EMPLOYEE WAS ASSISTING MECHANIC PUT A ROLLER ON D-10L DOZER. HE WELDED A BOGIE PIN TO THE FRAME TO GET ROLLER UNDER MACHINE WHEN THE WELD BROKE WHILE HE WAS PUTTING THE ROLLER IN PLACE. THE RO LLER BOGIE FELL DOWN, PINCHING HIS RIGHT ARM BETWEEN THE BOGIE AND A ROLLER ON THE GROUND.

EE HAD JUST FINISHED SETTING A WATER PUMP WITH THE HOE AND WAS GETTING DOWN OFF THE HOE WHEN HE SLIPPED AND LAND ON THE TRACKS WHILE CRACKING HIS RIB. HE THOUGHT THE RIB WAS ONLY BRUISED. AFTE R NINE DAYS HE WAS STILL FEELING PAIN AND WENT AND GOT THEM XRAYED ON 11-30-93. HE FOUND OUT AT THAT TIME THE RIB WAS CRACKED & FILED A REPORT.

EMPLOYEE WAS OPERATING LOAD AT DUSK AND WHILE BACKING UP HE BROKE OVER A SIN FOOT LEDGE LEFT FROM A BENCH FROM THE DAY SHIFT. WHEN HE WENT OVER THE LEDGE HE WAS THROUGH AGAINST THE SIDE OF TH E ROPS AND STRAINED THE MUSCLES IN HIS BACK. HE WAS NOT WEARING NO SEAT BELT AT THE TIME OF THE ACCIDENT.

TRUCKER WAS ON TOP OF HIS LOAD SMOOTHING IT OUT AND IUTELLING HIS TARP. FOREMAN NOTICED TRUCKER ACTING DISORIENTED, WENT OVER TO HIM AS HE BECAME UNCONSIOSIOUS. HE STEPPED BREATHING AND HE WAS APPLED CPR AND ARTICFICAL RESPIRATION UNTIL THE E-SQUAD ARRIVIED. HE DIED ABOUT 1-2 HOURS LATER IN THE HOSPITAL.
