Mason City Plant: 2,456 MSHA citations Metal/Non-Metal

Employee was using an air lance with 90 psi air to clear a chute plug in the #4 Finish Mill feed chute. The employee lost grip causing the air lance to hit them in the mouth. They initially declined medical attention. On 10/24/2025, they requested to be seen at the emergency room due to mouth pain.

Shift repair was working on the "small mills" FK pump to repair an air leak. While walking from the repair truck to the west side of the FK pump, they tripped on uneven floor plates and fell hitting their right knee. Employee was taken to the emergency room and was diagnosed with a fractured right patella (kneecap).

Employee was removing liners from ball mill when they dropped the liner striking them in the foot. Receiving a fracture in the foot.

Employee was cutting cardboard on the floor with a utility knife in the maintenance garage. Their left hand was holding the cardboard, and right hand was cutting. The cardboard slid on the floor and the knife slipped lacerating their left wrist.

While stripping a ProfiBus wire with a safety utility knife, the blade slipped off the cable and sliced their left-hand thumb. Employee was taken to local emergency care and received 5 stitches in left thumb

Employee was operating a jackhammer to remove material and as piece of material fell- hitting the jackhammer and pinning left index finger against port opening on the container causing injury.

Employee was assisting the shift repair at the finish mill 3 vibrating pan. The belt to the motor of the vibrating pan had seized and once the equipment was locked out, employee was cleaning to get material clear to start up vibrating pan. Equipment still locked out, employee pulled belt to get unstuck-pinky finger injured between the sheave and the belt.

Employee started feeling unwell, and it was called over the radio that the employee needed help. Supervisor got ice packs and water and headed up the stairs in the tower. It was hot in the tower. The employee was observed walking unsteady and was showing symptoms of heat exhaustion.

Employee was working in the chemical lab and was washing a 800 ml beaker and it slipped out of their hands and hit the edge of the sink. The glass from the beaker cut their left forearm causing a laceration. Employee required sutures to close the wound.

Injured employee was shoveling clinker into a floor hatch that was opened for clean-up. Employee lost footing and fell partially into the opening, catching themselves and landing on, and dislocating right elbow. Employee was taken to ER, shoulder was reset, and they returned to work with restricted duty (one handed) until follow-up.

While installing a new junction box for the #2 cooler fan, employee felt pain in right shoulder. Employee felt pain on 1/29, while working with supervisor, but did not report injury until 1/31. EE did not seek medical attention until 2/4/22.

Employee stood upright after rolling a 12 inch belt and experienced a back spasm. Employee sought treatment at a chiropractor.

On 9/28 @ approx. 3am, ee felt pain in back when dragging hose. EE did not report it until 9/30 @ 11pm. EE tried self-care until seeing Dr. on 10/3. EE taken off work on 10/4 and placed on restricted duty. Incident was not reportable until EE missed work on 10/4.

Working from a ladder, EE was bringing a light down to repair it and felt pain in lower back. EE proceeded to the Elec. Shop to repair the light. After repairing the light, EE went back up to the top of the Homo Silos to reinstall the light. EE said the pain became worse after installing light fixture and putting ladder away on top of the Homo Silo.

While working on a ladder, approx. 52" high, EE experienced personal medical event causing them to fall to the ground. Co-worker notified emergency services and performed CPR until EMTs arrived. EE was pronounced DOA. Cause of death unknown at time of report.

EE was transporting a 10 ft. probe in the silo elevator. The probe became stuck while sticking out of the roof. EE attempted to retrieve it, but it fell, striking EE's rt thumb. EE received x-ray which determined that the thumb was broken.

Employee lost footing while exiting locomotive as EE stepped onto concrete working surface. Employee landing on right knee and right forearm. Employee was seen by local doctor and was determined to have a muscle strain in right quadriceps. Employee received limited duty work restrictions for one day.

Employee used bare left hand to brush away the material from the area of an unknown leak. A bolt had backed out from the screw and created a hole in the housing. Employee was unaware of the bolt. Bolt tore nail off of left index finger.

Employee was removing wrapping tape from the seat of new conference chairs. Employee gathered paper to cut and remove, and cut the end of left middle finger with scissors. Skin was lacerated on three sides of the tip, remaining intact on the side closest to thumb. Employee received 6 stitches to aid in healing of the lacerated skin.

Employees used shotgun to breakup material buildup in 3rd stage preheater tower. EE held the stock in right hand which became sore and blistered the next day. EE requested medical treatment & was given prescription 9/28, making incident reportable.

While removing the #3 stator from the motor with a come along, employee was bent down ratcheting the come along and stood up feeling a pain in employee's lower back right side with shooting pain down the back of employee's right leg.

Employee was changing spray nozzles in the tower. On the third to last one, EE's back began to spasm. EE then proceeded to a call to the D-13 area. EE's back began to spasm again as EE finished taking the coupling apart and then EE reported it to the supervisor. Employee did not wish to seek medical attention. Employee was evaluated and given prescriptions 3/13/17.

Employee was breaking up material clogging a chute with a bar. Employee felt a pop and pain in employee's left shoulder. Employee denied medical care at the time, but requested to be seen on 1-23-2017.

Employee was walking to the truck after the shift. EE met another employee coming in. Other employee said "watch it, it is slick". Employee then proceeded to parking and was in front of the truck taking small steps when EE was looking where EE was stepping and heard a pop and then a crack when EE fell to the ground. Employee fell backwards. EE had pain in right shoulder.

Employee was clearing the D-16 to D-11 chute from a flat rock that became lodged in the chute. Employee pulled the e-stop lanyard on D-11 and got on the 3 step platform from the south side of D-11. Employee used a bar to pry the rock from the chute. The rock came loose allowing the bar to drop suddenly and employee's pinky finger got between the bar and the metal skirting.

Employee was water blasting equipment when hot water splashed back onto leg causing a burn. Employee developed blisters on skin and requested medical treatment where EE received a prescription medication.

Employee was walking in the west end of the tapping tunnel checking the area for flooding. Employee saw a piece of trash when employee bent over to pick it up, right foot went into an unseen hole and twisted right knee. Employee did not need to see the doctor at this time. An MRI on 10/5 revealed meniscus damage. Surgery took place to repair the damage on 12/8/2016.

While trying to get material to withdraw, employee put an air lace into a ¾ inch port hole on the side of the hopper. The air lance was 6 foot long. The material finally started to flow. Some of the material came out around the lance some dust got into right eye. Employee seen doctor and received prescription. No work restrictions.

Employee slipped while traveling down a set of stairs causing left foot loose grip on the stair. Employee maintained balance and did not fall. Employee refused medical treatment at the time of incident.

While using a 60' manlift and lowering to ground, the basket got caught on a bolt head. When basket came free of the bolt, the basket jerked and one of employees was ejected and landed on the ground. The employee fell approx. 33 feet. Employee was wearing harness with double lanyard but appears to not have been tied off at time of incident. Employee still being evaluated.

Haul truck was struck by FEL when operator inadvertently used wrong lever and lowered bucket against top of truck bed. Truck operator noted no initial report of injury after incident, but reported pain/soreness in neck/shoulder areas the next day. Initially cleared for duty, but then received restrictions on 3/20/16 when EE went to doctor on own accord.

Spray Tower tipping valve opened and dumped into bucket. Dust was in the air and floating as employee entered the area.

While driving waste dust truck the passenger front tire came off. Injured worker strained his right shoulder.

Injured worker pinched his middle finger on his left hand while adjusting a damper.

Injured worker was cleaning the tracks of the dozer with a shovel. The shovel slipped and the shovel handle hit the person in the mouth causing two teeth to become chipped, and one tooth to become loose.

While cleaning out a dust collector hopper injured worker became overheated.

Injured worker was waterblasting in the pre-heat tower and strained his right shoulder.

Injured worker was driving a forklift truck in the plant garage when he drove over the boards that were placed over the mechanic's pit. The forklift broke through the boards and pinned the injured workers right leg between the forklift truck and the wall of the mechanic's pit.

Employee was chiseling cement material when a piece of cement fell and hit his right elbow

Injured Work was removing snow and slipped on the ice spraining his left ankle. Was sent to the doctor on 12/04/2014 and was placed on restrictions.

Employee was changing a light bulb in the craneway. Material entered both eyes because of the wind. Was seen by the eye doctor. Both eyes were cleaned out. Prescribed antibiotic.

Employee was driving a Cat 777 Haul Truck and felt a twinge in his lower back. The twinge continued to get worse. Employee was taken to the clinic. Doctor said that employee has a lower back strain/spasm. Placed on restrictions.

Injured worker was doing general clean up sweeping, shoveling, and using a wheelbarrow. Was at home a couple hours after work. When getting out of a chair felt stiffness and shooting pain in his lower back. Reported the incident the next morning. Sent to a chiropractor. Was put on work restriction 6/25/14.

Injured worker was going down stairs and felt a pop in his right knee.

Injured worker was cleaning out a rotary valve and hit his left elbow on a support beam. Went to the doctor on 5/5/14 and was place on restrictions

he was raising himself up in a man lift, someone above him on the platform moved and dust fell into his left eye

Injured worker was walking from his car to the main change house to report to work. He slipped on ice and sprained his right shoulder. Was taken to E.R. to be checked out and returned to work. On 3/13/14 requested to see the doctor because his shoulder was continuing to bother him. The doctor took him off of work until an MRI was completed.

Mechanic was lifting a cylinder rod into the barrel of a wheel loader. The load shifted causing the rod to come out of the barrel and swing around landing on his left foot. Then the load block truck crane swung around and hit him in the head knocking his hardhat off causing a laceration.

Employee was repairing a guard when a piece of material entered his left eye. Was seen by an eye doctor. Material piece was removed. Prescribed an antibiotic.

Injured worker stepped out of pick up truck and strained / twisted right knee. Reported accident on 3/7/14. Doctors appointment on 3/10/14 and was placed on restrictions.

Injured worker was going down stairs and slip when he went from the last step to the ground. Right quad leg pain. Placed on restriction 12/04/2013.

Injured employee pinched her middle finger on the right hand underneath a pallet causing a laceration and fracture at the end of the finger.

9:05 p.m. employee reported smoke coming from the rotochopper. No personal injuries. It was discovered that the discharge conveyor broke causing the rotochopper to plug up. Smoldering material was found at the discharge of the rotochopper.

Back of right knee sore. Cause is unknown. Doctor appointment 05/01/2013 placed on restricted duty

While injured person was placing a sump pump in the shipping silo basement he stepped on a material chunk causing strain to his right knee

Pre-existing injury. Employee had injury while in the service. Had knee replacement. He was seeing the VA Hospital at Des Moines for problems he was having with the knee replacement. He had an appointment on 2/15/13 at the VA Hospital to schedule his surgery.

Injured worker was welding guards in the maintenance shop when he felt pain in his lower back. Doctor placed injured worker on restrictions on 12/12/14.

Injured worker got dust in his eyes on 1/2/13. He reported the accident on 1/7/13 and he felt there was something in his eye. Had Dr. appointment on 1/8/13 and there was nothing in his eye but he had infection. Was prescribed medication.

Injured worker was walking through the shop door and his shirt caught on the door handle and cause him to lose his balance. On 1/9/2013 he reported that his left side continued to be sore and want to see the doctor. On 1/10/2013 he was placed on restrictions.

While walking down a spiral stair case injured worker felt a pop in his right knee.

While water blasting at the exit of the kiln injured worker sprained his right shoulder.

Results from hearing test shows a standard threshold shift. There has been a second test done. Has been reviewed by an audiologist.

Injured worker was replacing the screen for the centrifuge when his right hand slipped and hit against piece of metal

Removing cement chunks from withdrawal silo 55. In the kneeling position. Stood up, started to walk down the stairs, felt pop in his left knee. He felt that his knee would be ok in a couple of days. He stated he wanted to see the doctor on 6/18/12. No restrictions at or medications at that time. Dr. visit of 7/10/12 prescribed medication and therapy with no restrictions.

Injured worker was operating a valve when his back became sore. He was sent to the doctor 5/29/12. The doctor placed him on physical therapy after visit on 6/19/12.

Worker lifted lip shrowd off ground, twisted to throw in box and felt a stabbing pain in the ribs (left side)

While resetting new structure on base bolts unit slid landing on left ring finger smashing it between base plate and 2X4.

While leaning over to remove air compressor motor base bolts injured worker felt pain in his lower back

After pushing in a tire plug to repair a leak injured worker stood up and felt a pain in his lower back. Doctor has now placed him on physical therapy.

While cleaning material around #1 Stone Conveyor Belt injured worker hit his right hand on a guard and which caused a laceration that required 5 stitches.

Injured worker felt numbness and tingling in both his feet after he had shut railcars.

Injured worker was using a hammer to separate two section of airslide. He swung the hammer and missed the airslide causing a strain in his right wrist. Reported on 7/11/11 his wrist became sore and wanted to see the doctor. He was placed on 50 pound restrictions on 7/12/11.

Injured worker reported he was getting out of the lab cart and slipped on some mud. He reported to the supervisor that he was not hurt or injured and was just turning in the hazard. On 6/9/2 011 injured worker stated that he would like to see the doctor for his left knee. Doctor didn't put him on restrictions until an MRI was done. Date of restriction was 6/20/2011

Injured worker complained of a sore wrist on 5/10/2011 from his normal job activities. He later requested to see a doctor. An appointment was made for 6/7/2011. Doctor placed him on restricted work duties for wrist tendonitis

Left ear at 2K and 3K showed a Standard Threshold Shift in annual hearing test. Employee was sent to Audiologist. Was confirmed that there was a STS.

Injured worker was walking down a wooden ramp and slipped causing a strain in her right knee. Accident wasn't reported until Saturday 3/26/2011.

Injured worker picked up a tote full of drill samples and strained his upper right side of his back.

Injured worker was cutting loose fabric from his sweat shirt with a personal jack knife and punctured his left thigh. He received 3 stitches.

Injured worker was working on 107 pan conveyor and complained of soreness due to bending and twisting on steps while removing guards. Reported the next day that he was suspicious of a possibl e hernia. Appointment was made to see the doctor. Was referred to another doctor.

Strained neck while shoveling material

Injured worker was water blasting and felt a sharp pain in his lower back

Injured worker was performing a silo change when the silo change bar slipped out of the knuckle of the valve and caused him to fall backwards hitting the splash box. Injured worker did not have any pain. As the week went on he started having pain and stiffness in his neck. He reported the pain on 10/31/2010.

Injured person was waterblasting and his lower back popped out and the popped back in. The same thing happened the next day while water blasting and opening freight elevator door.

Injured worker was making a silo change and the bar he was using slipped out of the knuckle on the valve causing his right little finger to be pinched between the silo change bar and the transport line.

Waterblasting exit of the kiln a chunk come down on the barrel of the waterblasting gun causing the gun to jolt the injured workers right shoulder. Doctor's appointment 9/28/10 was put on restricted duty 9/29/10.

While removing a tie rod end, employee was hamnmering on end and a piece broke off and punctured his left arm. The piece of metal lodged into his arm.

Strained lower back while waterblasting the top hole north side of riser while standing on a 3 step platform.

Injured worker was changing a filter bag out of the Alternate Fuels M-15 Dust Collector. The filter bag location was waste high and injured worker was bent over pulling the filter bag out of the dust collector when he felt a pop in his back.

Injured worker was kneeling on angle iron. When he got up and turned he felt pain in his right knee.

Injured worker was removing a gearmotor from the main spray tower tipping valves when the unit fell onto the injured workers left foot.

Injured worker was cleaning around clinker cooler with jackhammer and shovel. His back and upper left leg felt sore as the day went on. Reported to his supervisor around 10:15 a.m. Appointment was made to see the doctor.

Injured Worker was repairing #6 Finish Mill Separator stems inside the separator which requires the person to be in an awkward position. He was kneeling on metal which he used felt for cushion. His knee became stiff. He reported on 6-15-10 that his knee was still feeling stiff and wanted to see a doctor.

Injured worker was removing an inspection door at the bottom of the cement silos. When the door came loose the handle hit his left wrist.

Injured worker was jackhammering and removing chunks of cement from silo 54 when he felt his lower back tighten up. On 3/25/10 he requested to see a doctor and was placed on light duty.

Injured worker was taking a cooler for the cems monitoring unit up a flight of stairs using a two wheeled cart when he strained his left knee. Injured worker didn't request medical attention until 06/03/10. His treatment will be a knee support, and physical therapy. Restrictions are no lifting over 50 pounds and avoid kneeling, squatting

While tapping stone from stone storage injured worker was using a bar and felt a pinch in his lower back. He didn't feel that the pinch was a problem but just wanted to report it. On 3/23/10 he requested to see a doctor. The doctor placed him on light duty.

While repairing motor, pipe wrench slipped and injured worker. Was struck in face with pipe wrench. Was taken to ER and received 3 stitches in upper lip.

Injured worker was cleaning in the area of 302 drag conveyor. He slipped on wet material and tried to catch himself, when his left hand went into hot material, causing 2nd degree burns. Injured worker was wearing gloves at the time of the accident.

Injured worker was bricking inside the kiln when he got a dust particle in his right eye. He was taken to the ER for treatment. Injured worker was diagnosed with a corneal abrasion, and was given a prescription.

While moving plywood onto forks of the fork lift, injured worker slipped and fell on his tail bone. On 8/16/09 he went to the ER because of sorness. ER Doctor said that he had a bruised tail bone. On 8/17/09 he went to the clinic and the doctor there said that he had a fractured coccyx. Doctor placed him on work restrictions.

Strained lower back while closing a sampler valve for finish mills. Injured worker reported that his back was feeling better, until 8/5/09, when he requested to see the doctor.

While water blasting on the kiln exit area, injured worker stated that he knelt down to shoot the lower northwest port. Injured worker said when he was shooting the port that he must have twisted wrong while using the gun and felt a pop in the middle of his lower back. Injured worker also said that his back felt fine to finish, but his back felt sore after he took a break.

While inspecting a Dust Collector pipe injured work got clinker dust in his both eyes. Flushed eyes for 15 minutes, and then sent to the emergancy room along with the MSDS sheet. Was prescribed medication.

Injured worker stepped on a piece of material and rolled his left ankle. He was seen by the doctor on 5/18/09. He was not put on any formal restrictions. He went back for his check up on 5/28/09. His conditions were unchanged so he will be starting physical therapy 3 times per week for 2 weeks.

Injured worker was ratcheting a 1.5 ton come-a-long when he felt a pull in his left tricep muscle. Later in the day his muscle became sore and started to swell. He went to the doctor and was placed on restrictions. Avoid use of left arm until 5/13/09.

Left ear at 2K had a 25db loss in annual hearing test. Employee's hearing was re-checked and showed hearing loss. Was sent to Audiologist. Had MRI done on left ear. Audiologist results from the MRI was that the hearing loss was caused by sharp loud noise. Employee stated that he didn't know when it happened and he didn't remember any loud noise that caused damage to his ear.

While employee was tightening bolts on lower A-arm on #7 Toyota pickup, felt a pop in his right shoulder. On 4/7/09 employee was working on a dingo, experienced pain in his right shoulder. Made an appointment to see Dr. and was diagnosed with A/C Separation. Placed on restricted duty.

Injured worker was shoveling up heavy material and afterwards felt pain in his forearms and elbows. He wanted to see the doctor one month later. On 5-14-09 the doctor placed him in therapy.

Injured worker tried to poke a hole in an envelope with a scissors and cut her left index finger. Needed three stitches.

Injured worker was driving a 777 Cat Haul Truck while stripping in the quarry. He hit a rut while driving the truck causing him to bounce around in the seat. The next day he felt pain in his chest and stomach area. He reported the accident to his Supervisor on 4/6/09. He was sent to the Doctor the same day he reported the accident. He has been placed on restricted duty.

Injured worker stepped down off the bumper of the welding truck and rolled his ankle on a 3/4" steel mill grinding ball.

Injured worker cleaning out a screw conveyor, when he turned around he felt pain in his lower right back. On 3/18/09 he reported that his back was beginning to hurt. Made appointment to see the doctor on 3/19/09. Will be going through therapy the next couple of weeks with non formal restrictions.

While picking up a 40 pound bag of salt he felt pain in his lower back. Lower back continued to bother him so the doctor has placed him in Physical Therapy.

While working on a forlift, the injured employee came in contact with the fan blade of the motor and caused lacerations on index finger, middle finger and thumb. Injured employee was taken to the emergency room and received 10 Stitches.

Injured worker was lifting a 50 pound bag of rock salt and had it between right side of body and right arm. Was hiking bag up closer under his right arm, and felt burning sensation on his right elbow. Went to see Doctor and was put on light duty.

Injured employee tripped going up concrete stairs and went down on his right leg causing a laceration on his shin. He received 5 stitches at the emergency room.

Injured worker was switching a valve when he caught his left index finger between the valve handle and the frame work. He had 3 stitches in his left index finger and was able to return to work with no restrictions.

Employee was cutting a bag of discarded seed corn and cut his left thumb. He was taken to the hospital and received 3 stitches. Employee has no restrictions

Injured worker was replacing a trunnion roll on the kiln. He was pushing and pulling with a bar on the base of the trunnion roll in the attempt to line up the base. He strained his left shoulder.

Injured worker was cleaning out a screw conveyor transfer and got dust in his right eye.

Injured worker was repairing Dust Collector. Had facial irritation from sweat and cement dust.

Slipped while coming down a portable set of stairs. Causing a muscle strain on the right leg.

Monday morning 7/14/08 injured worker felt something in his right eye. He said that it bothered him and would go away. On Tuesday morning he woke up and felt something catching on his right eye lid. On Tuesday morning reported the problem to his supervisor. He was than taken to Health Works Occupational Health.

Employee was cutting a bag of seed with a utility knife and cut his right thumb.

Employee bent over to pick up a piece of board that he did not realize was stuck to the ground. When pulling up on the board strained his back.

Employee entered the center discharge of the Raw Mill to remove screen. After removing screen employee came out of the mill and said that his heart was beating very fast. He went back to the welding shop to rest. His heart continued to beat fast. Called ambulance. Went to the E.R. and they treated him for heat exhaustion.

While closing rail car hooper door injured lower left part of his back.

Waterblasting burning corn that was built up in south side of by-pass drop out. Hot steam or gases created from this action went under the person's hood and mask which caused the person to feel like gagging. Person left area for fresh air for two hours. Accident report turned in 5/21/08. Went to Dr. on 5-23-08. No worked missed. Treated for laryngitis.

IW was driving FEL and was jostled in seat causing strain to lower back

IW felt pain in shoulder and neck when tightening a vise on a grease pump.

Person was cleaning the north side of the clinker cooler, shoveling into a wheelbarrow. When wheelbarrow filled person picked up the handles of the wheelbarrow and the right handle broke causing the person to twist his back.

IW was pushing on lever and the lever broke, causing IW to fall and strike left shoulder against wall.

IW GOT OUT OF HIS TRUCK EAST OF THE ELECTRICAL SHOP AND SLIPPED AND FELL ON HIS RIGHT HAND AND THEN LEFT SHOULDER.

IW reported his right shoulder started to hurt while waterblasting.

IW was working under low overhead and backed into a bolt, cutting right shoulder blade requiring stitches.

IW was removing door from finish mill separator and felt something pull in lower abdomen.

Employee was working in a new clinker cooler and stated he felt some type of pain on his left leg at knee. He came to first aid and reported it. He stated nothing struck or hit his knee. Took employee to doctor. Doctor stated that it was unknown what caused it. Stated it could be a spider bite. Treated with antibiotic.

Wireman was installing a control transformer in motor starter compartment using a hand threading tool to tap threads into two existing holes in the back wall of the compartment. The tap's pointed end penetrated the wiring which ran along the back side of the compartment metal wall thereby causing the short circut from the energized wire to the ground via the thread tap.

IW WAS SHOVELING SNOW AND HIT HIS HAND ON A DOOR HANDLE, CUTTING MIDDLE KNUCKLE ON RIGHT HAND. REQUIRED 5 STITCHES.

He was tightening bolts on the outside of a ball mill with an impact gun while standing on a scissor type manlift. Another person raised the lift and smashed his finger between the lift and impact gun. It cut the second from little finger of his left hand requiring 9 stitches.

IW strained lower left back while shoveling raw materials.

IW tripped when walking up stairway and fractured bone in right hand.

IW got a piece of foreign material in left eye.

IW was lifting tow cable and felt lower back tighten up.

IW was removing bolts from a pump and strained his lower back.

IW apparently suffered heat stroke while cooling down after working working in hot environment.

IW had removed a handrail for maintenance work, then fell through red "danger" tape from work platform.

IW was swinging sledge hammer, missed desired impact, and struck his own right index finger.

IW was opening a valve when he got dust in both eyes.

IW WAS REPAIRING A DUST ARRESTOR WHEN AIRBORNE DUST BLEW INTO HIS EYES

IW strained back by using overloaded wheelbarrow.

INJURED EMPLOYEE WAS PULLING A MILL LINER OUT OF THE RAW MILL WHEN HE TWISTED HIS KNEE, CAUSING A STRAIN.

EE was cutting corrugated hose across his leg when his foot slipped, causing him to cut his leg with a utility knife. 3 stitches were required.

IW was cutting a piece of rubber with a utility knife and knife slipped and cut wrist. IW received 2 stitches.

IW was changing a light bulb and lost his balance, reached out to catch himself and pulled muscle in right forearm.

HE WAS LIFTING A HEAVY METAL PLATE THAT WAS BOLTED TO A HOPPER. HE GOT HIS FINGER BETWEEN THE PLATE AND ANOTHER PART OF THE HOPPER AS HE WAS MOVING IT. IT CUT THE LITTLE FINGER ON HIS LEFT HAND. REQUIRED 7 STITCHES.

EE WAS ADJUSTING DEAD GRATES HOLDING A BOLT, WENT TO HIT BOLT WITH HAMMER, HIT LEFT INDEX FINGER WITH THE HAMMER. EE RECIEVED 8 STITCHES

EE WAS STEPPING OFF A LADDER AFTER PULLING CABLE, THE GROUND WAS COVERED WITH MUD AND ROOTS, EE TWISTED ANKLE WHEN EE STEPPED ON A ROOT AND SLIPPED OFF DUE TO THE MUD

EE REMOVED 10? PIPE CAP WHILE STILL UNDER PRESSURE, CAUSING CAP TO STRIKE HIM IN THE RIGHT FOOT, BREAKING A BONE IN THE FOOT

EE caught heal of boot on step and started to fall down stairs. EE caught herself with right hand causing small fracture in right wrist.

EE carried motor cart down two flights of stairs. Later that night, back started to tighten up.

EE1 & EE2 were using high pressure water gun to clean buildup out of calciner. EE2 removed gun from port and trigger stuck. EE2 lost control of the gun and the water spray crossed EE1's back opening up the skin and requiring stiches.

EE was scraping vehicle window with razor blade and lost control of blade cutting left hand below base of thumb. Five stitches were required.

EE was walking down flight of stairs and turned right ankle on uneven landing.

EE was cleaning out screw conveyor when he got dust in his left eye

EMPLOYEE WAS GRINDING COOLER GRATES WHEN 9" GRINDER SLIPPED AND HIT HIM ABOVE THE LEFT KNEE. THE GUARD WAS IN PLACE ON THE GRINDER. RECEIVED SEVEN STITCHES ON THE KNEE.

While working in dusty area, employee got dust in right eye

Employee committed an unsafe act by trying to light a cigarette from a piece of hot metal when he moved his hand in the path of an acetylene torch flame being used by another person cutting nuts off the ball mill while in the manlift.

EE WAS WORKING ON SLOPED ROOF DECK IN HARNESS GEAR. HE SLIPPED ON METAL DECK SURFACE & HARNESSLINE PULLED HIM & STRAINED HIS BACK. EE HAD PAININ BACK & UPPER TORSO AREA OF THE BACK, BUT PAIN DID NOT CAUSE HIM UNDUE DISCOMFORT SUCH THAT HE COULDN'T SIT OR STAND, BUT RATHER PAIN WAS A MILD DISCOMFORT. EE WAS CUTTING ABANDONED STRAP WITH A POCKET KNIFE. EE WAS CUTTING INCORRECTLY AND SHOULD HAVE USED SIDE CUTTERS TO CUT THE BAND. NOTE: EE WAS TERMINATED PER COMPANY POLICY AFTER REFUSING TO TAKE A POST ACCIDENT DRUG SCREEN

EE WAS USING A PRY BAR AGAINST A PIECE OF METAL TO MOVE IT INTO POSITION. THE METAL MOVED SUDDENLY & THE BAR PULLED ON HIS ARM CAUSING RIGHT SHOULDER STRAIN. WENT TO DR HAD XRAY PUT ON LIGHT DUTY-RET'D TO WORK SAME DAY NO LOST TIME

EMPLOYEE WAS MOVING A DRILL COLLAR IN THE BACK OF THE SERVICE TRUCK WHEN HE TWISTED CAUSING A STRAIN OF THE LUMBAR AREA OF HIS BACK.

WHEN EMPLOYEE STEPPED DOWN OFF THE RECEIVER ON SILO 55 HIS FOOT CAME DOWN ON UNEVEN MATERIAL CAUSING HIS RIGHT ANKLE TO TWIST AND SPRAIN. EMPLOYEE WAS PLACED IN RESTRICTED WORK PROGRAM.

EMPLOYEE WAS USING A SKIDLOADER TO MOVE PILE OF WASTE DUST. FOREIGN MATERIAL IN EYES. THE PILE WAS ABOUT 5 FT HIGH. A SECTION AT THE TOP OF THEPILE FELL HITTING THE LOADER BUCKET AND WASHING BACK ONTO THE OPERATOR CAUSING DUST TO GET INTO HIS EYES.

EE WAS USING A 3'BAR TO LOOSEN A SILO VALVE. THIS VALVE HAD BEEN INCORRECTLY TIGHTENED AND EE STRAINED HIS LT. SHOULDER WHILE PULLING ON BAR TO LOOSEN THE NUTS. EE REPORTED INJURY ON 10/4/0 2 BUT DID NOT REQUEST MEDICAL ATTENTION UNTIL 11/14/02 AND AT THAT TIME WAS PUT IN WORK HARDENING PROGRAM WITH RESTRICTIONS.

STRAINED SHOULDER REPAIRING DRAG CONVEYOR.

EE WAS EXITING THE CAB OF FRONT END LOADER WITHOUT HIS HARD HAT ON AND STRUCK HIS HEAD ON THE TOP DOOR FRAME CAUSING A LACERATION WHICH REQUIRED 5 STAPLES TO CLOSE.

EE WAS REMOVING THE LOWER DOOR TO THE #6 FEED ELEVATOR. AS HE REMOVED THE BOLTS THAT SECURE THE DOOR THE MATERIAL IN THE ELEVATOR CAUSED THE DOOR TO PUSH OPEN. THE STEEL DOOR STRUCK THE EE CAU SING INJURY TO HIS RT. LEG AND RT. HAND. HE WAS TREATED AT THE HOSPITAL WERE IT WAS DETERMINED THAT HE HAD MINOR ABRASIONS AND BRUISING.

EE WAS SHOVELING MATERIAL FROM THE GROUND LEVEL TO A CONVEYOR BELT D-10 WHICH IS ABOUT 4' FROM THE GROUND. WHILE SHOVELING, THE EE TWISTED CAUSING A STRAIN TO HIS LOWER BACK.

WHILE REMOVING WOODEN FORMS INSIDE THE CLINKER COOLER THE EE LOST HIS FOOTING CAUSING HIM TO TWIST HIS KNEE.

USING A LONG BAR EE WAS POKING MATERIAL OUT OF A SPOUT ON THE BALL SORTER WHEN HE FELT A SHARP PAIN INHIS RIGHT SHOULDER AREA. EE WAS STANDING IN A WAY THAT CAUSED HIM TO BE OUT OF POSITION FO R DOING THIS IN A WAY THAT WOULD PREVENT INJURY.
