The EE was using a chain ratchet to pull bracket back in place. The ratchet chain slipped causing material to hit the side of the injured employee's face and eye.

He fell on the tracks and landed on his right side. On the day of the incident, he finished his shift, bu awoke with his right arm stiff and swollen and his right hip and wrist sore.
Employee was pushing rock on drill bench, rock became caught under blade raising dozer off ground and dropping suddenly, operator felt pain in lower back.
Cutting metal platting plate flexed upwards strking miner in the face injuring nose, and mouth area requiring stiches.

Injured employee was using a hydraulic boom to hold a hydraulic pump in place on a D 11 dozer. The boom control was beside him on the dozer, when he inadvertently activated the lower switch and lowered the boom onto his back.
Employee stepped down from a dozer onto an object resulting in injury to right foot. Diagnosed as Plantar Fasciitis.
Employee tripped on fuel hose while fueling his truck and fell against side of pickup truck.
As employee was exiting from under a 992G endloader, he raised and struck the access ladder with his head.
Employee was pushing shot rock to the 1800 excavator when one track of the D 10 bulldozer he was operating climbed on top of a large rock. When backed off the rock the dozer fell quickly causing the employee to hit his head and shoulder against the cab of the dozer.
Employee was pulling up access ladder on 1800 Excavator, when ladder stopped abruptly. After working approximately 30 minutes, the employee started hurting in his neck area, then radiating down his leg. Employee worked until dinner shutdown, notified foreman of pain in his neck and leg. Employee was the transported to hospital by ambulance.
Injured employee was operating a 992G endloader loading rock trucks. He was loading parallel with the shot, undercutting the shot material and some rock rolled into the cab resulting in contusions to the left hand and right knee. He had been instructed on two previous occassins to load perpendicular to the shot material.
Injured employee was using a crane to hold a step on a 992G loader. The hook slipped, catching the employees left thumb between the hook and the step, causing a laceration to the thumb.
Employee was replacing o-ring between wheen and tire ring lock when the slate bar he was using to pry the ring away from the wheel slipped resulting in the employee's finger being pinched between the wheel and lock ring.
The boom on the powder truck was swung in the wrong direction striking the employee in the head and shoulder.
Employee was running a D5G Dozer. Was working on underbrush in the valleyfill, was helping the 330 excavator, when a rock got under the dozer and turned him over. He went over a 20' ledge landing on its top, then rolling back onto its tracks. He has a small cut to the right side of his head and some pain in his left shoulder.
Employee was pushing with the D5 Cat Dozer. While backing up, he ran into a tree causing the dozer to shake very hard. He said he hit so hard it caused pain in his neck. He didn't feel like he needed to go have it checked, he just applied heat to it, said he just wanted to file it in case it got worse.
