EE HAD STARTED HIS SHIFT AT 500 AM AND HAD WALKED INTO THE DIESEL SHOP TO SEE IF THE D-9R DOZER THAT HE OPERATES HAD BEEN REPAIRED. THE DOZER HAD BEEN DOWN DUE TO ENGINE PROBLEMS. HE STEPPED U P ON ONE OF THE TRACK ASSEMBLIES, TO LOOK AT THEENGINE, WHEN HE LOST HIS BALANCE AND FELL. WHEN HE FELL HIS RIGHT RIB CAGE STRUCK THE DOZER BLADES PUSH-ARM.

Mostrando los 26THE EE WAS TRAVELING IN AN OPEN MINE JEEP. A WATER LINE WAS HANGING ACROSS THE MINE TRAKC, MAKING CLEARANCE LOW. HE FAILED TO LOWER HIS HEAD ENOUGH AND STRUCK THE WATER LINE WITH HIS HEAD.
DIESEL MECHANIC WAS REQUESTED TO CHECK THE OPERATION OF THE GEAR SHIFTER ON THE NO.9902 DART HAULAGE TRUCK. AFTER MAKING HIS CHECK HE EXITED THE TRUCK OPERATOR CAB AND WAS PREPARING TO DESCEND DOWN THE TRUCKS LADDER. AT THAT TIME THE TRUCKS OPERATOR CLOSED THE DOOR, NOT KNOWING HIS RIGHT HAND WAS ON THE DOOR POST, THIS PINCHED RIGHT MIDDLE FINGER, PARTIALLY REMOVING FINGER NAIL.
EMPLOYEE WAS PULLING #195 SHILED IN BUT IT WAS IRON BOUND (STUCK) WHEN HE THOUGHT IT BROKE LOOSE THE SHIELD WAS SET ON PULL. BUT THE PAN PULLED BACK CATCHING HIS RIGHT FOOT BETWEEN THE PONTOON ON #196 SHIELD AND THE PAN. (ONE FOOT WAS ON THE PONTOON OTHER WAS ON BOTTOM OR FLOOR)
EMPLOYEE WAS UNHOOKING HIGHLINE TO DROP ON GROUND WHEN THE HIGHLINE FELL BEFORE HE WAS READY CAUSING A HIGHLINE HANGER TO PUNC IN HIS RIGHT HAND BETWEEN THE THUMB AND FOREFINGER.
EE WAS BUSTING UP SLATE IN THE PANLINE AT #3 SHIELD. WHEN HE MOVED A PIECE OF SLATE, ANOTHER PIECE FLIPPED DOWN, SMASHING HIS LITTLE FINGER ON THE RIGHT HAND. (HE MISSED ON DAY TO HAVE HIS FIN GER STITCHED).
EMPLOYEE WAS PREPARING TO BOARD A MANTRIP WHEN HE STEPPED ON A BELT ROLLER CAUSING HIM TO FALL. WHEN HE FELL (BACKWARDS) HIS HEAD STRUCK A CONVEYER BELT RAIL. THIS RESULTED INTO A CONCUSSION. HE BEGAN TO MISS WORK ON 11-7-01.
EE WAS ATTEMPTING TO PULL A MANTRIP SEAT FORWARD TO SEE IF THERE WAS A SECTION OF AIR HOSE BEHIND THE SEAT. THE SEAT WAS STUCK AND WHEN HE GAVE IT A HARD TUG THE SEAT QUICKLY SPRANG FORWARD AN D HIS RIGHT MIDDLE FINGER WAS PINCHED BETWEEN THE FRAME OF THE SEAT AND THE END OF THE MANTRIP. THIS RESULTED INTO A FRACTURE TO THE TIP OF HIS RIGHT MIDDLE FINGER.
EMPLOYEE WAS BUILDING A WALL FOR AN AIRLOCK DOOR(HE WAS USING OMEGA BLOCK OR FIBER BLOCK). AS HE PICKED UP A BLOCK FROM THE SUPPLY CAR HE FELT SOMETHING PULL IN HIS BACK. (EE WORKED UNTIL 11/ 13/01.)
EE WAS REPAIRING THE WINDSHIELD WIPERS ON TRUCK.HE DROPPED A WRENCH & WHILE DESCENDING THE TRUCKLADDER TO RETRIEVE IT HIS FOOT SLIPPED ON THE BOTTOM STEP CAUSING HIM TO FALL ONTO THE CONRETE F LOOR. HE LANDED ON HIS LEFT SHOULDER. THE INJURYHAS INITIALLY BEEN DIAGNOSED AS AN ACUTE SPRAIN.
EMPLOYEE WAS FORCING OPEN A COVER ON A MANTRIP HE HAD JUST REPAIRED. HE DID NOT REALIZE THE RUBBER LATCH STRAP WAS HOOKED. WHEN HE FORCED THE COVER UP THE HINGE END OF THE STRAP BROKE LOOSE ST RIKING HIM ON THE RIGHT TESTICLE WITH GREAT FORCE.
EE WAS REACHING IN THE DECK OF A BATTERY CAR TO GET A SAFETY CHAIN THAT WAS TO BE INSTALLED BETWEEN THE BATTERY CAR AND A 15 TON MOTOR. IN DOING SO HE CONTACTED A CABLE THAT WAS PART OF THE WI RING CIRCUIT FOR AN EMERGENCY STOP BUTTON ON THAT BATTERY CAR. IT APPEARS THERE MUST HAVE BEEN A BARE PLACE IN THAT CABLE, AND WHEN HE CONTACTED THAT CABLE HE PUSHED IT AGAINST THE METAL FRAM
EMPLOYEE WAS WELDING ON THE TAIL END RANGING ARM WHEN A PIECE OF SLATE FELL FROM THE ROOF STRIKING A CO-WORKER ON THE SHOULDER THEN HITTING HIM ON THE LEFT KNEE. (WORKED UNTIL 3/18/02.)
AT 0430 A FIRE LIGHT WENT OFF ON 00R 3NE SECTION BELT. THE HOIST OPERATOR CALLED TWO PEOPLE TO INVESTIGATE. THIS SECTION WAS ABANDONED, NO ONE WAS ON THE SECTION. AT 0500 A ROOF FALL WAS FOUND IN THE #1 ENTRY AT THE 2030' MARKER.
INJURED AND ANOTHER EE WERE REMOVING THE CLUTCH ROTOR FROM THE OVERHEAD HOIST LOCATED IN THE G4 STATION OF THE 10 MILE OVERLAND CONVEYER. WHEN HE PULLED THE ROTOR OUT OF THE CLUTCH HE LOST CO NTROL OF IT AND HIS LEFT LITTLE FINGER WAS PINCHED BETWEEN THE ROTOR AND CLUTCH HOUSING. THIS RESULTED INTO A LACERATION THAT REQUIRED 7 SUTURES TO CLOSE.
A ROOF FALL HAS OCCURRED IN THE SOUTH MAINS A BELT AND TRACK ENTRY IN THE AREA OF SURVEY STATION NO 9871. THE FALL IS APPROX 24 FT WIDE BY 100 FT LONG AND IN EXCESS OF 15 FT HIGH. THE MATERIAL S IN THE FALL CONSISTED OF MOSTLY LIMESTONE AND SHALE. A VARIETY OF ROOF SUPPORTS HAD BEEN USED IN THIS AREA INCLUDING SEVERAL DIFFERENT LENGTH OF RESIN GROUTED BOLTS AND ROOF GROUTING.
A ROOF FALL HAS OCCURRED IN THE TAILGATE ENTRY OF THE CS LONGWALL SECTION NEAR THE 1600 FEET MARKER. THE FALL WAS APPROX 30 FT LONG BY 20 FT WIDE AND UP TO 5 1/2 FT HIGH. THE FALL MATERIAL CON SISTED OF LIMESTONE. THE AREA ROOF HAD BEEN SUPPORTED BY 4 FT LONG RESIN BOLTS AND "SUPERPOSTS".
EE WAS PULLING THE SHIELD ON THE C-5 LONGWALL FACE WHEN HE REACHED FOR THE CONTROLS THAT OPERATES THE SHIELD FUNCTIONS, A SHIELD TO SHIELD HIGHTPRESSURE HOSE BURST, SPRAYING EMULSION ON EE. EE SAID THE HOSE DID NOT STRIKE HIM HOWEVER, THE PRESSURE OF THE EMULSION CAUSED HIM TO BE KNOCKED TO FLOOR & HIS LEFT KNEE STRUCK A RELAY BAR THIS RESULTED IN FRACTURE TO LEFT PATELLA
A ROOF FALL WAS FOUND BY A FIREBOSS ON 8-13-01. THE ROOF FALL WAS FOUND IN THE RETURN OF THE 00 RIGHT CONTINUOUS MINER SECTION. THE SECTION HAS BEEN OUT OF PRODUCTION AND ABANDONED.
EE WAS ASSISTING ANOTHER EE IN LIFTING A RAM JACK THAT WAS LYING ON THE MINE FLOOR BETWEEN THE SHIELDS &FACE. A CHAIN WAS PLACED IN THE EYE OF THE JACK & BOTH EE'S ATTEMPTED TO LIFT IT ONTO THE PONTOONS. IT WAS DURING THAT TIME THAT INJ EE SAID HE EXPERIENCED A BURNING SENSATION IN HIS BACK
EE WAS ASSISTING HANG MONRAIL STRUCTURE IN THE NO.3 ENTRY OF THE C-5 LONGWALL PANEL. HE WAS ATTEMPTING HOOK ONE END OF A PIECE OF STRUCTURE INTOTHE PIECE THAT HAD JUST BEEN HUNG. THE END OF TH E STRUCTURE SLIPPED OUT OF EE'S HAND & IN HIS ATTEMPT TO CATCH IT, HE STRAINED HIS LEFT SHOULDER. HE BEGAN TO MISS WORK ON 8/15/01 DUE TO PROBLEM
EMPLOYEE WAS WALKING DOWN THE STEPS IN THE SLOPE STEPPED ON A PIECE OF LOOSE COAL AND FELT A POP IN HIS RIGHT KNEE.
EE WAS USING A HUCK GUN TO SECURE NEW LINER PLATES IN THE BOTTOM OF THE NO.1 WASHBOX. WHILE PERFORMING THIS TASK THE EE SLIPPED & FELL WHICH RESULTED IN A LACERATION TO THE BACK OF HIS HEAD WHICH REQUIRED SUTURES TO CLOSE. EE ALSO COMPLAINED OF SOME BACK PAINS.
EE WAS REMOVING A BRAKE UNIT FROM AN EIMCO SCOOP. HE HAD UNBOLTED THE UNIT AND HAD PRYED IT LOOSE TO RELEASE THE TENSION. THIS ALLOWED THE BRAKE TO TURN A COUPLE OF REVOLUTIONS. SEEING THAT TH E UNIT WAS LOOSE, HE ATTEMPTED TO REMOVE IT WHEN IT TURNED ANOTHER 1/4 REVOLUTION. WHEN THAT OCCURRED, HIS LEFT THUMB WAS PINCHED BETWEEN THE BRAKE UNTI AND A MOTOR PULLEY. THIS RESULTED IN A
EE HAD JUST ARRIVE AT THE LONGWALL SHEARER AND WAS PREPARING TO CHANGE SOME OF THE BITS ON THE TAIL AND DRUM. HE HAD HIT A COUPLE OF THE BITS, TO LOOSEN THEM, WHEN MATERIALS FELL FROM THE MINE ROOF, STRIKING HIM ON THE HEAD, NECK, AND RIGHT SHOULDER. THE MATERIALS WERE A COMBINATION OF ROCK AND COAL AND WAS APPROX. 30" X 36" X 1 1/2" THICK.
EE & ANOTHER EE WERE REPAIRING THE EMULSION LINENEAR THE LONGWALL POWER CTR. INJ EE WAS USING A CRESCENT WRENCH ATTEMPTING TO HOLD A COUPLER IN PLACE AS OTHER EE USED A PIPE WRENCH TO TIGHTEN THE BOLTS ON THE SAME COUPLER. WHILE PERFORMING THIS TASK INJ EE EXPERIENCED SOME PAIN IN UPPER RIGHT ARM HE IS NOW MISSING WORK DUE TO PROBLEM HOWEVER COMPLETED DIAGNOSIS HAS NOT BEEN MADE
