EE was operating a dozer (back blading) in a reclaim. area when he trammed over a section of shale rock causing him to be bounced in the cab jarring his neck and back. Lost work days due to the injury began on 5/1/12.

Preparing rod remove & install cutting edge. Operator engaged blade down causing crib block to flip up and strick employee on upper right side of the nose causing a laceration

While getting on the rock drill, the employee slipped on the cat walk and twisted his ankle

While putting anti freeze in the loader, the employee stepped on a loose metal plate and twisted his ankle.

Employee stated that while having his truck loaded, the loader operator dropped a big rock into the truck bed causing him to be jarred.

EE was dismounting endloader when he slipped on the ladder step and started to fall, his right hand became caught between the ladder rail and the machine.

Installing injector on 980H loader and adjusting valves. It was raining and ee wiped his face and glasses off with paper towel. Foreign body entered eye. Treated with opthamologist and given Rx.

Installing liner on D11R, using metal blocks to hold liner in place while welding. Using a 20 lb sledge hammer to remove the block, hammer slipped and hit employee in left lower leg causing contusion.

Technician was repairing D11; while using air hose, draped it over handrail of machine; turned around and when he turned back around hit mouth/teeth on air hose, breaking left canine tooth.

Employee was pushing in a shot when the dozer he was operating went over a rock. Employee stated that he was jarred.

EE was replacing a link on the miner track the pin came out and the track fell striking the EE on the right leg.

EE was operating a front end loader and was picking up a rock with the end loader the rock fell out of the bucket and caused the rear of the loader to fall down. This jarred the EE causing him to have pain in his lower back.

EE was getting on the drill when his foot slipped off the first step. EE fell backwards and struck his right elbow on the ground.

EE reported on 3/29/10 that on 3/20/10 while getting on the dozer he pulled a muscle on his back. EE stated that he did not report the accident at the time because he was going to treat the injury his self. On 3/22/10 he went to Cline Chiropractic for treatment.

This person has filed an occupational pneumoconiosis claim with Brick Street Mutual Insurance (West Virginia).

Cutting out old bed liner in 777D truck bed. Liner sprung upward and hit employee in left arm and face.

EE was walking to coal crusher his left foot got stuck in the mud. He went to step and twisted his left knee up under his body.

A small fire occurred on the tail pulley of the waste belt. The fire took more than 30 min. to extinguish due to limited access. The fire caused minor damage to belt conveyor and hyd. hoses. No injuries resulted from the fire.

This office has been notified of an occupational pneumoconiosis claim filed by this person on 5/31/09 with Brick Street Mutual Insurance (West Virginia).

Employee stepped on top of out-rigger to retrieve tool foot slipped and slid behind out rigger-employee lost balance and fell backward.

Employee was loading crib blocks into the school bus "man trip" when one of the crib blocks slid down the bus steps and struck employee on the knee.

Employee was exiting the cab when he slipped on the floor causing him to fall forward and hit his head and left shoulder on the gear box frame.

Employee notified management on 1/29/08 that on 1/26/08 while operating a rock truck, the loader bucket dropped and hit the truck bed. Employee stated that this hurt his head, neck, and back.

Employee stated that on 12-12-07 while operating the end loader, the bucket on the loader slipped off the corner of a rock and this caused him to be jarred. Employee did not miss work and did not go to the doctor until 2-9-08.

Employee stated that while chopping coal with the loader that he injured his back.

Employee left a message on voice mail 10/3/07 that he may not work the next day. A message was left on 10/4/07 that he had been scheduled for an MRI on his back. Emoloyee operates a D-10 Cat Dozer.

Employee stated that while operating a rock truck, he was bounced and hit his head on the cab of the truck. Employee stated he was wearing seat belt.

As employee was dismounting a D-11 dozer he stepped on a rock that was lodged in the step and fell backwards to the ground. He struck his head on a rock causing a laceration above his left ear.

The 785 Cat. rock truck was being loaded by the 5230 Cato shovel. The bucket of the shovel struck the bed of the rock truck. The jolt from the bucket striking the bed injured the truck operator's back.

EE was using air tool the pressureized hose "jumped" and caught hand between air hose & machine.

The dozer trammed too close to the edge of the spoil bank. The spoil bank failed and the dozer dropped over a steep embankment about 30 ft high.

PICKED UP EE AT END OF SHIFT. EE COMPLAINED SAYING BACK HURT DUE TO THE ROCKING OF HOE(MACHINE)

Employee was installing a track pin and while driving the pin in with a sledge hammer, part of the pin flew off striking employee in the left cheek; metal went through cheek into his left eye, damaging the eye lens and detaching the retina. Employee went to Bowman Gray Medical Center for surgery where the lens was removed;

EE RAISED THE ENGINE HOOD, HAND WAS LACERATED BY METAL PART.

THE TRAILER BED WAS RAISED TO DUMP THE COAL. THE HOIST JACK BROKE AND THE TRAILER BOX TURNED OVER. THE TRACTOR DID NOT TURN OVER.

EE WAS FUELING SERVICE TRUCK AND TRIPPED AND FELL

The APC Truck began to go up a hill, the brakes gave way and the truck began rolling back into an embankment, tipping the truck over. Before the truck tipped over John jumped out hurting his left foot.

Employee was under sitting front end loader remove the transmission drain plug. the wrench slipped out of the socket and he fell backwards ee's elbow struck a 4" rock.

Employee slipped and fell striking his head on hand rail.

loading holes with #50 wet bags, bent over to get a booster, could not get back up, hurt low back and left leg.

EMPLOYEE STATED THAT WHILE TRAMMING THE LOADER INTO THE PIT, HE RAN INTO A ROCK ON THE BOTTOM OF THE PILE OF SHOT ROCK. THE EMPLOYEE STATED THAT THIS JARRED HIS BACK AND NECK.

Pulled the bulk truck onto a shot, stepped down out of the truck, stepped in a hole, twisted right knee & struck left knee on the side of the truck

EMPLOYEE WAS DRIVING ON CUSTOMER'S HAUL ROAD WHEN HE HIT A LARGE HOLE IN THE ROAD CAUSING HIS HEAD TO HIT THE CAB OF THE TRUCK.

Was loading wet bags, went to pick one off the ground & back went out.

EE WAS DRIVING DOWN THE HAUL ROAD TO THE MINE WHEN THE VEHICLE WENT INTO A POTHOLE CAUSING THE EE TO JERK AND BOUNCE IN THE FIELD SERVICE VEHICLE.

Ee was under D11R inspecting roller and idler cap when debris fell into eye.

EE STATED THAT WHILE OPERATING A ROCK TRUCK, THE SEAT "BOTTOMED OUT" AND JARRED HIS BACK AND NECK.

EE WAS OPERATING A 777 CATERPILLAR ROCK TRUCK AND WHILE BEING LOADED, A PIECE OF ROCK FROM THE LOADER BUCKET FELL ON THE TRUCK BED. THIS BOUNCED THE EE. EE STATED THAT HE FELT A PAIN IN HIS NECK AND ARM. AS OF 5/24/04, EE IS WAITING ON RESULTS OF MRI EXAM.

WHILE PUSHING ROCK OUT OF NO. 12 COAL SEAM PIT, EE TRAMMED A CAT. D-11R DOZER OVER THE HIGHWALL. (DOZER FELL 120 FEET INTO SOFT MATERIAL; EE WAS NOT THROWN FROM CAB. SEATBELT CAUSED BRUISING TO HIP.)
