Contact with hot objects or substances

Employee was servicing the 6"" Multiquip pump. EE removed the filling cap and pressure from the reservoir caused hot water to escape the cap. EE received burns on the left arm, side, groin and thigh area. EE was transported to Hillcrest Medical Center in Tulsa for treatment.

Fall to the walkway or working surface

The employee had just finished moving a portable generator when they slipped and fell, striking an elbow on the deck of the dredge. This action resulted in the employee sustaining a closed fracture to the left ulna bone near the elbow.

Struck by... (Not Elsewhere Classified)

While replacing ladder cable, injured employee was instructing another employee on how to pull cable off of the drum of the winch. They were at the end of the cable where it attaches to the drum and the injured employee was standing by to secure the component that attaches the cable to the winch. The cable became free and struck the employee on the chin, causing a small laceration.

Fall to the walkway or working surface

Employee fell down on EE's right side and caused a sprain to EE's right wrist. The incident became recordable on 02-15-2022 due to physical therapy. Prior to this date, it has been a first aid only incident (hence the date of accident as 12/01/2021)

Bodily reaction, (Not Elsewhere Classified)

Employee was proceeding down the stairs, at the primary, to clock out for the evening. Employee placed left foot/leg on the first step. When employee went to bear weight on the leg, employee felt a pop in knee. Employee was diagnosed with a torn meniscus and sent the physical therapy beginning 11-02-2021. Employee is working full duty.
