EMPLOYEE WAS PULLING THEMSELVES UP AND THEIR SHOULDER POPPED RESULTING IN A LEFT SHOULDER STRAIN.

EE WAS TIGHTENING A TURNBUCKLE CONNECTED TO A CABLE ON A CONVEYER BELT ELEVATED 10 FT ABOVE GROUND. THE BOLT ON TURNBUCKLE BROKE, THE FORCE HE WAS APPLYING TO THE TURNBUCKLE PROPELLED HIM OFF THE CONVEYER TO THE GROUND. THE EE LANDED PRIMARLY ON HIS RIGHT ARM AND HIP. RESULTED IN A FRACTURE TO NAVICULAR BONE IN WRIST AND PROXIMAL HEADOF HIS RADIUS. IN CAST ABOUT 12 WEEKS.

WORKER STRUCK THE DOOR WITH THE HANDLE END OF A LARGE CRESCENT WRENCH. AFTER STRIKING THE DOOR, THE WRENCH TRAVELED DOWN THE CHUTE FOLLOWING THE ANGLE. UPON REACHING THE BELT, THE WRENCH HANDL E WAS DRAWN UNDER THE CHAIN AND PULLED INWARD. PINNED HIS HAND AGAINST THE GUARD OVER TAIL PULLEY, IT PINCHED AND PULLED OFF ONE HALF OF THE INDEX FINGER, ALSO A THUMB LACERATION.
