LEAD ON FRAME [...] & HAND WAS IN CLOSE PROXIMITY. HE WAS WORKING ON A LIGHT SWITH. ACCIDENT OCCURRED WHILE TROUBLE SHOOTING ON SWITCH. HE TEMPORARILLY TOOK GLOVES OFF TO SWITCH AMPUNATOR WHEN INJURY OCCURRED.
FAULTY TOP. TOP FELL IN NO. 4 ENTRY APPROXIMATELY 400' OUTBY FACE. FALL WAS ABOUT 20' INBY SPAD 54 + 20'. OUTBY SPAD 54 IN INTAKE. FALL DID NOT ENTERFER WITH TRAVEL OR PRIMARRY EXCAPEWAY.
CARRY ROOF BOLTS FOR ABOUT 2 BREAKS. SLIPPED & TWISTED BACK.
SHUTTLE CAR WAS LEAVING MINER WHEN IT HOOKED THEMINER CABLE PULLING EE'S FOOT FROM UNDER HIM EE POSSIBLE INJURED HIS BACK. ALSO HIS RIBCAGE WAS INJURED COULD BE BADLY BRUSIED.
MINER IN #3 HEADING; MINER WAS FINISHED WITH CUT & STARTED BACKING OUT; ONE OF THE CAT TRACKS HUNG, CAUSING THE MINER TO SWING INSTANTLY; THE OPERATOR GOT HIS HAND PINCHED BETWEEN THE MINER & THE RIB; THE REMOTE BOX & HIS HAND WAS CRUSHED; HIS HAND WAS BADLY BRUISED WITH SEVERAL LACERATIONS.
THE OPERATOR OF THE ROOF BOLTER SWUNG HIS DRILL BOOM TOWARDS THE RIB. HIS FOOT BECAME WEDGED BETWEEN THE BOOM AND SHALE ROCK. HIS FOOT WAS BROKEN.
ROCK FELL OUT OF TOP AND HIT EEE ON THE HEAD & SHOULDER.
STAND ON POT OF BOLTER PUTTING BOLT IN HOLE, SLIPPED OFF OF POT AND HURT BACK.
STEPPED ON CRIB BLOCK ANS TWISTED LEFT ANKLE.
RIB ROLLED & HIT EE ON TOP OF SHOUDER (MIDDLE)
HAD WIRE HANGING CABLE, WIRE STUCK IN LEFT EYE.
RIB ROLLED AND HIT HIM IN THE BACK.
HIT A HOLE AND PUSHED HIS HEAD AGAINST CANOPY.
SC CAR OPERATOR RAN OVER MINER OPERATORS RIGHT FOOT.
HOISTING BELT TOGETHER TO MAKE BELT SPLICE. HAD A MAN ON EACH SIDE OF BAR. HOIST DID NOT CATCH. ONE MAN LET LOOSE OF BAR AND HIT INJURED IN CHEST.

ROOF FALL 1 BRK INBY 660 #3 ENTRY MAINS 170" LONG18' WIDE 6' TO 22' HIGH. FELL ABOVE ANCHORAGE OF ROOFBOLTS. TOP IS SHALE AND VERY LAMINATED. NO ACTUAL WAY TO SUPPORT. ROOFBOLTS WERE DOING THI R JOB.
ANOTHER EE AND EE WERE PULLING BELT OFF AND THE BELT PINNED EE AGAINST RIB CATCHING HIS LOWER RIGHT LEG.

IT FELL AT SPAD #531 IN #3 ENTRY ON #4 BELT LINE. 80 FT. LONG, 18 FT. WIDE AND 5 FT. THICK. USING 5 FT. RESIN BOLTS.
A FALL OCCURRED AT VIC OF SPAD 608 ON 6-21. PREVIOUSLY REPORTE WITH OTHER FALL ON 10-21-96.

ROOF FALL FELL ABOVE ANCHORAGE 4' BOLTS WERE USED AS THE PRIMARY SUPPORT METHOD FELL WAS 0 TO 7 ' IN THICKNESS. THE FALL WAS LOCATED IN THE 1 ENTRY OF THE NORTHWEST MAINS IN THE INTERSECTION B ETWEEN SPAD 383 AND SPAD 393. FURTHERMORE THE FALL WAS OVER 800' FROM THE ACTIVE FACE.

ROOF FALL 0 TO 3 FT. 20' X 30' IN NO. 7 HDG. 1 1/2 BRKS INBY SPAD S3 PRIMARY ESCAPEWAY. TOP FALL 3 FT. THICK. IT WAS BOLTED WITH 4' CONVENTIONAL BOLTS. CRIBBED AND TIMBERED OVER FALL WITH IN 8 HRS. OF FINDING FALL. SECTION WAS IDLE. 3400 OUTBY FACE.

ROOF FALL INTERSECTION AT SPAD 58 IN NO. 6 ENTRY. APPROX 500' OUTBY FACE AREA. TOP FELL AT POIN OF ANCHOR 7' CONVENTIONAL BOLTS.
ROOF FALL FELL ABOVE SUPPORT--TOP FELL 4' AT POINT OF ANCHOR (4' BOLTS). NO. 5 ENTRY 20' OUTBY SPAD 59. 30' LONG, 18' WIDE.
LIFTING TIMBER TO SET ON ROADWAY, SAID HE HURY HIS LOWER BACK.
PULLING CURTAIN DOWN CAUGHT RIGHT HAND AGAINST ROOF BOLT, CUT AND BRUISED RIGHT HAND.
HIGH WALL COLLASPED TRAPPING EMPLOYEE FOR APPROX 4 HOURS. HE WAS NOT INJURED. A DECISION OF 4' ABOVE WILL BE MADE AFTER FURTHER INVESTIGATION. WE HAD TO REOPEN WITH IN 10 DAYS.
IN NO.4 FACE MINER WAS BEING MANEURING FROM LEFT TO RIGHT INDIV. SLIPPED MINER HEAD HIT LEG AND BROKE LEFT LEG - THE HEAD ON MINER WAS NOT RUNNING
