAGS.

WHILE EXITING THE LOADER THE EE MISSED THE BOTTOM STEP CAUSING HIM TO FALL TO THE GROUND. THE EESTATED HE HEARD A LOUD "POP" IN HIS KNEE. THE EE DID NOT SEEK MEDICAL TREATMENT UNTIL 5-28-03 AND THE ACCIDENT WAS REPORTED 5-29-03.

THE EMPLOYEE WAS CUTTING OFF THE TIP OF A TUBE OF CALK WHEN THE UTILITY KNIFE HE WAS USING SLIPPED CUTTING HIS HAND.

EE WAS PREPARING FOR LUNCH AFTER WORKING ON A CRUSHER. WHILE WRAPING UP SOME CABLE A BLOWBAR/HAMMER LYING ON THE GROUND FELL OVER ONTO HIS FOOT. HE WAS WEARING STEEL TOE BOOTS BUT RECEIVED A F RACTURE TO ONE OF THE METATORSAL BONES IN HIS LEFT FOOT.

AS THE EE WAS EXITING THE HAUL TRUCK HE CAUGHT HIS FOOT ON THE DOOR PANEL AND TWISTED HIS KNEE.ON 4/1/03 HE WAS PRESCRIBED PHYSICAL THERAPY.

EMPLOYEE ORIGINALLY FELL ON ICE WALKING ACROSS A ROADWAY. HE WAS FIRST DIAGNOSED WITH A CONTUSION WITH A NEGATIVE X RAY. THE EMPLOYEE COMPLAINEDTHAT HIS SHOULDER WAS IN PAIN. AN MRI ON 3-13-03 REVEALED A MAJOR TEAR IN HIS ROTATOR CUFF.

EMPLOYEES BACK WAS HURTING WHEN HE GOT UP THE NEXT MORNING AFTER WORKING NIGHT BEFORE. EMPLOYEE DID NOTHING OUT OF ORDINARY ON HIS NORMAL SHIFT.

EMPLOYEE FELT PAIN IN HIS LOWER BACK AFTER OPERATING A 50-TON CAT HAUL TRUCK. HE SAID HIS BACK WAS BOTHERING HIM. HE WENT TO THE DOCTOR ON 7/10/02 (LOWER BACK STRAIN.)

AFTER EE REMOVED LAST BOLT OUT OF A PAN IT SLIPPED PINCHING HIS LITTLE FINGER ON HIS RT. HAND. HIS X-RAY WAS POSITIVE FOR A SMALL HAIR LINE FRACTURE. TREATMENT CONSISTED OF TAPING ADJOINING FINGER TO INJURED DIGIT FOR SUPPORT.

EE STRAINED HIS LOWER BACK WHILE OPERATING A PITLOADER. HE RECEIVED CHIROPRACTIC TREATMENT.

LOADED DOZER TRACKS ON FLATBED TRACK FOR TRANSPORT. TRACKS WERE IN SPIRAL ROLLS, ONE END OF ROLL OPENED. EE WENT TO PULL OPEN END BACK TO ROLL, LOADER MOVED TRACK TO ADJUST HOLD. EE PINCHED TH UMB CAUSING LACERATION.

ROCK CRUSHED FINGER.

WHILE EMPLOYEE WAS DESCENDING, HE STEPPED ON A PIPE HIS FOOT SLIPPED AND HIS RT KNEE HIT THE CONCRETE SURFACE.

THIS INJURY WAS FIRST BELIEVED TO BE RELATED TO A CONDITION THAT IS PRESENTLY IN DISPUTE. EE EXPERIENCED LOWER BACK PAIN WHILE OPERATING A PIT LOADER. EE IS RECEIVING CHIROPRACTIC CARE.
