Struck by... (Not Elsewhere Classified)

Employee tried to drive a broom under a low clearance bar, causing the roof of the broom to break loose and strike the employee on the left index finger, and on the head. The left index finger required two stitches.

Over-exertion in lifting objects

Miner was removing aggregate material from a stopped conveyor belt. He was working out of a manlift basket when he felt a burning sensation in his right shoulder. He did not want to be examined at the time. He was taken to a clinic where he was diagnosed with a rotor cuff strain in his right shoulder. He received an injection and was prescribe meds for pain.

Inhalation of radiations, caustics, toxic and noxious substances

Employee / miner has been diagnosed with silicosis. This diagnosis was confirmed today, May 23, 2013, by an occupational specialist. Employee has worked at the mine site since January 11, 1993. Since that time, he has had consistent annual chest exams / x-rays with no significant findings, until this date. He has been released to full duty, effective today.
