Over-exertion (Not Elsewhere Classified)

Employee was tasked with climbing into a screen deck to perform maintenance. When the employee climbed in EE complained that EE hurt shoulder. Employee was taken to the doctor and was treated as a first aid case. On 11/2/2016 the case became reportable due to a second doctor's opinion saying that the employee needed surgery on the shoulder.

Struck against a moving object

Employees were using a Gehl 663 forklift to assist in moving a conveyor belt. During this task, employee rolled over the forklift and caught right leg / foot under the ROPS of the cab.

Contact with heat

Employee was performing maintenance on the primary crusher. The employee was cutting a steel plate with a gouging rod. The employee started to feel ill and progressively got worse. The physician treated the employee for heat stress. The temperature was in the 90's with a heat index of 106 degrees.
