Twisted and sprained right ankle while walking slope

A roof fall occurred in the #3 entry approx 10ft inby the mine opening. The fall area was cleaned and re-supported. MSHA examined the area on 1/20/2015. No injury occurred. Escapeways and ventilation were not disturbed. Fall was approx 10'x15'x3' & 3 bolts dislodged.

Twisted back removing ventilation curtain.

Electrician was installing a pump in a sump located outside on the hill ramp leading to the pit. He slipped into the sump while installing the pump, caught himself on the cable to the pump, tearing muscles in chest area. Did not report accident till 9-2-14, after visits to hospital

laceration of right hand upper palm and pinky finger on sharp rock

Cleaning around the tail roller of a de-energized belt, the load on the belt shifted, causing the belt to turn. This action pinched his left arm between the roller and the belt. Injury was a broken left forearm.

A piece of loose rock fell striking the left hand and lacerating the tendons to the left ring and middle fingers.

Prying down loose roof rock, which fell on right foot. Thus severely bruising the right foot.

A shard of flew off of drill steel while roof bolting at the face, and stuck in ee right pointer finger. Reported it to his fellow bolter and removed it and continued to work every day till Friday the 14th., where he reported it as infected and sought medical attention.

While traveling from face to face while doing his normal maintence job, the miner caught his head on a roof bolt while driving on a man trip, spraining his neck. Miner waited to report the injury till 1-10-14, thinking it would heal over time, did not seek medical attention till that past week.

While clearing a rock jam out of the bridge dolly, a rock flipped back, on the back of the operator's right hand and broke two small bones in his right hand.

Scoop operator was hand shoveling along the rib at the face of the crosscut B-4 production hole on the return side, when a piece of roof approx. 15" wide, 4' long and 10" thick fell from the roof in between bolts and struck the operator on the right shoulder, dislocating his shoulder.

While bolting a face cut. The bolter operator was in the process of preparing to drill the hole for the inside bolt on his side of the bolter. When the bit touched the roof, a piece of roof rock 1-2" thick, 2' wide and 5' long fell from the roof. It hit the support arm of the ATRS and slid down it, hitting the bolter operator on the right side of his head, shoulder and chest.

Hauling in crib block down the intake slope. The crib block banded together sitting in the scoop bucket was cut by metal arch roof support. The top layers of the crib block were frozen together and slid into the operators lap. Attempting to protect himself he raised his forearm in defense and received a puncture wound to his right forearm requiring two stitches.

While installing a roof bolt plate, EE roof bolter rested his left hand on the drill head of the bolter. While raising the drill head he pinched his thumb between the bolter head and the roof bolt plate that he was installing, causing a laceration to his thumb.

Operator of a CAT 980G loader had just taken a bucket of coal from the stockpile, when the loader quit. He turned to check the problem and found a fire under the hood. He proceeded to fight the fire with other mine personnel helping. The local fire department was contacted and arrived on scene to finish fighting. No one was injured

He was driving miner bits into the auger head of the miner, when something struck his left eye. He did not notice much discomfort and continued to work, even reporting to work the next day for his entire shift. Woke up the following day with discomfort and went to eye doctor. Tests found a sliver of metal in his left eye

While attempting to work loose a stuck drill steel in the roof, the drill steel became bound in the drill chuck with some side pressure. The drill steel snapped loose from the chuck striking the middle finger, lacerating the knuckle of his left hand.

Piece of shale (sharp) fell between bolts and lacerated EE's left ear.

While attempting to replace a splice on the 3-Rt Conveyor Belt, employee was attempting to re-cut the belt for a new splice and when closing the belt cutter, he pulled his left hand back and caught his middle finger against the belt cutter blade, lacerating the tip of his finger, requiring 3 stitches

EE was bolting in the #2 cross cut from Entry 4-5 in the 2 Rt. Room off of 2 Rt. Submain when a rock fell aprox. 18" X 24" X 2" thick. As he turned away he twisted his right ankle and was struck by the rock. Accident was not reported to management until 5/17/11.

EE was shoveling the rib in the 4-5 crosscut from #4 entry of the 1-Rt panel at Break #4 when a A-frame from the roof in between bolts (approx. 0-4" thk x 2' wd. x 3' lng.),fell, striking him in the back of the head and left shoulder.

Bolter operator was installing a roof bolt and felt a pain in his back as he straightened the roof bolt. Worked the rest of the shift with some discomfort.

While attempting to sledge a piece of steel into place, the sledge hammer head glanced off the piece of steel causing the handle to continue thru the swing and smashed his pointer finger between another piece of steel and the handle, causing a laceration.

Roof Fall - Intake slope entry, starting at the outby side of Break #5, approximately 28 feet long, 17 feet wide and 5 feet in height. No-one was injured and no equipment involved. Found at 7:37 am and reported to MSHA at 7:43 am. MSHA inspectors on site after reporting by phone.

Traveling in the intake on a mantrip, the miner was struck on the forehead above his left eye by a piece of draw rock that fell from in-between roof bolts as he traveled by, causing a laceration which required four (4) stiches.

While tramming crawler bolter, operator's left thumb became pinched between the bolter and the mine roof, when the machine teetered over a high spot in the mine floor causing the back of the machine to pivot up against roof. A laceration occurred requiring 4 stiches.

Operater of a 980F Cat loader was checking a possible problem with his loader. He slipped and fell off of loader, breaking his left ankle.

Roof Fall: Intake slope entry between cross-cut 4 and 5 approximately 40' long, 18' wide, 5' in height. No one injured, no equiptment involved. Found on pre-shift examination.

Operating the scoop from the opers. compartment with his right hand, looking toward the bucket, his left arm was in such a way that his hand was hanging out left side of the oper. compartment. When he started to turn the scoop he pinched the fingers of his left hand between the front stop block and the frame of the back half.

Roof fall 3-Rt. Submain B-4, at face from initial 3-4 pushover thru intersection of # 3 entry into 2-3 pushover, stopping before intersection of #2 entry. Fall 18'W X 90'L X 7'H. Sandstone channel and associated clay veins come down into roof strata. No one injured.

Outby roof fall Crosscut 11 of #4 Entry, 1-Rt Sub-main approx. 15' X 20' X 10' in height, no one injured, no equiptment involved Fall partially blocked primary escapeway and ventilation was not significantly affected.

Roof fall on section at x-cut 4 from right rib of #4 intersection thru #5 &6 intersections into pushover toward #7 intersection, approx. 100" long, 19 feet wide, 6-8 feet in height above anchorage, due to horizontal stress. No one injured, no equiptment involved. Area dangered off, section abandoned. Moved to new section location.

Roof fall last open cross cut between 3&4 entries of 1-right sub-main in the primary emergency escapeway, partially blocking escapeway. Did not affect air. No one injured and no equipment involved. Area cleaned, rebolted and prop-setter posts added for additional roof support.

Roof fall in #5 entry (intake & primary escapeway) at X-cut 67 of 2-Right Sub-main, approx. 18' wide, 50' long above roof bolt anchorage (60"). No one injured, no equipment involved. Ventilation & Primary escapeway re-established in adjacent entries.

Roof fall 3-Left Submain #4 Entry Crosscut #4, 30 feet in lenght, 18 feet in width, and 4 feet in height.

The truck driver slipped and fell into a ditch that was being excavated from which he was hauling away the materials being removed to make the ditch, dislocating his left shoulder.

Roof fall #3 entry between break 17 and 18 of the 2-Left Submain, initial fall 20' wide, 40' long and 5' in height above bolted anchorage(42") No one injured, fell on conveyor belt and belt structure. Did not affect ventilation or escapeways.

A piece of draw slate approx. 3" thick approx. 6'x6' seperated from the roof while he was bolting the roof between the face,rib/ATRS knocking him down and pinning his head (only) to the floor within his hard hat.Took to hospital for treatment,cat-scan found no injuries,just slight bruise to left temple area,rtw

Roof fall 3-left panel #2 entry intersection crosscut #5 outby 30',total length approx. 50', 5' in height, no equiptment involved, no one injured.

Roof fall in 1-Left Submain Crosscut #1, #2 enrty intersection 20' wide, 40' long, 5' high from wall in 2-1 over to 2-3. just above roof bolts. No one injured,

Miner was throwing belt structure into a scoop bucket, when the bottom stand of the structure he threw bounced off the bucket of the scoop and hit him in the left eye, causing swelling around the eye socket and redness in the eyeball.

Roof fall Intersection of #4 entry of the 1-Left Submain at break 11 . Approximately 30 feet long 20 feet wide 8 feet high. No one injured, no equiptment involved.

Roof fall, face of #4 entry of the 1-right submain at what would be the intersection of the #9 x-cut back outby to just inby the #8 x-cut intersection. Fall approximately 40 feet long, 19 feet wide, by 6 feet high. No one injured, no equiptment involved.

The mine was idle for a four days over holiday. A small roof fall near the working face occurred due to horizontal stress. It was discovered by the pre-shift examiner. Nodamage occurred. Noone injured. The fall was cleeaned up and bolted

Roof fall Break 49 crosscut between #3 and #4 entries of the North Submain above normal roof bolt anchorage. Did not affect ventilation, no equipment involved, no one injured.

Roof fall in #2 entry between Break #9 & #10, approx. 40' x 20' by 11' high. Does not affect ventilation, no one injured and no equipment involved. Found by employee during his preshift examination.

Employee was helping do a belt move on section; when he reached to pick up a stand for low style belt structure, throwing it into place he felt a pain in his lower back. Injury to lower back.
