MECHANIC WAS CUTTING BOGIE PINS WITH AN ACETYLENE TORCH AND LEFT HAND WAS BY HANDLE OF TORCH. THE HANDLE OF THE TORCH WAS LOOSE ALLOWING OXYGEN TO BUILD UP IN THE GLOVE AND IGNITE.
EE ALLEGES HE SLIPPED ON THE BOTTOM STEP OF #2664 VOLVO TRUCK WHILE MOUNTING TO BEGIN HIS SHIFT. HE COMPLAINED OF PAIN IN THE RT. FOOT & WAS DIAGNOSED WITH A SPRAIN. HE WAS RELEASED TO WORK WITH RESTRICTIONS.
EMPLOYEE STATED HE WAS PULLING A HYDRAULIC PUMP FROM UNDERNEATH A TABLE WHEN HE FELT PAIN IN LOWER BACK AREA. HE WAS DIAGNOSED WITH A SPRAIN BACK - LUMBAR SPINE. (THREE DAY RESTRICTION).
EE STATED THAT WHILE DISMOUNTING THE MAINTENANCE LUBE TRUCK, HIS FOOT SLIPPED OFF THE BOTTOM STEP OF THE OPERATORS CAB CAUSING HIM TO FALL TO THE GROUND. HE STRUCK HIS LEFT ELBOW ON THE GROUND RESULTING IN A CONTUSION. BY DR.'S ORDERS HE WAS RESTRICTED FOR ONE DAY.
EE WAS WORKING ON DREDGE DRAINING WATER FROM GEAR BOX. AS HE ROSE UP AND BEGAN TO LEAVE THE PUMPROOM HE STRUCK HIS HEAD ON THE STEEL CROSS FRAMERESULTING IN 8 SUTURES. HE HAD TEMPORARILY REMOV ED HIS HARD HAT DUE TO THE TIGHT CONFINING SPACEOF THE WORK AREA.
EE HAD BEEN LOADED WITH MUD BY THE 1800 HITACHI SHOVEL. HE WAS STARTING OUT THE RAMP WHEN THE MUD TRANSFERRED TO THE BACK OF THE BED. THIS CAUSED THE 777 TO RAISE IN THE FRONT. WHEN THE TRUCK CAME DOWN, THE SUDDEN STOP CAUSED HIS HEAD TO WHIP FORWARD & STRIKE THE STEERING WHEEL (10 SUTURES REQUIRED).
THE EE WAS USING A PRY BAR TO DISLODGE A ROCK FROM THE BREAKER. WHEN THE ROCK MOVED THE BAR SLIPPED CAUSING HIM TO FALL APPROX. ONE FOOT. EE LANDED AWKWARDLY ON HIS FEET AND EXPERIENCED A SHAR P PAIN IN THE MIDDLE OF HIS BACK. THE EE MISSED ONE SHIFT BECAUSE THE DR. WASNT IN & HAS BEEN RELEASED FOR FULL DUTY.
EE WAS IN THE PROCESS OF INSTALLING THE FRONT DECK ON THE EUCLID R-190 TRUCK. A CRANE WAS BEING USED TO PERFORM THIS TASK. WHEN THE DECK FREED, IT SWUNG SLIGHTLY TO THE LEFT PINCHING EE'S LEFT HAND AGAINST THE HANDRAIL. (NO FRACTURE, CONTUSION ONLY).
EMPLOYEE HAD BEEN PRYING ON A LODGED ROCK IN THEMACHINE. HE LEANED OVER WITH THE PRY BAR IN HIS HANDS AND FELT PAIN IN LOWER BACK AREA. THIS BECAME REPORTABLE WHEN HE RECEIVED PRESCRIPTION MED ICATION. THERE WAS NO TIME LOST FROM THE JOB.
EMPLOYEE WAS STEAMING OFF THE CONCRETE FLOOR OF THE OIL CONTAINMENT AREA. HE APPARENTLY SLIPPED BUT DID NOT FALL ON THE SLICK SURFACE AND STRAINED HIS RIGHT KNEE. X-RAYS WERE NEGATIVE. THE EMP LOYEE HAS HAD SURGERY ON THE RIGHT KNEE FOR AN UNRELATED WORK INJURY.
THE EMPLOYEE WAS REPLACING HYDRAULIC HOSES ON NO6011 DOZER WHEN HE FELT WEAKNESS FROM THE HEAT. HE WAS TREATED WITH COOL WATER TO THE UPPER BODYAND TRANSPORTED TO HOSPITAL WHERE HE WAS KEPT OV ERNIGHT FOR OBSERVATION.
THE EE STARTED UP THE SPOIL RAMP IN #1969 TRUCK. WHEN HE WAS APPROX. 3/4 OF THE WAY OUT OF THE PIT THE ENGINE STALLED. WHEN THIS HAPPENED, THE TRUCK LOST ITS STEERING CAPABILITIES AND THE RETA RDER ALSO BECAME INOPERABLE THE TRUCK IS EQUIPPED WITH EMERGENCY STEERING. AS THE TRUCK DESCENDED BACKWARD DOWN THE RAMP, THE OPERATOR CALLED ON THE RADIO TO STAY OUT FROM BEHIND HIS TRUCK. T
EARLY IN THE SHIF, EE HAD POURED 2 OZ. OF SOLVENT INTO 4 EXHAUST PORTS FOR THE PURPOSE OF DETERMINING IF THE VALVES WERE BENT ON THE WATER TRUCK WHICH WAS LOCATED IN THE EAST BAY OF THE REPAIR SHOP AT COLUMBIA MINE. APPROX. 1530 HE WAS SITTING ON THE RT. FRONT FEND OF THE TRUCK REPLACING THE CYLINDER HEAD. HE ASKED COWORKER TO START THE TRUCK. WHEN STARTED FLAMES HOT FROM THE EXHAU
EMPLOYEE WAS ASSISTING TWO OTHER EMPLOYEES ROLLING A 20 FT LONG 48 INCH DIAMETER PIPE INTO POSITION. EMPLOYEE WAS POSITIONED ON THE END OF THE PIPE. AS THE PIPE ROLLED, A JAGGED EDGE OF STEEL FROM THE PIPE PENETRATED THE SIDE OF HIS LEFT HAND. FOUR SUTURES REQUIRED.
