No medical would see them. We took next available with previous surgeon. Miner was immediately released to full duty.***

Moving too fast as I was climbing ladder and slipped on step.
Slipped off step of truck and fell on his elbow. Small fracture occurred to bone near elbow. Restricted activities using one hand.

Great River Material EE was standing next to his endloader at the time of blasting of a rock ledge. A piece of fly rock from the blast struck his right leg at knee level, breaking his leg.

The primary crusher was shut down due to a boulder. Our plant man/mech had positioned himself to open chamber door, he did so before impeller had come to a complete stop. Then thinking he could stop the impeller, he put his hand into the pinch point between impeller and door opening.

EE WAS REPLACING BARS ON THE CRUSHING PLANT. THE BAR SLIPPED AND FELL ON HIS HAND.
EE WAS GETTING OUT OF THE STOCKPILE TRUCK AND TURNED HIS ANKLE WHEN HE LANDED ON THE GROUND.
EE AND OTHER WERE HOLDING TO SCREENS UPICKT ON GROUND INTO LOADER BUCKET. LOADER PULL UP EE GAVE STOP SIGNAL, LOADER HIT SCREEN'S KNOCKING THEN OUT OF ARE HOLD. EE SPUNG TO GET AWAY BUT SCREEN HOOK GRAB TO OF HIS HAND, TEARING THE SKIN OPEN.

DRIVE SHAFT WAS STUCK ON HAMMER MILL. SUPERVISOR AND EMPLOYEE BOTH TRYING TO PRY LOOSE WORKING FROM OPPOSITE SIDES SUPERVISOR DID PRY SHAFT LOOSE NOT REALIZING EMPLOYEES ARM WAS IN THE WAY. HAMMERS WEDGED EMPLOYEES ARM AGAINST THE DOOR.
