EMPLOYEE WAS AT WAREHOUSE SECURING PARTS TO DO A REPAIR. AS HE WAS CARRYING PARTS FROM THE WAREHOUSE TO HIS VEHICLE, HE SLIPPED ON THE ICE LANDING ON THE RIGHT SIDE OF HIS BODY. HE DIDN'T BELI EVE HE WAS INJURED AND CONTINUED TO WORK UNTIL 1-29-94, WHEN HE SOUGHT MEDICAL ATTENTION AND WAS DISABLED UNTIL 2-8-94.
EMPLOYEE WAS ASSISTING IN LOADING BARRLES OF GREASE ONTO THE DRAGLINE WITH THE USE OF THE OVERHEAD CRANE. HE SLIPPED AND FELL SUSTAINING A LACERATION TO HIS HEAD.
EMPLOYEE ORIGINALLY COULD NOT REACALL AN INCIDENT WHERE HE HURT HIS BACK. HOWEVER, AFTER 1 WEEK HE NOTIFIED HIS SUPERVISIOR THAT HE BELIEVED HIS INJURY OCCURED WHILE LOOSENING A HYDRACULIC TAN K DRAIN PLUG TO PERFORM AN OIL CHANGE. DUE TO THE EMPLOYEE MISSING 5 DAYS OF WORK [1/19 THROUGH 23], IT WAS FELT THIS SHOULD BE REPORTED. IT IS QUESTIONABLE AS TO WHETHER THIS INJURY IS COMP
EMPLOYEE WAS ASSISTING IN CLEANING SNOW FROM RAILROAD CARS, SO THE CARS COULD THEN BE LOADED WITH COAL. AS HE WAS SHOVELING SNOW THROUGH THE BOTTOM OF A CAR, HE SLIPPED AND FELL THROUGH THE O PEN DOOR, LANDING ON THE RAILROAD TRACES. HE CONTINUED TO WORK AND SOUGHT MEDICAL TREATMENT ON 1-14-94 AND BECAME DISABLED ON 1-15-94.
EMPLOYEE WAS PRYING UPPER HOIST CHAIN SHACKLE THROUGH WHEN THE PRY BAR SLIPPED CAUSING ;HIM TO FALL BACKWARDS SUSTAINING THE INJURY
FROM OPERATING EQUIPMENT FOR YEARS EMPLOYEE HAS SUFFERED CARPAL TUNNEL SYNDROME OF BOTH HANDS.SURGERY PERFORMED ON 12-14-93 ON THE RIGHT HAND.SURGERY SCHEDULED FOR 12-28-93 ON THE LEFT HAND.
EMPLOYEE WAS ASSISTING IN REPLACING SHAKER SCREEN IN PREP PLANT. AS NEW SCREEN WAS BEING LOWERED INTO PLACE WITH USE OF ELECTRIC HOIST IT BECAME LODGED BETWEEN THE SIDEWALLS. EMPLOYEE STEPPE D ONTO THE SCREEN IN ATTEMPT TO DISLODGE IT. ITINSTANTLY CAME FREE CAUSING EMPLOYEE TO FALL ONTO TWO SUPPORT ANGLES SUSTAINING INJURY.
WHILE OPERATING AN OVER THE ROAD TRUCK THE EMPLOYEE SUFFERED A STROKE ON 10-18-93 A WORKERS COMPENSATION CLAIM WAS FILED FOR THIS ILLNESS THIS CLAIM HAS BEEN DENIED UNDER PA WORKERS COMPENSATI ON AS WE FEEL THE ILLNESS IS NOT WORK RELATED
THE EMPLOYEE WAS DISMOUNTING HIS PIECE OF EQUIPMENT WHEN HE SLIPPED AND FELL TO THE GROUND.
THE EMPLOYEE FEELS EXPOSURE TO CONTINUOUS JARRING IN THE OPERATOR'S SEAT CREATED ON ACUTE BACK STRAIN. THE EMPLOYEE FIRST REPORTED THE INJURY ON 9-14-93 BUT DID NOT SEEK MEDICAL TREATMENT UNT IL 9-22-93. THE EMPLOYEE FEELS THE INJURY STARTED SOMETIME IN AUGUST 1993.
EE AND HIS SUPERVISOR WERE UNLOADING AN OIL BARREL FROM THE TAILGATE OF A PICK UP TRUCK INTO THE BUCKET OF A CATEPILLAR 966 HILIFT. AS THEY WERE UNLOADING THE BARREL THE TOP OF THE BARREL CROS SED OVER IT CENTER OF GRAVITY AND STARTED FALLING INTO THE HILIFT BUCKET. THE EE ATTEMPTED TO STOP THE BARREL AND HIS HAND BECAME PINCHED BETWEEN THE TOP OF THE BARREL AND THE HILIFT BUCKET.
THE EE WAS ASSISTING WITH THE REMOVAL OF AN ELECTRICAL POLE. THE EE WAS POSITIONED AT THE TOP OF THE POLE AS HE REMOVED THE NEUTRAL CABLE THE POLE BROKE OFF, BELOW GROUND LEVEL. THE EE WAS USI NG A LADDER AT THE TIME AND WHEN THE POLE BROKE THE POLE THE LADDER AND THE EE ALL TOPPLED TO THE GROUND.
THE EMPLOYEE WAS CLIMBING OUT OF THE CARGO AREA OF A PICK UP TRUCK. WITH HIS RIGHT LEG OUTSIDE THE TRUCK. AS THE LEFT LEFT LEG WAS BEING SWUNG OVER THE TAILGATE, IT STRUCK AN AIR NOZZLE,THAT WAS BEING STORED IN THE LEFT REAR STAKE POCKET OF THE TRUCK BED, CAUSING A LACERATION TO THE LEG.
AS EE W AS WALKING AROUND HIS SERVICE TRUCK HE WAS HOLDING ON TO THE BACK-UP ALARM. AS HE WAS GRABBING THE BACK-UP ALARM HE RECEIVED A LACERATION FROM THE BACK-UP ALARM. *THIS INJURY IS LATE B EING REPORTED DUE TO THE FACT IT WAS BELIEVED TOBE A FIRST-AID CASE, UNTIL THE MEDICAL BILL ARRIVED AND IT BECAME KNOWN SUTURES WERE APPLIED.
EE WAS ASSISTING MECHANICS LIFT A DEFFERENTAIL INTO PLACE ON THE DRILL UNIT AT THAT TIME HE NOTICED A PAIN IN HIS BACK, BUT CONTINUED TO WORK AND WORKED HIS NEXT SHIFT ON 11-19-93 THE PAIN WAS SEVERE HE SOUGHT MEDICAL ATTETION.
THE EMPLOYEE WAS INVOLVED WITH THE UNLOADING OF A TRAIN AT AN OFF SITE LOCATION, NOT ON MINE PROPERTY. THE TRAIN WAS BEING UNLOADED AT THE LEHIGH-PORTLAND CEMENT. CO. AT CEMENTON N.Y. WHILE U NLOADING THE TRAIN HE SLIPPED AND FELL AND STRUCK HIS RIGHT KNEE ON THE GRATING THAT IS PART OF HTE UNLOADING FACILITY.
EMPLOYEE WAS USING A PRY BAR TO LOOSEN FROZEN COAL FROM RAILROAD CAR AT AN OFFSITE LOCATION NOT ON MINE PROPETY SECTION OF FROZEN COAL HE WAS PRYING LEFT LOOSE CAUSING THE EMPLOYEES HAND TO BE COME CAUGHT BETWEEN THE PRY BAR & THE STEEL GRATING UNDER THE RAILROAD CAR TRAIN WAS BEING UNLOADED AT LEHIGH PORTALDN CEMENT CO IN CEMENTON NY INJURY NOT REPORTED UNTIL 03-19-93 BY EMPLOYEE
EMPLOYEE WAS WALKING ACROSS PARKING AREA TO HIS PERSONAL VEHICLE AT THE END OF THE WORK SHIFT, WHEN HE SLIPPED AND FELL. FREEZING RAIN HAD CREATED A SLIPPERY CONDITION.
EMPOLOYEE WAS ATTEMPTING TO CHANGE THE LEFT FENDER LIGHT OF THE BULLDOZER HE WAS OPERATING. AT THE TIME OF THE INJURY WEATHER WAS FREEZING RAIN. WHILE STANDING WITH HIS LEFT FOOT ON THE DOZER TRACK TRACK AND HIS RIGHT FOOT ON THE PUSH-ARM, HE SLIPPED AND FELL. HE FELL BETWEEN THE PUSH-ARM AND THE BULLDOZER AND BENT OVER FORWARDS STRAINING HIS RIGHT KNEE,
EMPLOYEE WAS DISMOUNTING A FRONT-END LOADER.AS HE STEPPED TO THE GROUND,HE STEPPED INTO A SLIGHT DEPRESSION IN THE GROUND,CAUSING THE INJURY.
EMPLOYEE WAS MANUALLY PULLING ON A TRAIL CABLE OF AN ELECTRIC DRAGLINE WHEN HE FELT HIS RIGHT KNEE POP.HE REPORTED THE INJURY AT THE TIME BUT DID NOT RECEIVE ANY FIRST AID TREATMENT OR MEDICAL CARE.ON 09-09-92 HE CLAIMS HIS KNEE STARTED BOTHERING HIM AND HE SOUGHT MEDICAL TREATMENT AT THAT TIME.

EMPLOYEE WAS GUIDING METAL PLATE INTO POSITION, HAND INJURED WHEN PLACED BETWEEN METAL PLATE AND SUPPORTING WALL.
EMPLOYEE WAS CUTTING OFF A PIECE OF STEEL, APP 7"X9", THAT WAS OVERHEAD OF HIM. WHEN THE PIECE OF STEEL FELL TO THE GROUND, IT BOUNCED BACK AND STRUCK THE EMPLOYE EON THE LEFT LEG CRETING A L ACERATION.
EMPLOYEE WAS LOOSENING A NUT ON A EYE BOLT THAT WAS TIGHT. AFTER HE LOOSENED THE NUT HE FELT A PAIN IN HIS NECK AND LEFT SHOULDER.
AS THE EMPLOYEE WAS IDSMOUNTING HIS FRONT-END LOADER, IN A DESIGNATED PARKING AREA, HE STEPPED ON A NAIL, WHICH PUNCTURED HIS LEFT FOOT.
THE EMPLOYEE BELIEVES THE NORMAL WORKING CONDITION OF SLIDING BACK AND FORTH IN THE OPERATOR'S SEAT OF THE EQUIPMENT HE IS OPERATING GENERATED HEAT WHICH AGGRAVATED THE INFECTED BUMP OR IN-GRO WN HAIR.
EMPLOYEE HAS BEEN EXPERIENCING ONGOING PAIN IN HIS LEFT SHOULDER. HE BELIEVES THE CAUSE OF THIS PAIN IS FROM THE CONTINUAL OPERATION OF USING HIS LEFT ARM, HAND AND SHOULDER TO STEER HIS FRONT -END LOADER.
EMPL WORKING ON A ROTARY DRILL. BENT OVER TO STAND UP A 7 7/8" DRILL BIT,FELT A SHARP PAIN IN LOWER BACK,CONTINUED TO WORK.1/2 HR LATER,NOTICED A ROLLER HAD FALLEN OFF THE V-BLOCK.AS HE DID HE FELT A SHARP PAIN IN THE MIDDLE OF HIS BACK.
EMPLOYEE WAS FUELING WATER PUMP HE TURNED AROUND & STEPPED INTO A HOLE WITH HIS LEFT FOOT CAUSING HIM TO LOOSE HIS BALANCE LANDING ON HIS RT ARM/HAND & STRIKING HIS CHEST ON A LUMP OF FROZEN D IRT
AT APPROX 12:15 PM EMPL WAS OPERATING ROAD GRADER IN YARD OF WAREHOUSE. AT APPROX 12:15 PM SUPERVISOR WENT TO GRADER TO EXPLAIN OPERATION & NOTICED EMPL SLUMPED TOWARDS STEERING COLUMN. NOT GE TTING RESPONSE FROM EMPL SUPERVISOR CLIMBED ONTOMACHINE NOTED SIGNS OF HEART ATTACK & SUMMONED ANBULANCE.
AT THE TIME OF THE INJURY THE EMPLOYEE WAS DISMOUNTING HIS PAN/SCRAPER. HE REACHED ACROSS HIS BODY TO PULL THE OPERATOR'S DOOR SHUT AND TWISTED HIS BACK. HE PROCEEDED HOME AND AFTER A PERIOD O F TIME HIS BACK BECAME WORSE.
EMPLOYEE WAS OPERTING A FRONTEND LOADER WHEN HE H IT A DIP IN THE ROADWAY CAUSING THE BUCKET TO STRIKE THE GROUND & THROWING THE EMPLOYEE AGAINST THE WINSHIELD INVESTIGATION REVEALED THE EMPLO YEE WAS WEARING A SEATBELT AT THE TIME OF THE INCIDENT
THE EMPLOYEE WAS PULLING CABLE FROM THE LAGGING OF THE DRAG DRUM OF A DRAGLINE. HE HAD PLACED HIS KNEES AGAINST THE LAGGING TO BRACE HIMSELF WHILE PULLING THE CABLES. A SHARP EDGE OF THE LAGGI NG CUT HIS LEFT KNEECAP.
EMPLOYEE WAS REMOVING SCREENING GRATES FROM THE INSIDE OF A TRUCK BED. THREE SECTIONS OF THE SIX SECTIONS WERE ALREADY REMOVED, WHEN A SECTION HE WAS STANDING ON BROKE LOOSE LEAVING HIM FDALL INTO THE TRUCK BED. ONE END OF GRATE WAS SUSPENDED BY AN OVERHEAD CRANE AND THE OTHER END WAS HOOKED ON A STEEL CHANNEL ATTACHED TO THE SIDE OF THE TRUCK BED.
EMPLOYEE WAS CHANGING PULL DOWN CYLINDERS ON 750 DRILL.THE EXHAUST PIPE WERE BELOW HIM WHEN A SPECK OF RUST BLEW OUT OF THE PIPES AND LODGED IN HIS RIGHT EYE.
EE WAS CHANGING TRACK ROLLERS ON UNIT 1461. AS HE WAS TIGHTENING ROLLER BOLTS A SPECK OF STEEL APPARENTLY CAME OFF THE BOLT AND STRUCK HIM IN THE RIGHT EYE. THE EE WAS WEARING SAFETY GLASSES A T THE TIME.
ASSISTING FELLOW WORKER REMOVING BOLTS IDLER IT CAME FREE CAUSING IDLER FELL PINCHING THE L INDEX FINGER BETWEEN THE CAP AND BOLT
TRANSFERRING 50 POUND TUBES TO TRUCK SHARP PAIN IN GROIN AREA
EE HAD DISMOUNTED FROM D-5 DOZER WENT TO THE BACK OF HIS MACHINE AND WAS BENDING TO HIS RIGHT TO LOOK FOR AN OIL LEAK WHEN HE FELT A PAIN IN HIS BACK.
EE WAS OPERATING CO VECHILE ON THE PINEY ROAD WHEN HE STRUCK A ROCK ON THE ROAD CAUSING THE INJURY TO HIS BACK.
THE EMPLOYEE WAS REMOVING A 6" WATER PUMP FROM PIT 537 WITH A HILIFT WHEN THE RIGHT FRONT TIRE SUNK INTO THE MUD. HE DISMOUNTED FROM THE HILIFT TO UNHOOK THE WATER PUMP AND AFTER HAVING DONE THIS HE WAS WALKING TO THE LEFT SIDE OF THE HIL LIFT TO MOUNT, WHEN HE STEPPED INTO A HOLE WITH HIS RIGHT FOOT SUSTAINING THE INJURY.
EMPLOYEE WS OEPRATING WELDING TRUCK ON HAUL ROAD, WHEN THE TRUCK STRUCK SOME RUTTS IN THE ROAD, CAUSING THE EMPLOYEE TO BOUNCE OFF HIS SEAT. HE LANDED WITH HSI RIGHT LEG COMPING DOWN ON HIS R IGHT TESTICLE, SUSTAINING THE INJURY.

EMPL WAS DISMOUNTING FROM ROTARYDRILL. HE SLIPPEDFROM BOTTOM STEP & FELL APPROX 3' LANDING ON BACK ON FROZEN EARTH. THIS IS THIRD RECURRENCE OF ORIGINAL INJ WHICH OCCURRED ON 01/05/88. PREVOU SLY EMPL HAS MISSED 40,81 & 154 DAYS.
EE WAS ASSISTING IN LOADING RAILROAD CARS AT LOAD OUT FACILITY AS HE WAS DROPPING EMPTY CARS IN PREPARATION TO BE LOADED, THE BRAKES FAILED CAUSING THE CARS TO STRIKE ANOTHER CAR WAITING TO BE LOADED. THE IMPACT OF THE CARS COMING TOGETHER CAUSED THE EE TO STRIKE THE TOP OF THE CAR HE WAS ON WITH HIS CHEST SUSTAINING THE INJURY.
EMPL WAS ASSISTING IN REPLACING FAIRLEAD BOLTS UON UNIT 1245. HE WAS TRYING TO HOLD AIR WRENCH & SOCKET IN PLACE WHILE COWORKER WAS OPERATING AIR WRENCH. NEW BOLTS WERE BEING TIGHTENED WHEN U JOINT ON AIR WRENCH BROKE STRIKING EMPL ON LEFTHAND.
EMPLOYEE WAS OPERATING HIS LOADER WHEN HE SMELLED WHAT HE BELIEVED TO BE BURNING OIL.HE STOPPED HIS MACHINE,WENT TO THE REAR OF THE MACHINE AND KNELT DOWN TO OPEN THE HOOD OF THE MACHINE WHEN HE FELT A SHARP PAIN IN HIS RIGHT KNEE.
