While replacing a cooler on the Atlas Copco D65 drill, the Miner was underneath the drill securing the bottom bolts. Dirt fell from the machine and struck the Miner in the face resulting in a a paint chip in eye.

While walking around the front of the loader to talk to the foreman, EE collapsed. No trip, slip or fall was attributed to the collapse. An ambulance was called and it was later learned that EE's hip was fractured and that EE has been experiencing pain for at least one month prior to the incident.
While dismounting the Komatsu D375 dozer, the Miner slipped from the push arm causing them to fall and break a bone in their hand on the ground.
While operating a Komatsu HD785 haul truck during loading operations, a large rock fell from the Komatsu PC3000 backhoe that was loading the truck. The rock struck the tail end of the truck bed causing the front of the truck to bounce on the ground. The miner jammed leg causing discomfort to back and leg.
While helping the welder spread out a tarp for protection from the rain, Miner was splashed in the face causing a small metal shaving to get into the left eye. Miner did not immediately seek medical attention but later had it removed by a specialist.
While troubleshooting the wiring for the rear lights on a Caterpillar 992 loader the employee slipped and fail when crossing from one side to the other.
Employee was readying bulk ANFO truck in preparation for loading blast holes. A hydraulic line ruptured at the hand controls. The employee raised his arm and stepped back tripping in a frozen tire rut and fell to the ground. The fall caused strains and contusions (lumbar, left hip, left knee, common peritoneal parenthesise).
Contractor employee stated he brought the John Deere backhoe to the bag hours and was waiting form co-worker to return. He noticed water droplets coming from the discharge end of the rotary air valve. He went to investigate. He reached up into the discharge opening of the rotary air lock valve that was running and injured two fingers on his right hand.
EE fell down the steps of the processing plant. EE fell straight down on his back broke 2 vertebrae and 1 rib.
EE was repairing parking brake on haul truck. His hand was on drive shaft when operator accidentally put truck in gear. EE's hand was pulled into drive shaft which jerked his entire right arm in a circular motion. EE refused first aid and drove himself to local ER He was treated and released.
The plant operator stopped the plant feed belt to make a repair. The operator & another ee, the plant runner forgot to turn off a torch on the plant feed belt that he left unattended. The belt caught fire and was put out in less than 10 min. 2 ee went to the hosp to be checked out for smoke inhalation and returned to work in a few hours.
Mechanic was installing a motor in a water truck. Upon installation, the motor got stuck in the engine compartment. The mechanic was prying on the motor to free it when the motor shifted pinning his hand between the motor and frame. He received 7 stitches in his right hand.
Employee was cleaning floor with a wheelbarrow and pulled his back when the wheelbarrow tipped to the side with material in it and he tried to stop it from upsetting.
Employee slipped on ice exiting crane while working around shovel - 9W dragline.
Employee exited the loader cab and tripped over the fuel cap on the deck outside the cab and struck his knee on the rail/deck. Employee was taken to the hospital for x-rays and has not returned to work as of this date. Carelessness of the employee contributed to the injury.
EE WAS ASSISTING ELECTRICIAN INSTALL A NEW MOTOR AT THE FLOOR SUMP PUMP. WHILE THE ELECTRICIAN WAS LOWERING THE MOTOR WITH CHAIN FALL, INJ EE WHO WAS STANDING APPROX 3FT AWAY, RUSHED OVER TO THE MOTOR AND PLACED HIS FINGERS BETWEEN THE MOTOR & ITS BASE FOR NO APPARENT REASON.
