ASSISTING OTHER ROOF BOLT CREW- HAD HIS LT. HAND LAYING ON THE STATIONARY PART OF THE ROOF BOLTER. WHEN THE OPERATOR HIT THE EXTENTION HANDLE TO SLIDE THE HEAD BACK IN, IT MASHED HIS LEFT RING FINGER BETWEEN THE TRAY & BODY OF THE BOLTER. BROKEN FINGER AND TORN LIGAMENTS ON THAT FINGER.

THE INJURED WAS OPERATING A LOCOMOTIVE TO PUSH A ROOF BOLTER BACK TO CONNECT IT TO THE SLOPE HOIST. THE TRS PAD ON THE HELPERS SIDE STRUCK A STEEL LEG CAUSING THE BOARDS AND ROCK ON TOP OF THE M TO FALL FROM BETWEEN THE BEAMS STRIKING THE INDIVIDUAL.
INJURED HAD COMPLETED ERECTING AN OMEGA BLOCK STOPPING. AS HE WALKED TOWARD THE MATRIP FOR SHIFT CHANGE HE FELT IRRITATION IN HIS RIGHT EYE. AS TIME PASSED HE HAD TO SEEK MEDICAL ASSISTANCE TO HAVE A PIECE OF FIBERGLASS REMOVED FROM HIS EYE. HE HAD RUBBED HIS EYE FREQUENTLY AFTER IT HAD BECOME IRRITATED.
EE WAS SEATED IN A MANTRIP OPERATORS COMPARTMENT WHEN THE MANTRIP INBY HIM, WHICH WAS BEING MOVED BY ANOTHER WORKMAN, INADVERTENTLY WAS PLACED INTO REVERSE TRAM AS THE OPERATOR SET THE BREAKER . BEFORE THE OPERATOR COULD STOP THE MANTRIP IT HAD STRUCK THE INJURED'S MANTRIP. BOTH MEN WERE SEATED.
EE WAS CARRYING 50# BAG OF ROCK DUST THROUGH A MANDOOR WHEN HE FELT PAIN IN HIS LOWER BACK. DID NOT BEGIN MISSING WORK UNTIL 3/5/02.

AN UNINTENTIONAL ROOF FALL OCCURRED AT 53 BLK ON C-MAINS SECTION IN X-CUT 6-7 AND FACE OF #7. TOP IS VERY SOFT UNCONSOLIDATED "SLICKEN SIDE" ROCK. DEMINSIONS: LENGTH- 60', WIDTH- 18', HEIGHT- 6-12'. SUPPORT: 5'RESIN.
INJURED STRAINED LOWER BACK WHILE UNLOAIDNG ROOF BOLTER BITS FROM BED OF PICK UP TRUCK. BEGAN MISSING 1-22-02.
INDIVIDUAL WAS DOING TORCH WORK IN THE BAG OF THE FEEDER. ON TWO OCCASSION SPARKS WENT DOWN HIS CLOTHING CAUSING HIM TO JUMP OR EACH TIME THIS OCCURRED HE STRUCK HIS RT KNEE ON THE FEEDER. THE KNEE BECAME SWOLLEN AND PAINFUL OVER THE NEXT DAY. HE WORKED ON 1-18-02 BEGAN MISSING ON 1-21-02.
AN UNINTENTIONAL ROOF FALL OCCURED ON B-MINES SECTION AT N6 BLOCK. HEIGHT- 6', WIDTH- 18', LENGTH- 70'. SUPPORT- 5' RESIN. ADDITIONAL SUPPORT- A FEW 8' RESIN.
AN UNINTENTIONAL ROOF FALL OCCURRED ON B-1 PANEL 1 BLOCK INBY STA 9909. HEIGHT - 6', WIDTH - 18', LENGTH - 18'. SUPPORT - 5' RESIN ADDITIONAL SUPPORT - 6'RESIN & 10' CABLE BOLTS.
AN UNINTENTIONAL ROOF FALL OCCURRED ON B-3 PANEL IN LEC 4-5 ENTRY AND 1 BLOCK #5 ENTRY. LENGTH 100', WIDTH 18', HEIGHT 6'. AREA WAS SUPPORTED WITH 5' RESIN. PANEL HAS BEEN ABANDONED.
INDIVIDUAL HAD PLACED PUSHER STEEL ON FINISHED AND WAS RAISING HEAD UP ONTO STERIS. PUSHER FOUNED IN CHUCK CAUSING STEELS TO FLEX BOW THEN THE PUSHER SPRANG INTO BOTTOM OF THE CHUCK. INDIVIDUA L HAND WAS HOLDING STEEL WHEN IT FLEXED BACK STRAIGHT STIRKING THUMB.
EMPLOYEE WAS RIDING IN PASSENGER AREA OF 2 MAN- TRIP WHEN HE STRUCK HIS HEAD AGAINST A URNT PIPEWHICH CROSSED OVER THE TRACK. THE PIPE HAD DROP-PED DOWN AND THE OPERATOR TRIED UNSUCCESSFULLY, TO STOP. NOTICE: BEGAN MISS WORK AGAIN ON 11/6.
INDIVIDUAL WAS IN KNEELINE POSITION ALONG BRLT PREPARING TO SQUARE IT FOR A SPLICE. AS HE PULLED HIS KNIFE THROUGH THE BELT, IT CAME OUT AND HE STABBED HIS LEG.
THE AREA HAS BEEN BAD FOR SOME TIME AND WAS CRIBBED OFF. LOOSE ROOF HAS FALLEN AROUND CRIBS, PULLING BOLTS IN PART OF THE AFFECTED AREA WHICH IS 20' LONG BY 17' WIDE BY 7' HIGH. BOLTS 6' TORQU E TENSION.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE INTERSECTION OF #3 ENTRY, 8 BLOCKS OUT BY THE FACE. THE AREA HAD SOME SLICK SIDE CONDITIONS IN IT AS IT WAS MINED. FALL MEASURED 20' LONG BY 17' WID E BY 10' HIGH, BOLTED WITH 5' RESIN 6 BY 16 PLATES.
INDIVIDUAL WAS UNSTALLING INSIDE BOLT WHEN A ROCK 30" X 24" X 1" FEEL STRIKING CANOPY THEN THE PERSON.
WHILE RAISING DRILL HEAD OF FLETCHER BOLTER TO PUSH STEEL INTO STARTED HOLE, THE STEEL MISSED THE HOLE AND SPRUNG. IT SPRUNG BACK STRAIGHT WHILE TAR INDIVIDUAL WAS HOLDING THE STEEL. THIS ACTI ON WAS VERY FAST, NOT ALLOWING THE OPERATOR TIMETO REACT.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE RETURN 4 BLOCKS OUT #10 SEAL. THIS IS IN THE WEEKLY EXAMINERS WALKWAY FROM 10 SEAL TO ELEVATOR SHAFT. HEIGHT 4-6' WIDTH 20' LENGTH 22'.
ROOF FALL OCCURRED ON B-1 SECTION (002-0), #6 ENTRY. BOLTS: 5' RESIN. HEIGHT: 6'-7'. WIDTH: 18'.LENGTH: 22'.
INDIVIDUAL WAS CUTTING CONVEYOR BELT TO SQUARE IT USING A UTILITY KNIFE. AS HE PULLED THE KNIFE THROUGH THE BELT IT CAME TO A THIN EDGE CAUSING HIM TO PULL THE KNIFE ON THROUGH INTO HIS KNEE.
INDIVIDUAL WAS REHANGING LINE CURTAIN, AFTER EQUIPMENT HAD TORN IT DOWN, WHEN A ROCK, APPROX. 12" X 24" FELL OUT STRIKING HIS RIGHT SHOULDER. THIS OCCURRED APPROX. 30' INBY LOC. NO VISIBLE CUT TERS IN MINE ROOF AT THIS TIME.
SHUTTLE CAR DROPPED INTO HOLE IN PAVEMENT CAUSING OPERATOR'S HEAD TO BE JAMMED UP AGAINST CANOPY.
INDIVDUAL WAS EXITING THE SCOOP WHEN HE CAUGHT HIS FOOT ON THE SIDE OF THE DECK, TRIPPING HIM. HE PUT HIS HANDS OUT TO CATCH HIMSELF. UPON HIS HANDS STRIKING THE POURMENT HE FRACTURED HIS LEFT WRIST.
