While moving the plastic dredge pipe one of the Miners was injured. The pipe was pulled out of the lake to point A. Then we needed to reposition the wheel loader on the other side of the pipe. When he pulled out to move to the other side, the pipe swung wide knocking the miner to the ground and falling onto his legs breaking them both.

Driver for our company, came to me on 9/20/05 and said he had injured his back while decsending a ladder after loading his truck on 9/15/05. He did not report the injury till 9/20/05 around 4:30 pm. I sent him to the doctor right after he told me of his injury, he did not miss any work, as he returned to work the next day. The doctor gave him light duty work.

EE WAS SENT TO THE DOCTOR FOR AN EXAMINATION IMMEDIATELY & DOCTOR HAS RESTRICTED HIS ACTIVITY TO NO KNEELING, SQUATING, BENDING, STOOPING, PUSHING, PULLING OR PICKING UP MORE THAN 10 LBS. THE DOCTOR WILL RE-EVALUATE HIM ON MONDAY (7/18/05).
