Mostrando los 16On the date of 2/7/2022 at 1:30 PM, EE came to me with an injury to EE's finger. EE said that EE was trying to turn a pump pulley which resulted in EE's finger being rolled up in the drive belt. Pump was de-energized and guard was removed to repair. Report was made at the time of incident and EE was driven to medical center where EE was treated for injury.
Repairing tailgate latch on RD800 Mack truck, accident happened when removing the cribbing that was installed to provide safety while repairs were made. The operator and help pulled back to remove crib, operator failed to move hand and was pulled into latch and crushed fingers.
Traveling uphill and truck lost control and overturned on hill.
Prying a rock out the feeder with a metal bar and strained back.
Loading rail car and removing a hook from car. Hook fell on his hand.
Walking & struck top of head on a low beam.
A catwalk had been damaged by coal truck and was barricaded off with metal straps on both ends to prevent entrances on either end. He went over straps, grabbed handrail, it broke and he fell backwards and hit his head on main belt structure.
Three employees were pulling off pulley sheave on crusher. They used pusher bolts but it wouldn't come off. Got wheel puller & two employees tightened wheel puller while the other employee applied heat with a torch. Never showed any signs of movement - just jumped off all at once and edge caught one employee in the forehead at the hairline.
Torque arm on speed reducer in tunnel came loose. EE turned it off to work on it, but didn't lock it out. Another EE turned it back on.
EMPLOYEE WAS WORKING @ LOADOUT USING A PIPE WRENCH TO TIGHTEN BELT ON CRUSHER. WRENCH SLIPPED PINCHING HIS PINKY FINGER BETWEEN THE WRENCH & A PIECE OF ANGLE IRON.
Employee was working in preparation plant. A chain was wrapped around a beam overhead. Employee pulled chain and it struck him on right cheekbone.
EMPLOYEE HAD FINISHED FUELING AND GREASING DOZER. HE WAS COMING BACK DOWN OFF TRACKS AND SLIPPED AND FELL OFF DOZER.
EE WAS PULLING A TAIL ROLLER WITH CHAIN ATTACHED TO A RED DEVIL. CHAIN BROKE AND STRUCK LEFT HAND - INJURING LEFT INDEX AND MIDDLE FINGER.
EMPLOYEE WAS ON FINES BELT INSTALLING GREASE HOSES ON BEARING; THE BELT WAS TURNED ON BY MIS-TAKE AND THE EMPLOYEE FELL APPROXIMATELY 15-20' TO THE GROUND.
EE WAS CLIMBING LADDER TO CHECK SCREENS ON VIBRATOR WHEN HE FELL - APPROX 6'.
