Employee was using a long-handled scraping tool to clean screen plant. While scraping, EE pulled tool back and hit self in the mouth with butt of tool. Chipped two teeth.
Miner was using a pry bar trying to dislodge rock stuck in the tail pulley. The bar came loose striking the miner in the mouth damaging a tooth.

Employee had been operating skidloader and was having back pain. He believes the cause is operating the skid loader extended amounts and the jarring off the ride on rough terrain.
THE EMPLOYEE WAS ATTEMPTING TO CHARGE 1 SET OF BATTERIES BY THE OTHER SET ON THE LOADER. THE CABLE WERE INCORRECTLY CONNECTED CAUSING THE BATTERY TO EXPLODE IN HIS FACE THERE WERE LACERATIONS CONNECTED CAUSING THE BATTERY TO EXPLODE IN HIS FACE. THERE WERE LACERATION IN NOSE & RIGHT EYE. BURNS IN RIGHT EYE AND BROKEN TOOTH.
